'South Asian cocktail'--the concurrent use of opioids, benzodiazepines and antihistamines among injecting drug users in Nepal and associations with HIV risk behaviour.

BACKGROUND: Data of the Central Bureau of Statistic of Nepal from 2008 show a total of more than 46,000 illegal drug users, out of which 61% are injecting drug users (IDU). An injecting mix of medicines like opioids, benzodiazepines and antihistamines (the so-called South Asian cocktail) was prevalent. Furthermore, it is estimated that about 70,000 people are living with human immunodeficiency virus (HIV). The government of Nepal has started realizing and recognizing drug use and HIV as significant health and social issues. Harm reduction programs such as needle syringe exchange and opioid sub

A Decline in HIV Testing Among Persons Who Inject Drugs in the Seattle Area, 2004-2015.

BACKGROUND: Promoting HIV testing is a key component of the public health response to HIV. Assessing HIV testing frequency among persons who inject drugs (PWID) monitors the status of these efforts and can identify unmet needs and opportunities to more effectively promote testing. METHODS: Data were combined from 4 Seattle-area surveys of PWID from the National HIV Behavioral Surveillance (NHBS) program (2005-2015) and 6 surveys of Needle Exchange clients (2004-2015). RESULTS: The proportion of PWID reporting an HIV test in the previous 12 months declined from 64% in 2005% to 47% in 2015 in

Social networks, secondary syringe exchange, and opioid agonist therapy retention among people who inject drugs in Hartford, CT.

BACKGROUND: Opioid agonist therapies (OAT) and  harm reduction such as syringe service programs (SSP) have been shown to be effective in preventing adverse outcomes such as overdose deaths, HIV and Hepatitis C infections among people who inject drugs (PWID). The importance of social network influence on disease transmission is well established, yet the interplay between harm reduction and network structures is, generally, not well understood. This study aims to analyze how social networks can mediate the harm reduction effects associated with secondary exchange through syringe service programs

Increasing prevalence of cocaine as the primary detoxification diagnosis among admissions presenting with current intravenous drug use: a review of detoxification records from northern British Columbia, 1999-2005.

OBJECTIVES: This study sought to document the trends in drug use among intravenous drug users (IDUs) in northern British Columbia, and to discuss the public health implications. METHOD: We conducted a 7-year medical-chart review of all IDU-related admissions (n = 2072) to an inpatient alcohol and drug detoxification centre in Prince George, British Columbia. Primary detoxification diagnosis was modeled onto year of admission using generalized estimating equations (GEE). RESULTS: Our study demonstrated an increasing prevalence of cocaine as the primary detoxification diagnosis in IDU-related

Optimal investment in HIV prevention programs: more is not always better.

This paper develops a mathematical/economic framework to address the following question: Given a particular population, a specific HIV prevention program, and a fixed amount of funds that could be invested in the program, how much money should be invested? We consider the impact of investment in a prevention program on the HIV sufficient contact rate (defined via production functions that describe the change in the sufficient contact rate as a function of expenditure on a prevention program), and the impact of changes in the sufficient contact rate on the spread of HIV (via an epidemic model).

HIV infection among persons who inject drugs: ending old epidemics and addressing new outbreaks.

AIDS among persons who inject drugs, first identified in December 1981, has become a global epidemic. Injecting drug use has been reported in 148 countries and HIV infection has been seen among persons who inject drugs in 61 countries. Many locations have experienced outbreaks of HIV infection among persons who inject drugs, under specific conditions that promote very rapid spread of the virus. In response to these HIV outbreaks, specific interventions for persons who inject drugs include needle/syringe exchange programs, medicated-assisted treatment (with methadone or buprenorphine) and antir

Access to highly active antiretroviral therapy for injection drug users: adherence, resistance, and death.

Injection drug users (IDUs) continue to comprise a major risk group for HIV infection throughout the world and represent the focal population for HIV epidemics in Asia and Eastern Europe/Russia. HIV prevention programs have ranged from HIV testing and counseling, education, behavioral and network interventions, drug abuse treatment, bleach disinfection of needles, needle exchange and expanded syringe access, as well as reducing transition to injection and primary substance abuse prevention. With the advent of highly active antiretroviral therapy (HAART) in 1996, dramatic clinical improvements

History of injection drug use in the month preceding incarceration in Iranian prisons.

This study aimed to investigate the prevalence and pattern of injecting drug use (IDU) in Iranian incarcerated people one month before their imprisonment. This cross-sectional study used data from the national Bio-Behavioral Surveillance Surveys conducted from a random sample of people incarcerated in 33 prisons covering the major geographical areas of Iran in 2017 (n = 5,785). A multivariable logistic regression model was used to determine factors related to IDU one month before imprisonment. The lifetime prevalence of IDU among Iranian incarcerated people was 12.2% (95% confidence interval [

Substance abuse and HIV pregnancy.

Substance abuse is a significant risk factor for HIV in pregnancy. Most substances can have deleterious effects on perinatal outcome, and in pregnancies complicated by maternal HIV infection, substance abuse may contribute to the rate of vertical transmission and the progression of maternal disease. Therefore, pregnant substance-abusing women should be identified and targeted for HIV counseling and testing. In addition, drug rehabilitation should be strongly advocated. In populations with high prevalences of substance abuse and HIV infection, exceptional measures such as voluntary routine drug

A continuum of disconnection: the impacts of safer supply program closures on continuity of care, transition, and total loss of service access.

OBJECTIVE: In March 2025, the expiration of federal pilot funding for Safer Supply Programs (SSPs), combined with restrictive provincial legislative changes to harm reduction service provision in Ontario, disrupted care for thousands of clients at risk of toxic drug poisoning. This study examines the impacts of these changes on client health, social stability, and overdose risk across a continuum of access. METHOD: We conducted 30 semi-structured virtual interviews between September and November 2025 with clients who were enrolled in an SSP prior to March 31, 2025. Participants were recruited

Optimal provision of opiate substitution therapy and needle and syringe programmes: a multi-method realist evaluation.

BACKGROUND: The barriers and facilitators to engagement with Opiate Substitute Treatment and Needle and Syringe Provision need to be better understood to develop interventions to attract people into services. OBJECTIVE(S): We conducted a multi-method realist evaluation to generate an in-depth theory of the factors that influence the optimal provision of services in terms of access, engagement, retention and successful exit. DESIGN: Multi-method involving an online survey of United Kingdom drug and alcohol service commissioning leads and a meta-synthesis of qualitative literature (phase 1) to

"Because we're the experts": centering the expertise of people who use drugs to identify and propose solutions for overdose prevention.

BACKGROUND: People who use drugs possess valuable expertise, knowledge, needs, and preferences as part of their lived experience, which should be incorporated into overdose-focused public health programs and policies. METHODS: We formed a community advisory board composed of 16 clients from four syringe services programs (SSPs) in Wisconsin for an overdose prevention research study to inform project priorities. Over 6 meetings spanning 10 months, the board engaged in a root cause analysis facilitated by SSP staff and University researchers using a semi-structured assessment technique from the

Barriers and facilitators to buprenorphine delivery: Results from a qualitative study of syringe services provider experiences.

BACKGROUND: Access to medications for opioid use disorder (MOUD) is essential for the 6.1 million Americans with OUD. However, only one in five adults with past-year OUD received MOUD. Syringe services programs (SSPs) have existing and trusting relationships with people who use drugs and are an alternative service provider to traditional healthcare settings. SSPs are uniquely positioned to facilitate buprenorphine inductions and increase overall MOUD access. We aimed to understand models for implementing buprenorphine and barriers and facilitators SSPs encounter. METHODS: We interviewed 23 SS

The prevalence of fentanyl in New York City's unregulated drug supply as measured through drug checking offered at syringe service programs.

BACKGROUND: Drug checking is a harm reduction strategy that provides people who use drugs (PWUD) information about the composition of a substance. Drug checking has been identified as a key public health response to the continually evolving overdose crisis. METHODS: The New York City Health Department of Health and Mental Hygiene (DOHMH) partnered with existing syringe programs to launch a drug-checking pilot study. The study offered multiple point-of-care drug-checking technologies including fentanyl test strips (FTS) and Fourier-transformed infrared (FTIR) spectrometry, as well as secondary

Use of harm reduction strategies by individuals with a history of incarceration: A short report using baseline data collected from the STAMINA clinical trial.

BACKGROUND: Due to the considerably heightened risk of overdose immediately following jail or prison release, expansion of harm reduction interventions for citizens returning to the community after an incarceration episode should be of utmost concern. However, there are no studies examining the adoption and use of harm reduction among this population. This short report examines the use of individual-level harm reduction strategies (HRS) among people who use drugs, comparing those who have lifetime histories of incarceration with those who do not. METHODS: This study included baseline data (N

Reframing Dutch drug policies: a new era for harm reduction.

In this article the authors offer their perspective on the changes in the Dutch harm reduction field. From the 1970s to the 1990s, the Netherlands emerged as a leader in harm reduction services, driven by grassroots movements like the Medisch-sociale Dienst Heroïne Gebruikers (MDHG) (Medisch-sociale Dienst Heroïne Gebruikers (MDHG) translates to Medical-Social Service Heroin Users in English) in Amsterdam and Junkiebond in Rotterdam. These organisations advocated for health-centred policies, initiated needle exchange programmes, and created safe consumption spaces. Their efforts led to signifi

Participants of a mail delivery syringe services program are underserved by other safe sources for sterile injection supplies.

BACKGROUND: In the United States, accessing sterile injection supplies remains challenging for many people who inject drugs (PWID). Although women are less likely to inject drugs than men, women who do inject are disproportionately affected by IDU-related complications. Needle Exchange Technology (NEXT), the first formal online accessed mail delivery syringe services program (SSP) in the US, may overcome access barriers. We evaluated whether NEXT was reaching women participants and people without access to other safe sources of sterile injection supplies. METHODS: This cross-sectional study e

Primary Care for Persons Who Inject Drugs.

More than 750,000 persons in the United States inject opioids, methamphetamine, cocaine, or ketamine, and that number is increasing because of the current opioid epidemic. Persons who inject drugs (PWID) are at higher risk of infectious and noninfectious skin, pulmonary, cardiac, neurologic, and other causes of morbidity and mortality. Nonjudgmental inquiries about current drug use can uncover information about readiness for addiction treatment and identify modifiable risk factors for complications of injection drug use. All PWID should be screened for human immunodeficiency virus infection, l

Two steps forward, one step back: current harm reduction policy and politics in the United States.

Harm reduction policies and attitudes in the United States have advanced substantially in recent years but still lag behind more advanced jurisdictions in Europe and elsewhere. The Obama administration, particularly in its last years, embraced some harm reduction policies that had been rejected by previous administrations but shied away from more cutting edge interventions like supervised consumption sites and heroin-assisted treatment. The Trump administration will undermine some of the progress made to date but significant state and local control over drug policies in the US, as well as grow
