Amputees in a hospital of the International Committee of the Red Cross.

The object of this study is to describe the treatment of patients with an amputation in terms of causes and distribution of injuries, mortality and drain on surgical resources. A group of patients with war injuries, who did not undergo an amputation (1796) and a group of war-injured patients with an amputation (245) were analysed retrospectively. All these patients were treated in the hospital for Afghan war wounded of the International Committee of the Red Cross. The most common amputation was below-knee amputation (BKA). In more than 90 per cent of the patients a BKA was the result of a mine

Leriche syndrome diagnosed due to polytrauma: a case report.

BACKGROUND: Leriche syndrome is caused by atherosclerosis and is often characterized by symptoms such as intermittent claudication and numbness and coldness of the lower limbs. Its exact prevalence and incidence are unknown because it is a rare disease. We report a case of Leriche syndrome diagnosed incidentally on trauma pan-scan computed tomography (CT). CASE PRESENTATION: A 61-year-old Asian male was driving a passenger car and had a head-on collision with a dump truck that required an emergency call. The patient was transported to our hospital in a doctor's helicopter. Physical examinatio

Assessment of Vacuum-Assisted Closure (VAC) Therapy in Orthopaedic Infections.

Introduction Chronic and infected orthopaedic wounds may result in profound morbidity, amputation, sepsis and even death. It may need prolonged hospitalization and multiple surgical procedures for treatment. Vacuum-assisted dressing (VAD) is a comparatively newer modality for treating chronic non-healing wounds which helps in faster wound healing, decreases the frequency of dressing and reduces hospitalization time. The aim of our study is to evaluate the outcome of vacuum-assisted dressing (VAD) in the management of orthopaedic wounds. Materials and methods A case series including 20 patients

Care of the Dying Patient: Maximizing Compassionate Care on the Battlefield.

Palliative care is a philosophy of treatment that focuses on relieving suffering while aligning care with a patient's goals. This treatment modality has proven benefits for civilian patients of all ages with both traumatic injuries and non-traumatic disease states. It results in the reduction of distressing symptoms and improved patient satisfaction, as well as reducing painful, ultimately futile interventions. Palliative care has also been employed throughout history on the battlefield and is a vital component of compassionate care for dying patients. Unfortunately, its use on the battlefield

Hypothenar hammer syndrome: proposed etiology.

PURPOSE: Finger ischemia caused by embolic occlusion of digital arteries originating from the palmar ulnar artery in a person repetitively striking objects with the heel of the hand has been termed hypothenar hammer syndrome (HHS). Previous reports have attributed the arterial pathology to traumatic injury to normal vessels. A large experience leads us to hypothesize that HHS results from trauma to intrinsically abnormal arteries. METHODS: We reviewed the arteriography, histology, and clinical outcome of all patients treated for HHS in a university clinical research center study of hand ische

[Indications and incidence of spongiosa transplantation in open fractures: analysis of 470 open fractures].

From a prospective series of 651 open fractures, treated at the institution of the authors in the years 1984-1989, 124 cancellous bone grafts in 470 open long bone fractures--651 open fractures excluding amputation after Type III open fractures, excluding traumatic amputations and open fractures of the ankle joint and foot--were analyzed using approximately 155 variables per individual fracture to deduct the influencing factors for indication and frequency of bone grafts. As a result of this analysis it could be shown that bone grafting depends significantly on variables describing the bone in

Active Blood Cell Capture and Synergistic Coassembly: Self-Gelling Gelatin-Polysaccharide Powders for Rapid Hemostasis.

While effective for irregular wounds, traditional hemostatic powders often lack posthydration cross-linking, yielding fragile barriers susceptible to washout under high-pressure hemorrhage. To address this, a novel self-gelling composite powder (GOQL) was developed by incorporating dopamine-grafted gelatin (GelDA), oxidized sodium alginate (OSA), and lauric acid-grafted quaternized chitosan (QCS-LA). Upon contact with bleeding sites, the porous powder swiftly absorbs interfacial fluids, triggering rapid in situ self-gelation through Schiff-base reactions and physical interactions, followed by

How To Improve Patient Selection in Individuals With Lower Extremity Amputation Using a Bone-anchored Prosthesis.

BACKGROUND: Many patients with a lower limb socket-suspended prothesis experience socket-related problems, such as pain, chronic skin conditions, and mechanical problems, and as a result, health-related quality of life (HRQoL) is often negatively affected. A bone-anchored prosthesis can overcome these problems and improve HRQoL, but these prostheses have potential downsides as well. A valid and reliable tool to assess potential candidates for surgery concerning a favorable risk-benefit ratio between potential complications related to bone-anchored prostheses and improvements in HRQoL is not av

Single-Stage Press-Fit Osseointegration of the Radius and Ulna for Rehabilitation After Trans-Forearm Amputation.

BACKGROUND: Upper limb (UL) amputation is disabling. ULs are necessary for many domains of life1, and few effective motor and sensory replacements are accessible2. Approximately 41,000 people in the United States have UL amputation proximal to the fingers3, two-thirds of (all) traumatic amputations are UL4, and 80% of UL amputations are performed for trauma-related etiologies5. Socket prosthesis (SP) abandonment remains high because of the lack of sensation, limited prosthesis control, perceived weight, and difficulty comfortably wearing the SP6. Ectopic implantation of an avulsed scalp with a tissue expander on a forearm for combined total scalp avulsion and spine injuries: A case report.

Total scalp avulsion with severe cervical spine injury is a contraindication for emergency replantation of the scalp to its anatomical site. We describe a case involving the ectopic implantation of an avulsed scalp on the forearm. A 41-year-old woman presented with severe total scalp avulsion and tears in the intervertebral discs at the C4/5 and C5/6 levels. The avulsed scalp was ectopically implanted on the left forearm with a tissue expander to provide support. Two-stage replantation of the scalp at its anatomical site was performed 19 and 40 days later. Replantation was successful, and the

Ectopic banking of amputated great toe for delayed thumb reconstruction: case report.

Ectopic banking of amputated parts is a recognized technique for delayed replantation of an amputated part when the amputation stump will not permit immediate replantation. This is conventionally performed with the intent of transferring the injured part back to its anatomic position when the amputation stump is more appropriate for replantation. Current warfare conditions have led to a commonly encountered military trauma injury pattern of multiple extremity amputations with protected trunk and core structures. This pattern poses many challenges, including the limit or absence of donor sites

[Vacuum assisted closure (V.A.C.) therapy is an essential tool for treatment of complex defect injuries of the upper extremity].

INTRODUCTION: Complex injuries of the upper extremity remain a therapeutic challenge. The prognosis of the damaged extremity is often limited by soft tissue defects. Vacuum Assisted Closure (V.A.C.) therapy facilitates temporary coverage of soft tissue defects prior to surgical reconstruction. METHODS: In a retrospective study all patients with complex defect injuries of the upper extremity that were treated by V.A.C. prior to reconstruction between August 2003 and September 2005 were analyzed. RESULTS: 7 patients (6 male, 1 female, 14-70 years) were included in the study. The patients suffe

[Treatment of fractures of long bones associated with extensive skin avulsion].

The work analyses the experience in the treatment of 46 patients with fractures of long tubular bones and extensive wounds with avulsion of the skin at the department of emergency traumatology from 1983 to 1988. The method of choice in the treatment of fractures of the long tubular bones in this category of patients is functionally-stable osteosynthesis mainly with external fixation apparatuses on a nail or rod basis. A rational succession of the manipulations for surgical debridement of the extensive wound was determined, which made the surgeon's work much easier and reduced the time needed f

Management of severe crush injury in a front-line tent ICU after 2008 Wenchuan earthquake in China: an experience with 32 cases.

INTRODUCTION: The experience on management of crush injury after a devastating earthquake is lacking, and there are even less reports on the front-line critical care of these patients. A front-line intensive care unit (ICU) was set up in a tent after the disastrous Wenchuan earthquake (May, 12, 2008, China), where 32 patients suffering from crush injury were treated from May 12 to May 26. This study summarized our experience on management of 32 crush injury patients in a front-line tent ICU. Use of an extracorporeal membrane oxygenation circuit as a bridge to salvage a major upper-extremity replant in a critically ill patient.

Major replantation of the upper extremity is defined as replantation at or above the level of the wrist. Selection of appropriate candidates is complex and requires consideration of many patient- and injury-associated factors including patient age, associated injuries, patient desire, mechanism of injury, ischemia time, wound condition, and presence of multiple-level injury. With respect to age, younger patients, especially children, are deemed to have a distinct advantage over more elderly patients due to improved nerve regeneration, and many advocate making every effort to replant this popul
