Inhalation beyond respiratory: the surprising applications of dry powder inhalers.

Dry powder inhalers (DPIs) are the newer devices whereby medicine is dispensed as a fine powder inhaled deeply into the lungs, which is more simplified and stable compared to the pressurized metered dose inhalers. DPIs were developed primarily for respiratory disorders such as asthma and chronic obstructive pulmonary disease, but are being investigated for other uses. This review explicates the features of DPIs and how they are made, looking at their effectiveness in respiratory therapy, noting features such as usability, stability of the drugs, and flexibility. It also discusses the potential

Hydrofluoroalkane preparations of fluticasone propionate.

Fluticasone propionate is approved for the long-term maintenance therapy of persistent asthma of all severities, and its safety and efficacy has been well established in clinical trials and practice. With the need to phase out chlorofluorocarbons (CFCs) as propellants in pressurized metered-dose inhalers (pMDIs), hydrofluoroalkane (HFA) propellants have been introduced as a safer, environmentally friendly alternative. A HFA formulation of fluticasone propionate has been developed as a microgram-equivalent replacement for the traditional CFC pMDI. Clinical trials have demonstrated that the flut

Inhaled biologics for respiratory diseases: clinical potential and emerging technologies.

The pulmonary route has gained significant attention as a drug delivery method, particularly for managing respiratory diseases. This approach provides several benefits, such as rapid therapeutic action, minimized systemic exposure, improved patient adherence, and the ability to deliver high drug concentrations directly to the lungs. Advances in inhalation devices, including pressurized metered-dose inhalers (pMDIs), dry powder inhalers (DPIs), and nebulizers, have established the pulmonary route as effective for administering both small-molecule drugs and complex biologics. Recent research has

Dysphonia associated with the use of inhaled corticosteroids.

PURPOSE OF REVIEW: This article discusses the relationship between inhaled corticosteroids and dysphonia, with discussion of the therapeutic use of inhaled steroids in laryngeal disease and a review of negative laryngeal effects of this class of medication in patients with reactive airway disease. RECENT FINDINGS: Although prescribed for their anti-inflammatory effects (predominantly for pulmonary disease and less often for laryngeal conditions), corticosteroid inhalers can cause laryngeal inflammation. This may relate to chemical irritation from the inhaler itself as well as fungal inflammat

Green inhalers: reducing the carbon footprint of asthma care.

PURPOSE OF REVIEW: The respiratory community faces an urgent need to reduce the environmental impact of care as the wider climate crisis threatens to worsen airways disease worldwide. Inhalers contribute a disproportionate share of healthcare emissions because of the hydrofluorocarbon (HFC) propellants in pressurized metered-dose inhalers (pMDIs). We already have effective, low-carbon, per- and polyfluoroalkyl substances (non-PFAS) options; particularly dry-powder inhalers (DPIs). This review summarizes recent developments in propellant technology and evidence on optimizing asthma care to impr

The effect of nedocromil sodium on nasal provocation with allergen.

We have investigated the effect of nedocromil sodium in preventing the symptoms of rhinitis occurring immediately after allergen provocation in 10 patients with allergic rhinitis. The study had a double-blind, placebo-controlled, crossover design. Nedocromil sodium (1% w/v) and placebo were administered intranasally from identical metered-dose inhalers 20 minutes before allergen provocation. Three variables were measured: nasal airway resistance, the production of secretion, and the number of sneezes. Nedocromil sodium was significantly more effective than placebo in preventing nasal obstructi

The president speaks: prevention is best: lessons from protecting the ozone layer.

The Montreal Protocol was signed 25 years ago. As a result, the irreversible destruction of the ozone layer was prevented. However, stratospheric ozone will not recover completely until 2060 and the consequent epidemic in skin cancer cases will persist until 2100. Many millions of patients with asthma and chronic obstructive pulmonary disease have safely switched from chlorofluorocarbon (CFC)-powered metered-dose inhalers (MDIs) to either hydrofluorocarbon (HFC) or DPIs. China will be the last country to phase out CFCs by 2016. HFCs are global warming gases which will be controlled in the near

Thoughtful prescription of inhaled medication has the potential to reduce inhaler-related greenhouse gas emissions by 85.

INTRODUCTION: Both physicians and patients are increasingly aware of the environmental impacts of medication. The shift of treatment paradigm towards MART-treatment (Maintenance and Reliever Therapy) in asthma affects the treatment-related emissions. The carbon footprint of inhaled medication is also tied to the type of the device used. Today the most commonly used propellant-containing pressurised metered-dose inhalers (pMDIs) have a carbon footprint typically 20-40-fold higher than propellant-free dry powder inhalers (DPIs) and soft mist inhalers. METHODS: We analysed the carbon footprint o

Consensus Recommendations on Home Maintenance Nebulization with Focus on Obstructive Airway Diseases: An Update on Best Practices and Future Directions.

Asthma and chronic obstructive pulmonary disease (COPD) are obstructive airway diseases (OADs) that contribute significantly to the burden on healthcare systems in low- and middle-income countries like India.1,2 Inhaler therapy (primarily involving pressurized metered-dose inhalers and dry powder inhalers) remains the cornerstone of maintenance therapy; however, not all patients can use them effectively.3 Home maintenance nebulization (HMN) has been increasingly used as an alternative in appropriately identified patients, particularly those with advanced disease, physical or cognitive limitati

A novel device for aerosol plume evaluation of orally inhaled and nasal drug products.

Accurate measurement of aerosol velocity is important for evaluating the performance of orally inhaled and nasal drug products (OINDPs). This study aims to demonstrate and evaluate the accuracy and reliability of a novel instrument, the "OptoSleeve," which integrates a compact optical diagnostic system designed to measure aerosol velocity and plume duration across various aerosol drug emitting devices. The OptoSleeve employs the laser extinction concept to capture aerosol flow behaviour. Experiments in this study were performed using nasal sprays, pressurised metered dose inhalers (pMDIs), and

Awareness of environmental issues and the acceptance of CFC-free inhalers.

With the recent availability of a chlorofluorocarbon (CFC)-free metered dose inhaler (MDI) (Airomir), a patient survey was carried out to evaluate awareness of the role of CFCs in our environment and acceptance of this new inhaler. A questionnaire survey was conducted on parents and guardians of 201 children. Depending on respondents' preference, the interview was conducted in English (71%), Chinese (23%), Malay (5%) or Tamil (1%). A 'taste' test was also conducted on 103 of these children. Only 13% (26/201) of parents/guardians were aware that MDIs contained CFCs. Although 70% of children wer

The inhalers of the future? A review of dry powder devices on the market today.

International agreements to ban the use of environmentally damaging chlorofluorocarbons (CFCs) have signalled an end to the traditional CFC-propelled pressurised metered dose inhaler (pMDI) which has long been the mainstay of topical asthma therapy. The need for acceptable and cost-effective replacement inhalers, combined with opportunities to develop generic formulations of patent-expired drugs, has fueled a lively response from the pharmaceutical industry. Improvements in pMDI design and reformulation with propellants such as hydrofluoroalkanes may offer significant advantages over CFC-pMDIs

A systematic review of nicotine by inhalation: is there a role for the inhaled route?

INTRODUCTION: A considerable minority of adults remain addicted to smoking cigarettes despite substantial education and public health efforts. Nicotine replacement therapies have only modest long-term quit rates. The pulmonary route of nicotine delivery has advantages over other routes. However, there are regulatory and technical barriers to the development of pulmonary nicotine delivery devices, and hence, none are commercially available. Current knowledge about pulmonary nicotine delivery is scattered throughout the literature and other sources such as patent applications. This review draws

[Inhalotherapy: Recommendations for Argentina 2021].

Last decades, a broad spectrum of inhaled devices (ID) had been developed to enhance efficacy and reduce adverse events. The correct use of IDs is a critical issue for controlling obstructive respiratory diseases. There is no recommendation on inhalation therapy in Argentina. This document aims to issue local recommendations about the prescription of IDs. Each device was reviewed regarding biophysical laws, indication, strength, limitations, correct technique of use, frequent mistakes, and device cleaning and maintenance. Nebulization should be restricted to drugs that are not available in oth

Prescription patterns of inhaler medications from 2017 to 2023: A retrospective study using Ontario administrative healthcare data.

BACKGROUND: Inhalers are essential for respiratory disease management, but commonly used metered-dose inhalers (MDIs) can often be replaced, when clinically appropriate, with lower-emission alternatives such as dry powder inhalers (DPIs) or soft mist inhalers (SMIs). The COVID-19 pandemic may have influenced prescribing patterns. We aim to describe changes in inhaler prescription trends and determine whether MDI use decreased during the pandemic. METHODS AND FINDINGS: Retrospective cohort study using Ontario's health administrative data. Individuals aged 65 and older with new inhaler prescrip

Environmental impact of inhaled medicines: A Thoracic Society of Australia and New Zealand position statement.

Globally, more than 1.2 billion inhalers are purchased for asthma and chronic obstructive pulmonary disease (COPD) annually. In Australia and New Zealand, pressurized metered dose inhalers (pMDIs) are the leading delivery device prescribed and pMDI salbutamol can be purchased over the counter in Australia. These inhalers are a major contributor to healthcare related greenhouse gases. This is due to the propellants that they currently contain which have extremely high global warming potential (GWP). In this position paper, we report the findings of a Thoracic Society of Australia and New Zealan

Aerosol drug delivery: developments in device design and clinical use.

Aerosolised drugs are prescribed for use in a range of inhaler devices and systems. Delivering drugs by inhalation requires a formulation that can be successfully aerosolised and a delivery system that produces a useful aerosol of the drug; the particles or droplets need to be of sufficient size and mass to be carried to the distal lung or deposited on proximal airways to give rise to a therapeutic effect. Patients and caregivers must use and maintain these aerosol drug delivery devices correctly. In recent years, several technical innovations have led to aerosol drug delivery devices with eff

Effect of formulation parameters on hydrofluoroalkane-beclomethasone dipropionate drug deposition in humans.

Chlorofluorocarbon metered dose inhalers (MDIs) and dry powder inhalers currently deliver drug that deposits primarily in the oropharynx and secondarily in the large central airways. Chlorofluorocarbon-beclomethasone dipropionate (CFC-BDP) MDIs deliver more than 90% of the drug in the oropharynx and less than 10% in the lungs. The elimination of chlorofluorocarbons from MDIs provided the opportunity to more optimally target corticosteroids directly to all inflammatory sites. Hydrofluoroalkane-BDP (HFA-BDP) MDIs (QVAR(trade mark)) deliver 50% to 60% of the drug to the lungs with approximately 3
