PURPOSE: To study the efficacy of reirradiation as salvage treatment in patients with locally recurrent nasopharyngeal carcinoma.
PATIENTS AND METHODS: Between 1993 and 2000, 20 consecutive patients (twelve males and eight females) with nasopharyngeal cancer, previously irradiated in different Hungarian institutions, were reirradiated for biopsy-proven locally recurrent tumor. Histologically, 85% of the patients had WHO type III, 5% type II, and 10% type I disease. Permeative infiltration of the meninges appears to be a distinct form of recurrent nasopharyngeal carcinoma (NPC). We present the case of a 34-year-old pregnant woman who had an elevated maternal serum alpha-fetoprotein level and sonographic findings of a semisolid mass protruding from the fetus's oral cavity. The large, heterogeneous mass filled the oropharynx and nasopharynx. Abnormal Doppler waveforms were detected in the umbilical artery of the fetus, who died in utero. Postmortem examination revealed a nasopharyngeal teratoma.

We report a case of myiasis caused by Phaenicia sericata during mechanical ventilation. An 86-year-old woman with bronchiectasis was admitted to our hospital with severe respiratory failure. In order to investigate the p16 gene alterations in nasopharyngeal carcinoma (NPC), we have examined for mutations and deletions of the p16 gene in samples of NPC including 3 cell lines, 3 xenografts, and 20 primary tumors with matched blood DNA as controls. Using single-strand conformation polymorphism and direct sequencing analysis, no p16 gene mutations were detected in the NPC primary tumors and xenografts. We report a case of nasopharyngeal carcinoma (NPC) accompanied by hyponatremia and polyuria which was induced by tumor invasion into the pituitary gland. Magnetic resonance imaging showed the pituitary gland pushed upward by NPC. Hypopituitarism due to an intracranial tumor and the extracellular volume excess due to chemotherapy caused hyponatremia and polyuria. As the intracranial tumor was diminished by chemotherapy and irradiation, pituitary hormones were normalized. In patients with T4 NPC, pituitary-adrenocortical function should be endocrinologically evaluated before treatment.

We describe a 42-year-old Chinese patient with polymyositis and nasopharyngeal carcinoma. The polymyositis was complicated by respiratory failure needing mechanical ventilation despite conventional therapy. Treatment of the carcinoma was associated with recovery of muscle strength. To our knowledge this is the first reported association of polymyositis with nasopharyngeal carcinoma.
