Associations of Military Service History and Health Outcomes in the First Five Years After Traumatic Brain Injury.

For many years, experts have recognized the importance of studying traumatic brain injury (TBI) among active-duty service members and veterans. A majority of this research has been conducted in Veterans Administration (VA) or Department of Defense settings. However, far less is known about military personnel who seek their medical care outside these settings. Studies that have been conducted in civilian settings have either not enrolled active duty or veteran participants, or failed to measure military history, precluding study of TBI outcomes by military history. The purpose of the present st

Exploring factors associated with posttraumatic stress disorder symptoms among deployed military healthcare personnel.

Little is known about what factors contribute to posttraumatic stress disorder among military healthcare personnel during a deployment. Accordingly, the purpose of this study was to explore the association among demographic variables, psychological and occupational stressors, and protective resources with probable PTSD caseness and PTSD symptom severity in a sample of 1161 military healthcare personnel who were deployed to a combat hospital in Balad, Iraq, between 2004 and 2011. In this cross-sectional observational study, participants completed anonymous self-report measures while in theater.

Insomnia and sleep apnea in the entire population of US Army soldiers: Associations with deployment and combat exposure 2010-2019, a retrospective cohort investigation.

OBJECTIVES: This retrospective cohort study examined clinically diagnosed insomnia and sleep apnea and analyzed associations with deployment and combat exposure in active-duty soldiers (n=1,228,346) from 2010 to 2019. METHODS: Retrospective data were obtained from the Soldier Performance, Health, and Readiness database. RESULTS: Overseas soldier deployments peaked in 2010, decreasing thereafter as soldiers were withdrawn from Iraq and Afghanistan. From 2010 to 2012 insomnia incidence increased at a rate of 6.7 cases/1000 soldier-years, then decreased after 2012 at 5.3 cases/1000 soldier-year

Connecting active duty and returning veterans to mental health treatment: interventions and treatment adaptations that may reduce barriers to care.

Recent military operations in Afghanistan and Iraq have involved multiple deployments and significant combat exposure, resulting in high rates of mental health problems. However, rates of treatment-seeking among military personnel are relatively low, and the military environment poses several obstacles to engaging in effective clinical interventions. The current paper first reviews barriers and facilitators of treatment-seeking and engagement among military personnel, including stigma, practical barriers, perceptions of mental health problems, and attitudes towards treatment. Next, this paper

Predictors of emotional distress reported by soldiers in the combat zone.

OBJECTIVE: Few studies have examined rates of distress of military personnel during deployment to a war zone. Our study sought to (a) identify rates of self-reported posttraumatic stress disorder (PTSD) and depression symptoms during combat deployment, (b) characterize higher order dimensions of emotional distress experienced by soldiers during deployment, and (c) identify predictors of these dimensions of emotional distress. METHOD: Participants were 2677 National Guard soldiers deployed as part of Operation Iraqi Freedom in 2006-07. We performed a principal components factor analysis on ite

Malaria as a military factor in Southeast Asia.

Malaria remains a major cause of military casualties in Southeast Asia. The numerous conflicts in the region have been greatly affected by malaria. Besides guerrilla warfare, refugees and other civilian movements across international borders contribute heavily to the continuing problem of multiple drug-resistant malaria. Drug resistance is an increasing problem with few available prophylactic options. The malaria threat to any potential deployment of United States military forces remains unsolved.

Military and occupational exposures among Veterans in the Million Veteran Program by survey self-report: a descriptive study.

OBJECTIVE: We aimed to characterise self-reported military and occupational exposures including Agent Orange, chemical/biological warfare agents, solvents, fuels, pesticides, metals and burn pits among Veterans in the Department of Veterans Affairs Million Veteran Program (MVP). METHODS: MVP is an ongoing longitudinal cohort and mega-biobank of over one million US Veterans. Over 500 000 MVP participants reported military exposures on the baseline survey, and over 300 000 reported occupational exposures on the lifestyle survey. We determined frequencies of selected self-reported occupational e

Advanced Surgical Skills for Exposure in Trauma (ASSET) course improves military surgeon confidence.

OBJECTIVE: Active duty military surgeons often have limited trauma surgery experience prior to deployment. Consequently, military-civilian training programs have been developed at high-volume trauma centers to evaluate and maintain proficiencies. Advanced Surgical Skills for Exposure in Trauma (ASSET) was incorporated into the predeployment curriculum at the Army Trauma Training Detachment in 2011. This is the first study to assess whether military surgeons demonstrated improved knowledge and increased confidence after taking ASSET. DESIGN: Retrospective cohort study. SETTING: Quaternary car

Constrictive Bronchiolitis Attributable to Inhalation of Toxic Agents: Considerations for a Case Definition.

OBJECTIVE: We identified cases of constrictive bronchiolitis (CB), an inflammatory injury obliterating the small airways, in adults caused by inhalational exposure to determine an appropriate case definition. METHODS: We performed a systematic review with meta-analysis for these cases from 1990 to 2017. Publications were included if there was 1) inhalational exposure; 2) respiratory symptoms/signs; 3) pulmonary function test results; and 4) computerized tomographic chest imaging. Many had a lung biopsy. RESULTS: Two hundred seventy-four articles were retrieved; 22 manuscripts comprising 102

Lesions Arising in a Tattoo of an Active Duty US Marine Corps Woman.

Tattoos are ubiquitous in modern society; however, they do not come without risk of medical complications. When complications arise in the military community, a particularly thorough differential diagnosis should be considered based on the increased exposures service members have during deployment and throughout their military career. We present a case of a 38-year-old active duty US Marine Corps woman with worsening skin lesions arising within a tattoo 6 weeks after acquiring the tattoo on her right chest. Given environmental exposures from a recent deployment to the Middle East, a wide diffe

Prevalence and Nature of Traumatic Brain Injury During Army Pre-Deployment Training.

INTRODUCTION: Active duty and reserve service members (SMs) are at increased risk of sustaining a traumatic brain injury (TBI). Given the high frequency of TBIs during recent military operations, much is known about combat-related TBI. Nevertheless, and notably, most recorded TBIs occur in non-deployed settings, where Soldiers regularly engage in operational training activities that mimic combat and participate in routine activities that carry TBI risk. There is sparse literature examining the risk of TBI during pre-deployment training independently from other non-combat settings. This study a

Using Military Working Dog Records to Assist With Exposure-Informed Care.

INTRODUCTION: Military working dogs (MWDs) are deployed to and work in the same environments as Service members and, therefore, are exposed to the same hazards. To better understand the potential consequences of military-related exposures on health, the Exposure Science Program, Department of Veterans Affairs partnered with Army Veterinary Services and the Joint Pathology Center to create the Deceased MWD Database (DMWDD). This database includes all MWDs that had completed veterinary records from January 1991 onward, with demographics, deployment information, and pathology findings of 2,922 MW

Postdeployment Plasmodium Infections among South Korean Soldiers Receiving Mefloquine Chemoprophylaxis in South Sudan.

Postdeployment Plasmodium infection continues to occur despite recommended chemoprophylaxis among military personnel and long-term travelers deployed to malaria-endemic regions. We evaluated factors associated with postdeployment Plasmodium infection among South Korean soldiers receiving weekly mefloquine chemoprophylaxis during prolonged deployment to South Sudan. We conducted a retrospective study of 42 soldiers from a military unit who completed a standardized 40-week deployment to South Sudan between 2020 and 2023. All personnel were prescribed weekly mefloquine throughout deployment and 4

Prevalence, incidence and determinants of PTSD and other mental disorders: design and methods of the PID-PTSD+3 study.

Investigation of the prevalence, incidence, and determinants of post-traumatic stress disorders (PTSD) and other mental disorders associated with military deployment in international missions poses several methodological and procedural challenges. This paper describes the design and sampling strategies, instruments, and experimental procedures applied in a study programme aimed to examine military deployment-related mental health and disorders (prevalence and trajectories) and to identify vulnerability and risk factors (e.g. age, gender, type of mission, rank, and duration of deployment and a

Psychiatric profiles in the U.S. Air Force: a clinical interpretation of Air Force Instruction 48-123.

The foundations of our current system for profiling military psychiatric patients were laid during World War II, well before the development of the first version of the Diagnostic and Statistical Manual of Mental Disorders. The general principles and terminology remain in use today through Air Force Instruction 48-123, Medical Examination and Standards. The terminology used is clearly outdated, making it difficult to use and risking misuse, deploying the wrong person or denying deployment to an appropriate person. Our objective is to review the current standards for making psychiatric profiles

Overview of depression, post-traumatic stress disorder, and alcohol misuse among active duty service members returning from Iraq and Afghanistan, self-report and diagnosis.

Previous studies have found deployment to combat areas to be associated with an increased risk of post-traumatic stress disorder (PTSD), depression, and alcohol abuse, but many previous studies were limited by samples that were not representative of the deployed military as a whole. This study presents an overview of these three mental health problems associated with deployment among Air Force, Army, Marine Corp, and Navy service members returning from deployment to Iraq and Afghanistan between January 2007 and March 2008. With postdeployment health data on over 50,000 service men and women, i

US Department of Defense consensus guidelines on neuromodulation for pain management in an active-duty military population from a tri-service, multispecialty working group.

Military pain medicine physicians often treat Service Members with chronic pain with implantable neural stimulators in the spine and near peripheral nerves. These procedures are performed to improve readiness, restore function and deployability; however, there are no consensus practice guidelines on the use of these devices, which has led to inequities in their application and the disposition of patients who receive them.In January 2025, the Defense Health Agency Pain Management Clinical Support Service validated the need for consensus practice guidelines and endorsed their creation. Psychiatric Aeromedical Evacuations of Deployed Active Duty U.S. Military Personnel During Operations Enduring Freedom, Iraqi Freedom, and New Dawn.

INTRODUCTION: The primary objective of this study was to describe the demographic, clinical, and attrition characteristics of active duty U.S. military service members who were aeromedically evacuated from Iraq and Afghanistan theaters with a psychiatric condition as the primary diagnosis. The study links the U.S. Transportation Command Regulating and Command and Control Evacuation System (TRAC2ES) data with the Defense Manpower Data Center (DMDC) to conduct an examination of the long-term occupational impact of psychiatric aeromedical evacuations on military separations and discharges. Anxiety and depression in Marines sent to war in Iraq and Afghanistan.

Although the effects of combat deployment on posttraumatic stress disorder have been extensively studied, little is known about the effects of combat deployment on depression and anxiety. This study examined the factors associated with anxiety and depression in a sample of 1560 US Marines who were deployed to Iraq and Afghanistan. Eleven demographic and psychosocial factors were studied in relation to depression and anxiety. Five factors emerged as significant in relation to depression: deployment-related stressors, combat exposure, attitudes toward leadership, mild traumatic brain injury symp
