Combining other antihypertensive drugs with β-blockers in hypertension: a focus on safety and tolerability.

Combining multiple classes of antihypertensive drugs together is one of the most important factors for achieving blood pressure control in most hypertensive patients. The benefits of combination therapy in comparison with monotherapy include: a synergistic enhancement of each drug's hypertensive effects and a potential reduction of side effects if each drug is used at a lower dose. Although long-acting dihydropyridine calcium channel blockers and β-blockers are a good fit for combination therapy, because of the risk of atrioventricular block and bradycardia, the combination of verapamil and β-

Vasodilatory mechanisms of beta receptor blockade.

Beta-blockers are widely prescribed for the treatment of a variety of cardiovascular pathologies. Compared to traditional beta-adrenergic antagonists, beta-blockers of the new generation exhibit ancillary properties such as vasodilation through different mechanisms. This translates into a more favorable hemodynamic profile. The relative affinities of beta-adrenoreceptor antagonists towards the three beta-adrenoreceptor isotypes matter for predicting their functional impact on vasomotor control. This review will focus on the mechanisms underlying beta-blocker-evoked vasorelaxation with a specif

Discovery of potential AXL inhibitors using virtual screening, molecular docking, molecular dynamics, molecular mechanics, and in vitro validation.

AXL is overexpressed in many cancers, including breast, lung, colon, osteosarcoma, kidney, stomach, and gliomas, and associated with poor patient survival and plays an important role in cancer cell proliferation, survival, tumor growth, progression and metastasis. In vivo studies involving genetic targeting of AXL validated it as a promising therapeutic target. In this study, we performed integrated in silico analyses, including virtual screening, molecular docking, molecular dynamics (MD) simulations, and MM/GBSA calculations to search the compound libraries from various small molecule databa

Target Blood Pressure and Combination Therapy: Focus on Angiotensin Receptor Blockers Combination with Either Calcium Channel Blockers or Beta Blockers.

BACKGROUND: The target blood pressure has changed many times in the guidelines in past years. However, there is always a question; is it good to lower blood pressure below 120/80 or not? Control of blood pressure in hypertension is very important in reducing hypertension-modified organ damage. So, the guidelines recommend combining more than one antihypertensive drug to reach the target blood pressure goal. RESULTS: Combination therapy is recommended by guidelines to reach the blood pressure goal. The guidelines recommend many combinations, such as the combination of angiotensin receptor bloc

Slow Coronary Flow: Pathophysiology, Clinical Implications, and Therapeutic Management.

Coronary slow flow (CSF) is an angiographic phenomenon with specific epidemiologic characteristics, associated clinical presentation, and prognosis. Although patients with CSF are diagnosed as having "normal coronary arteries," it seems appropriate to consider CSF as a distinct disease entity requiring specific treatment. The patient with CSF is usually male, smoker, obese, with a constellation of risk factors suggestive of metabolic syndrome. Unstable angina is the most common clinical presentation, with recurrent episodes of chest pain at rest associated with electrocardiographic changes oft

An Energy Metabolism Nanoblocker for Cutting Tumor Cell Respiration and Inhibiting Mitochondrial Hijacking from Cytotoxic T Lymphocyte.

Energy metabolism modulation emerges as a highly regarded strategy for tumor therapy. However, the efficacy of targeting energy metabolism in tumor cells remains unsatisfactory due to the alternate energy production pathways by switching between mitochondrial respiration and glycolysis. In addition, tumor cells can hijack mitochondria from peripheral immune cells to maintain their energy metabolism as an extra respiratory pathway. In this study, a CD44 receptor-targeted hyaluronic acid energy metabolism nanoblocker is developed to achieve bidirectional blockade of basal respiration in tumor ce

A Review on Pharmacokinetic and Pharmacodynamic Drug Interactions of Adrenergic β-blockers with Clinically Relevant Drugs-An Overview.

Adrenergic β-blockers are used to treat many conditions, including hypertension, cardiac arrhythmias, heart failure, angina pectoris, migraine, and tremors. The majority of the β-blockers including Propranolol, Metoprolol, Acebutolol, Alprenolol, Betaxolol, Carvedilol, Nebivolol and Oxprenolol are metabolised majorly by CYP2D6, and Bisoprolol is primarily metabolised by CYP3A4 enzymes. The drugs inhibiting or inducing them may alter the pharmacokinetics of those β-blockers. The plasma concentrations of Propranolol might be elevated by the concomitant use of drugs, such as SSRIs (Fluoxetine, Pa

Pharmacological prevention of eNOS uncoupling.

Under physiological conditions, nitric oxide (NO) is produced in the vasculature mainly by the endothelial NO synthase (eNOS). This endothelium-derived NO is a protective molecule with antihypertensive, antithrombotic and anti-atherosclerotic properties. Cardiovascular risk factors such as hypertension, hypercholesterolemia, cigarette smoking and diabetes mellitus induce oxidative stress mostly by stimulation of the NADPH oxidase. Overproduction of reactive oxygen species leads to oxidation of tetrahydrobiopterin (BH4), the essential cofactor of eNOS. In BH4 deficiency, oxygen reduction uncoup

[Practice of antihypertensive treatment in diabetic patients with hypertension in Hungary].

Diabetes mellitus as comorbidity is present in 20-25% of patients suffering from high blood pressure. Because simultaneous presence of these two diseases results in a significant increase of cardiovascular risk, various guidelines chiefly focus on the antihypertensive treatment of patients with diabetes. Combined drug therapy is usually required to achieve the blood pressure target value of <140/85 mmHg defined for patients with diabetes, which must be based on angiotensin converting enzyme-inhibitors or angiotensin receptor blockers. These can be/must be combined with low dose, primarily thia

Cardiopulmonary Exercise Test as a Tool to Choose Therapy in Heart Failure.

Heart failure treatment depends on several drugs, all providing improvement in outcome, but that cannot be realistically used all together in the same patient. It would be useful to have a tool that allows the arrangement of the most appropriate therapy cocktail for each patient. The aim of this article is to show the main differences in the effects of several drugs on cardiopulmonary function in patients with heart failure, both while resting and during exercise, and to discuss how these differences can be taken into account when choosing the most appropriate therapeutic protocol. In summary,

Development of pH-Sensitive β‑Adrenergic Receptor Antagonists.

G protein-coupled receptors (GPCRs) can trigger distinct signaling pathways depending on their localization. Because their appearance is not restricted, GPCRs can signal not only from the plasma membrane but also from internalized compartments. The pH of endosomes is significantly lower compared to the extracellular environment, and thus, location-biased ligands can be developed by carefully adjusting their physicochemical properties. This work describes the development of pH-sensitive β-adrenergic receptor antagonists with weak basicity, conferring enrichment in acidic compartments. Starting

Sexual Dysfunction, Cardiovascular Risk and Effects of Pharmacotherapy.

BACKGROUND: Sexual dysfunction affects millions of people with an increasing prevalence, worldwide. The pathophysiology of the disease shares several similarities with cardiovascular disease (CVD), including atherosclerosis, endothelial dysfunction, structural vascular damage and subclinical inflammation. Erectile dysfunction (ED) and female sexual dysfunction are common among patients with CVD and risk factors such as hypertension, diabetes, obesity and metabolic syndrome. Given the common pathogenesis of the diseases, ED is an independent prognostic factor of future ED events. Patients with

Case Report: A reversible autonomic-endothelial migraine pattern: a conceptual perspective illustrated by a clinical case.

Migraine is commonly regarded as a primary neurovascular disorder; however, systemic endothelial and autonomic factors may modulate its expression and course. A 41-year-old male with a 10-year history of migraine without aura and vasomotor rhinitis experienced sustained remission after initiation of nebivolol 5 mg and (perindopril 5 mg/indapamide 1.25 mg/amlodipine 5 mg) once daily for mild hypertension. Attacks occurred 3-4 times per month-often on Saturdays-were triggered by strong odors such as perfume, and occasionally accompanied by nausea or vomiting. Headaches fulfilled ICHD-3 criteria

Anti-hypertensive medications and erectile dysfunction: focus on β-blockers.

PURPOSE: Although anti-hypertensive medications, including thiazides and β-blockers (BBs) in particular, have been suggested to cause erectile dysfunction (ED) their real contribution is still conflicting. The aim of this paper is to summarize available evidence providing an evidence-based critical analysis of the topic. METHODS: An overall comprehensive narrative review was performed using Medline, Embase and Cochrane search. In addition, to better understand the impact of BBs on ED a specific systematic review was also performed. RESULTS: The negative role of centrally acting drugs, such a

Antihypertensive drugs and glucose metabolism.

Hypertension plays a major role in the development and progression of micro- and macrovascular disease. Moreover, increased blood pressure often coexists with additional cardiovascular risk factors such as insulin resistance. As a result the need for a comprehensive management of hypertensive patients is critical. However, the various antihypertensive drug categories have different effects on glucose metabolism. Indeed, angiotensin receptor blockers as well as angiotensin converting enzyme inhibitors have been associated with beneficial effects on glucose homeostasis. Calcium channel blockers
