Automated Matching of Patients to Clinical Trials: A Patient-Centric Natural Language Processing Approach for Pediatric Leukemia.

PURPOSE: Matching patients to clinical trials is cumbersome and costly. Attempts have been made to automate the matching process; however, most have used a trial-centric approach, which focuses on a single trial. In this study, we developed a patient-centric matching tool that matches patient-specific demographic and clinical information with free-text clinical trial inclusion and exclusion criteria extracted using natural language processing to return a list of relevant clinical trials ordered by the patient's likelihood of eligibility. MATERIALS AND METHODS: Records from pediatric leukemia

Natural Language Processing for Information Extraction of Gastric Diseases and Its Application in Large-Scale Clinical Research.

Background and Aims: The utility of clinical information from esophagogastroduodenoscopy (EGD) reports has been limited because of its unstructured narrative format. We developed a natural language processing (NLP) pipeline that automatically extracts information about gastric diseases from unstructured EGD reports and demonstrated its applicability in clinical research. Methods: An NLP pipeline was developed using 2000 EGD and associated pathology reports that were retrieved from a single healthcare center. The pipeline extracted clinical information, including the presence, location, and siz

Exploring the Applicability of Using Natural Language Processing to Support Nationwide Venous Thromboembolism Surveillance: Model Evaluation Study.

BACKGROUND: Venous thromboembolism (VTE) is a preventable, common vascular disease that has been estimated to affect up to 900,000 people per year. It has been associated with risk factors such as recent surgery, cancer, and hospitalization. VTE surveillance for patient management and safety can be improved via natural language processing (NLP). NLP tools have the ability to access electronic medical records, identify patients that meet the VTE case definition, and subsequently enter the relevant information into a database for hospital review. OBJECTIVE: We aimed to evaluate the performance

Natural language processing and network analysis in patients withdrawing from life-sustaining treatments: a retrospective cohort study.

BACKGROUND: Providing palliative care to patients who withdraw from life-sustaining treatments is crucial; however, delays or the absence of such services are prevalent. This study used natural language processing and network analysis to identify the role of medications as early palliative care referral triggers. METHODS: We conducted a retrospective observational study of 119 adult patients receiving specialized palliative care after endotracheal tube withdrawal in intensive care units of a Taiwan-based medical center between July 2016 and June 2018. Patients were categorized into early inte

Prediction and evaluation of combination pharmacotherapy using natural language processing, machine learning and patient electronic health records.

Combination pharmacotherapy targets key disease pathways in a synergistic or additive manner and has high potential in treating complex diseases. Computational methods have been developed to identifying combination pharmacotherapy by analyzing large amounts of biomedical data. Existing computational approaches are often underpowered due to their reliance on our limited understanding of disease mechanisms. On the other hand, observable phenotypic inter-relationships among thousands of diseases often reflect their underlying shared genetic and molecular underpinnings, therefore can offer unique

Automated tracking of emergency department abdominal CT findings during the COVID-19 pandemic using natural language processing.

PURPOSE: During the COVID-19 pandemic, emergency department (ED) volumes have fluctuated. We hypothesized that natural language processing (NLP) models could quantify changes in detection of acute abdominal pathology (acute appendicitis (AA), acute diverticulitis (AD), or bowel obstruction (BO)) on CT reports. METHODS: This retrospective study included 22,182 radiology reports from CT abdomen/pelvis studies performed at an urban ED between January 1, 2018 to August 14, 2020. Using a subset of 2448 manually annotated reports, we trained random forest NLP models to classify the presence of AA,

Natural language processing of prehospital emergency medical services trauma records allows for automated characterization of treatment appropriateness.

BACKGROUND: Incomplete prehospital trauma care is a significant contributor to preventable deaths. Current databases lack timelines easily constructible of clinical events. Temporal associations and procedural indications are critical to characterize treatment appropriateness. Natural language processing (NLP) methods present a novel approach to bridge this gap. We sought to evaluate the efficacy of a novel and automated NLP pipeline to determine treatment appropriateness from a sample of prehospital EMS motor vehicle crash records. METHODS: A total of 142 records were used to extract airway

Speech Analysis by Natural Language Processing Techniques: A Possible Tool for Very Early Detection of Cognitive Decline?

Background: The discovery of early, non-invasive biomarkers for the identification of "preclinical" or "pre-symptomatic" Alzheimer's disease and other dementias is a key issue in the field, especially for research purposes, the design of preventive clinical trials, and drafting population-based health care policies. Complex behaviors are natural candidates for this. In particular, recent studies have suggested that speech alterations might be one of the earliest signs of cognitive decline, frequently noticeable years before other cognitive deficits become apparent. Traditional neuropsychologic

Alignment of natural language processing-derived RDoC scores with depression, anhedonia, and anxiety in psychiatric electronic health records.

Traditional categorical diagnoses often fail to capture the dimensional nature of psychiatric disorders such as depression and anxiety. Natural Language Processing (NLP) offers a promising approach to extract dimensional constructs relevant to psychopathology from clinical documentation. In this cross-sectional study, we analyzed clinical notes from 358 psychiatric patients treated at McLean Hospital and enrolled in the "Psychometrics at McLean" (PAM) study to examine the alignment between NLP-derived Research Domain Criteria (RDoC) constructs and psychometric assessments of depression, anhedo

Signal detection of adverse events in medical devices using natural language processing: a case study in pelvic mesh.

Disproportionality analysis is used to detect safety signals for post-market surveillance from adverse events reported to regulatory bodies but is challenging when reports contain unstructured free-text. We implemented a proof-of-concept system combining natural language processing of free-text data with disproportionality analysis, using a known safety signal from pelvic mesh. Free-text reports in an Australian spontaneous adverse event report database between 2012 and 2017 were classified using topic modelling. 'Pain' was the most frequent clinical topic and was a known adverse event from pe

Using natural language processing to extract carotid stenosis severity from clinical notes to create a nationwide veteran cohort.

OBJECTIVE: The prevalence of moderate to severe asymptomatic carotid stenosis (ie, atherosclerotic narrowing of the extracranial carotid arteries) is generally approximately 6% and 2%, respectively. Most prior studies of carotid stenosis risk factors have been small. This study describes the development and validation of a natural language processing (NLP) tool to identify carotid stenosis and uses it to identify significant risk factors, presence, and severity of carotid stenosis. METHODS: We created an NLP tool to extract the ratio of peak systolic velocity of the internal carotid artery to

Machine learning and natural language processing to assess the emotional impact of influencers' mental health content on Instagram.

BACKGROUND: This study aims to examine, through artificial intelligence, specifically machine learning, the emotional impact generated by disclosures about mental health on social media. In contrast to previous research, which primarily focused on identifying psychopathologies, our study investigates the emotional response to mental health-related content on Instagram, particularly content created by influencers/celebrities. This platform, especially favored by the youth, is the stage where these influencers exert significant social impact, and where their analysis holds strong relevance. Anal

Natural language processing diagnosed behavioural disturbance phenotypes in the intensive care unit: characteristics, prevalence, trajectory, treatment, and outcomes.

BACKGROUND: Natural language processing (NLP) may help evaluate the characteristics, prevalence, trajectory, treatment, and outcomes of behavioural disturbance phenotypes in critically ill patients. METHODS: We obtained electronic clinical notes, demographic information, outcomes, and treatment data from three medical-surgical ICUs. Using NLP, we screened for behavioural disturbance phenotypes based on words suggestive of an agitated state, a non-agitated state, or a combination of both. RESULTS: We studied 2931 patients. Of these, 225 (7.7%) were NLP-Dx-BD positive for the agitated phenotyp

Using natural language processing to automatically classify written self-reported narratives by patients with migraine or cluster headache.

BACKGROUND: Headache medicine is largely based on detailed history taking by physicians analysing patients' descriptions of headache. Natural language processing (NLP) structures and processes linguistic data into quantifiable units. In this study, we apply these digital techniques on self-reported narratives by patients with headache disorders to research the potential of analysing and automatically classifying human-generated text and information extraction in clinical contexts. METHODS: A prospective cross-sectional clinical trial collected self-reported narratives on headache disorders fr

Natural Language Processing and Machine Learning for Identifying Incident Stroke From Electronic Health Records: Algorithm Development and Validation.

BACKGROUND: Stroke is an important clinical outcome in cardiovascular research. However, the ascertainment of incident stroke is typically accomplished via time-consuming manual chart abstraction. Current phenotyping efforts using electronic health records for stroke focus on case ascertainment rather than incident disease, which requires knowledge of the temporal sequence of events. OBJECTIVE: The aim of this study was to develop a machine learning-based phenotyping algorithm for incident stroke ascertainment based on diagnosis codes, procedure codes, and clinical concepts extracted from cli
