Percutaneous nephrolithotomy in solitary kidneys: experience with 412 cases from Southern Iran.

Some patients with nephrolithiasis who become candidates for percutaneous nephrolithotomy (PCNL) have a single kidney. This poses a challenge for the physician regarding the safety and efficacy of this procedure for these patients. This study has aimed to evaluate the safety and efficacy of PCNL in these patients. Between 2002 and 2014, out of 10,800 cases who underwent PCNL in our centres, 412 had a single kidney. We recorded the results and complications in these patients. Out of 412 patients, 279 were men and 133 women. Their mean age was 46.4 years (range 19-71) and mean stone size was 26.

Do patients benefit from miniaturized tubeless percutaneous nephrolithotomy? A comparative prospective study.

BACKGROUND AND PURPOSE: A benefit of miniaturized percutaneous nephrolithotomy (MPCNL) compared with conventional percutaneous nephrolithotomy (PCNL) has not been demonstrated as yet. Thus, the aim of this study was to evaluate the outcome of conventional vs MPCNL and to determine if MPCNL offers an advantage for the patient. PATIENTS AND METHODS: A prospective, nonrandomized series of 50 consecutive patients with solitary calculi (lower pole or the renal pelvis) were treated either by conventional PCNL (26F) or MPCNL (18F). Ultrasound or holmium laser were used for lithotripsy. Patients were

Sterile water versus isotonic saline solution as irrigation fluid in percutaneous nephrolithotomy.

INTRODUCTION: We evaluated the safety of sterile water as an irrigation solution for percutaneous nephrolithotomy (PCNL). MATERIALS AND METHODS: Forty-four patients with kidney calculi were enrolled in this study and randomly divided into two groups for PCNL. Approaches to the calculi were through a single subcostal access with an Amplatz sheath, and either sterile water or isotonic saline solution was used as the irrigation fluid. Serum hemoglobin, haptoglobin, sodium, potassium, and creatinine were measured before and 12 hours after the procedure. The patients were evaluated for signs of tr

Percutaneous nephrolithotomy for staghorn calculi: a single center's experience over 15 years.

BACKGROUND AND PURPOSE: Percutaneous nephrolithotomy (PCNL) for staghorn calculi is one of the more challenging endourologic procedures. Although excellent stone-free rates are universally reported in the literature, complication rates vary widely, especially related to the need for blood transfusion. The purpose of this study was to evaluate the outcomes of PCNL for patients with staghorn calculi in a large series of patients at a single, tertiary referral, endourologic stone center. PATIENTS AND METHODS: Between July 1990 and December 2005, 1338 patients underwent PCNL for renal stone disea

Plastic within renal pelvis on nephroscopy: a potential hazard during percutaneous nephrolithotomy.

The use of adhesive plastic drapes is common throughout laparoscopic and percutaneous surgery. We report a case in which a significant portion of the plastic drape was translocated into the renal pelvis during tract dilation for percutaneous nephrolithotomy. The complication was recognized only on endoscopy and the plastic successfully removed. The potential for morbidity in other patients in the future remains, and we recommend operators be aware of this unusual, but entirely preventable, complication. We highlight some measures to help avoid and check for this complication before the end of

Prevention of ureteral stone migration during percutaneous nephrolithotomy: a randomized controlled trial.

OBJECTIVE: With the increasing adoption of totally tubeless percutaneous nephrolithotomy (ttPCNL), prevention of stone fragment migration into the ureter has gained clinical importance. This study aimed to compare the incidence of ureteral fragment migration at the end of PCNL using a 5 Fr open-ended ureteral catheter (5FrUC) versus an occlusion balloon catheter (OBC). METHODS: In this IRB-approved randomized controlled trial, 108 eligible patients were randomized 1:1 to either the 5FrUC or OBC group and 99 were included in the final analysis. The primary outcome, ureteral fragment migration,

Transient Urinothorax Following Nephrostomy Tube Placement and Percutaneous Nephrolithotomy: A Case Report.

Urinothorax is a rare complication of urological procedures. This report presents a case of a patient who developed urinothorax following nephrostomy tube placement and percutaneous nephrolithotomy (PCNL). The patient was managed conservatively with chest tube and Foley catheter placement, without the need for surgery. Computed tomography (CT) and chest tube output indicated that the urinothorax occurred immediately after nephrostomy tube placement but resolved within a couple of days without further intervention. Unlike some other cases that required surgical intervention due to persistent ur

Thulium laser-based hemostasis during percutaneous nephrolithotomy: a prospective observational multicentric study.

PURPOSE: Percutaneous nephrolithotomy (PNL) is a controlled grade IV renal trauma and intraoperative bleeding is the most crucial complication. Contemporarily, pulsed mode thulium laser has been reported to confer adequate hemostasis. Therefore, the primary outcome of this study was to assess safety and efficacy of thulium laser-based hemostasis during PNL. METHODS: This was a prospective observational study including all patients, aged  ≥ 18 years old with renal stone(s)  ≥ 20 mm in the maximal dimension, candidate for PNL. Pregnant patients or those with uncorrected coagulopathy were exclud

Paravertebral block vs. epidural block for percutaneous nephrolithotomy: A prospective, randomized study.

BACKGROUND: Percutaneous nephrolithotripsy (PCNL) is the main method for urinary calculi. An anesthesia method with little effect on the blood circulation and which does not affect the postoperative activity of a patient is lacking. OBJECTIVE: To compare the effects of paravertebral nerve block (PNB) and epidural block (EPB) on quadriceps femoris muscle (QFM) strength in patients after PCNL. METHODS: 163 patients were separated into two groups: EPB (81) and PNB (82). Primary outcome parameters were QFM strength and range of motion (RoM) of the knee 1 h, 2 h, 3 h, and 24 h after anesthesia in

Frequency and risk factors for antegrade ureteral stone migration after percutaneous nephrolithotomy.

In a recently published paper in the "British Medical Journal", a joint collaboration of well-known international scientists represented by Islami et al. has tackled this issue.1 In a well-designed, laborious case-control study in Golestan, Iran, they looked at 300 cases of esophageal SCC and 571 age and sex matched neighborhood controls, assessed the temperature of the commonly used drink "tea" among them and compared it between the two groups. After adjusting for several confounders including ethnicity, daily vegetable intake, alcohol consumption, tobacco or opium use, duration of residence
