Analysis of late-onset neonatal sepsis cases in a level three neonatal intensive care unit.

OBJECTIVE: Newborns in neonatal intensive care units (NICUs) are at high risk for developing nosocomial infections (NIs), which may result in morbidity and mortality. In this study, we aimed to ascertain the bacteriological profiles and their antimicrobial susceptibility patterns in NIs. METHODS: We conducted a cross-sectional study in the NICU. Late-onset sepsis (LOS) cases confirmed with blood cultures were evaluated retrospectively. Laboratory parameters, demographics, and clinical data were collected and analyzed from hospital records retrospectively. RESULTS: Of the 1210 infants in the

Toward the early diagnosis of neonatal sepsis and sepsis-like illness using novel heart rate analysis.

BACKGROUND AND OBJECTIVE: Abrupt clinical deterioration because of sepsis is a major cause of morbidity and mortality in neonates, and earlier diagnosis should improve therapy of this potentially catastrophic illness. In practice, clinical signs and laboratory data have not been perceived as sensitive or specific for early stages of sepsis. Because heart rate characteristics (HRC) are abnormal during fetal distress and neonatal illness, we hypothesized that abnormal HRC might precede the clinical diagnosis of neonatal sepsis, adding independent information to standard clinical parameters. Early-onset neonatal sepsis and neonatal antibiotic exposure in Hungary a nationwide population-based study.

UNLABELLED: Neonatal sepsis remains a challenge in perinatal medicine. We aimed to estimate incidence of culture positive early-onset sepsis (CP-EOS), causative pathogens and overall antibiotic exposure in newborn infants in Hungary. Retrospective national registry analysis (2020-2023). Data were mainly collected from the Hungarian National Perinatal Registry covering all level II-III neonatal intensive care units (NICUs) in Hungary. The number of livebirths in each gestational age group was provided by Hungarian Central Statistical Office. Overall CP-EOS incidence was 0.41/1,000 livebirths (0

Empiric antibiotic regimens for neonatal sepsis in Australian and New Zealand neonatal intensive care units.

AIM: Neonatal sepsis remains an important cause of morbidity and mortality, and requires prompt empiric treatment. However, only a minority of babies who receive antibiotics for suspected sepsis have an infection. Antimicrobial exposure in infancy has important short- and long-term consequences. There is no consensus regarding empirical antimicrobial regimens. METHODS: The study included a survey of empiric antimicrobial regimens in all tertiary neonatal intensive care units in Australia and New Zealand in 2013-2014. RESULTS: All 27 units responded. For early-onset sepsis, all units used a c

Costs and cost-effectiveness of an infection prevention bundle to reduce neonatal sepsis and mortality in Zambia: The Sepsis Prevention in Neonates in Zambia (SPINZ) trial.

The Sepsis Prevention in Neonates in Zambia (SPINZ) trial was a prospective observational cohort study conducted in the neonatal intensive care unit of the University Teaching Hospital in Lusaka, Zambia. Introduction of an infection prevention and control (IPC) bundle reduced hospital-associated mortality, total mortality, suspected sepsis, and confirmed bloodstream infections. This companion analysis was undertaken to analyze intervention costs and cost-effectiveness in this low-resource setting. We conducted a retrospective cost analysis, using SPINZ study-related records, and expressed cost

Sepsis-induced coagulopathy in preterm neonates with Gram-positive sepsis presents with hypercoagulation and reduced platelet activation compared with healthy preterm neonates.

BACKGROUND: Neonatal sepsis is frequently accompanied by coagulopathy and thrombocytopenia attributed to the cross-link between inflammation and coagulation. However, sepsis-induced coagulopathy and platelet function in septic preterm neonates remain to be elucidated. In addition, there is no robust evidence for a causal relationship between thrombocytopenia and bleeding in preterm neonates with sepsis. OBJECTIVE: This single-center prospective cohort study aimed to assess sepsis-induced coagulopathy and platelet function in preterm neonates during sepsis. METHODS: We included 25 preterm neo

Targeted proteomics in late-onset neonatal sepsis: an exploratory analysis.

BACKGROUND: Late-onset neonatal sepsis (LONS) is a major cause of morbidity and mortality in preterm infants, but early diagnosis remains difficult. We aimed to identify candidate proteomic biomarkers of LONS and evaluate whether these changes could be reproduced by in vitro stimulation. METHODS: Longitudinal blood samples were collected from preterm neonates with suspected LONS and non-septic controls in two neonatal intensive care units, at onset and after 24 and 48 hours. In parallel, whole blood from preterm neonates without LONS was stimulated in vitro with E. coli and S. epidermidis. Pr

Neonatal sepsis: any role for procalcitonin as a diagnostic marker?

INTRODUCTION: In low and middle-income countries, a key driver of neonatal morbidity and mortality is neonatal sepsis. Rapid and precise diagnosis of neonatal sepsis remains a critical challenge due to limited laboratory facilities. Blood culture results, the diagnostic gold standard, are often unavailable or delayed; therefore, procalcitonin (PCT) has been considered as a potential inflammatory biomarker of bacterial infection. This study assessed the diagnostic performance of PCT in neonates with sepsis at the Federal Teaching Hospital, Owerri, Nigeria. METHODS: This cross-sectional study rec

Beyond contamination rates: a broader lens on diagnosing neonatal sepsis.

OBJECTIVES: To evaluate whether anti-TNF-α therapy (infliximab) administration alters circulating levels of ghrelin, an anti-inflammatory gastric peptide. We also assessed possible associations of circulating ghrelin concentrations with CRP and ESR levels, metabolic syndrome, demographic characteristics and other adipokines in ankylosing spondylitis (AS) patients. METHODS: We studied 30 consecutive non-diabetic AS patients, without history of cardiovascular (CV) events, on periodical treatment with infliximab. Serum ghrelin levels were determined immediately prior to and after an infliximab i

Diagnosis for Neonatal Sepsis-Associated Encephalopathy: Don't Forget Neuropathological Biomarkers.

BACKGROUND: Neonatal sepsis-associated encephalopathy (nSAE) poses significant diagnostic challenges due to nonspecific clinical manifestations and limitations of current diagnostic standards. It is imperative to conduct extensive research to elucidate the intricate relationship between neuropathological biomarkers and neonatal sepsis, which represents an intriguing and significant subject for exploration. This article emphasizes the critical need to integrate neuropathological biomarkers, such as neuron-specific enolase (NSE), S100β, glial fibrillary acidic protein (GFAP), Tau, and ubiquitin

Angiogenesis-associated pathways play critical roles in neonatal sepsis outcomes.

Neonatal sepsis is a major cause of childhood mortality. Limited diagnostic tools and mechanistic insights have hampered our abilities to develop prophylactic or therapeutic interventions. Biomarkers in human neonatal sepsis have been repeatedly identified as associated with dysregulation of angiopoietin signaling and altered arachidonic acid metabolism. We here provide the mechanistic evidence in support of the relevance for these observations. Angiopoetin-1 (Ang-1), which promotes vascular integrity, was decreased in blood plasma of human and murine septic newborns. In preclinical models, ad

Artificial and human intelligence for early identification of neonatal sepsis.

Artificial intelligence may have a role in the early detection of sepsis in neonates. Machine learning can identify patterns that predict high or increasing risk for clinical deterioration from a sepsis-like illness. In developing this potential addition to NICU care, careful consideration should be given to the data and methods used to develop, validate, and evaluate prediction models. When an AI system alerts clinicians to a change in a patient's condition that warrants a bedside evaluation, human intelligence and experience come into play to determine an appropriate course of action: evalua

Neonatal Sepsis: A Review of Pathophysiology and Current Management Strategies.

BACKGROUND: Early-onset sepsis, occurring within 72 hours of birth, and late-onset sepsis, occurring after this time period, present serious risks for neonates. While culture-based screening and intrapartum antibiotics have decreased the number of early-onset cases, sepsis remains a top cause of neonatal morbidity and mortality in the United States. PURPOSE: To provide a review of neonatal sepsis by identifying its associated risk factors and most common causative pathogens, reviewing features of the term and preterm neonatal immune systems that increase vulnerability to infection, describing

Recent advances in developing biosensing based platforms for neonatal sepsis.

Neonatal sepsis is a bloodstream infection primarily caused by Escherichia coli (E. coli), Group B Streptococcus (GBS), Listeria monocytogenes, Haemophilus influenzae, S. aureus, Klebsiella spp. and non-typhoidal Salmonella bacteria. Neonatal Sepsis is referred as a critical response to the infection in the neonatal period that can lead to the failure of body organs and thereby causing damage to the tissues resulting in death of the neonates. Nearly 4 million deaths across the world are occurred due to neonatal sepsis infections. In order to prevent the bloodstream infections in the neonates,
