We present a patient who had a ruptured distal middle cerebral artery (MCA) aneurysm at the M2-M3 junction with a temporal lobe hematoma, in which intraoperative ultrasound (iUS) proved critical for real-time aneurysm localization when neuronavigation was compromised by brain shift. An elderly patient with subarachnoid hemorrhage and a large temporal lobe intracerebral hematoma underwent craniotomy, hematoma evacuation, and aneurysm clipping. Preoperative computed tomography angiography (CTA) identified a 5-mm aneurysm at the left M2-M3 junction. During surgery, neuronavigation was initially p

BACKGROUND: Ventricular catheter placement can be a challenging procedure when treating patients with slit ventricles, despite the use of a neuronavigation system.
METHODS: We report the case of 3 patients with idiopathic intracranial hypertension who had required revision of their ventricular catheter due to malpositioning, despite initial placement using neuronavigation. Owing to the absence of intraoperative computed tomography in our center, we used the O-arm imaging system to confirm placement of the optimal ventricular tip position intraoperatively.
RESULTS: Optimal ventricular drain p

OBJECTIVE: Intraoperative localisation of the sensorimotor cortex using the phase reversal of somatosensory evoked potentials (SEPs) is an essential tool for surgery in and around the perirolandic gyri, but unsuccessful and perplexing results have been reported. This study examines the effect of tumour masses on the waveform characteristics and feasibility of SEP compared with functional neuronavigation and electrical motor cortex mapping.
METHODS: In 230 patients with tumours of the sensorimotor region the SEP phase reversal of N20-P20 was recorded from the exposed cortex using a subdural gr

A 47-year-old man presented with a subarachnoid hemorrhage (SAH) and right cerebellar hematoma was referred for evaluation. Cerebral angiography revealed a distal anterior inferior cerebellar artery (AICA) aneurysm associated with an arteriovenous malformation (AVM). Successful obliteration and complete removal of the aneurysm and AVM were obtained using transcortical approach under the guidance of neuronavigation system. The association of a peripheral AICA aneurysm and a cerebellar AVM by the same artery is unique. The reported cases of conventional surgery for this disease complex are not c

UNLABELLED: Performing microsurgery with the support of navigation in falcine meningioma management shows significant impacts in short and middle-time follow-up, including unilaterally skull opening with smallest and nearest skin incision, lessen the surgical duration, limit blood transfusion and prevent tumours from recurrence.
MATERIALS AND METHODS: Sixty-two falcine meningioma patients treated by microoperation applying neuronavigation were enroled from July 2015 to March 2017. Patients are evaluated before and 1 year after surgery according to The Karnofsky Performance Scale (KPS) for com

OBJECTIVE: Patient outcomes of ventriculoperitoneal (VP) shunt surgery, the mainstay treatment for hydrocephalus in adults, are poor because of high shunt failure rates. The use of neuronavigation or laparoscopy can reduce the risks of proximal or distal shunt catheter failure, respectively, but has less independent effect on overall shunt failures. No adult studies to date have combined both approaches in the setting of a shunt infection prevention protocol to reduce shunt failure. The goal of this study was to determine whether combining neuronavigation and laparoscopy with a shunt infection

Awake craniotomy is a surgical procedure that has been gaining significance over the past decades. Neuronavigation is an intraoperative technology that locates tumors and monitors the brain cortex during awake craniotomy. The presence of cerebral low-grade gliomas in the frontal lobe creates a risk of affecting vital centers of the brain cortex during surgery. We present a clinical case of a 42-year-old male patient who entered the neurosurgery clinic with a clinical manifestation of headache for two months. MRI showed evidence of the recurrence of a left frontal glioma. Differential diagnoses

BACKGROUND: Obsessive-compulsive disorder (OCD) imposes a substantial burden on patients and health systems alike: despite established pharmacological and psychological interventions, up to 40% of patients fail to achieve full remission with first-line treatment, driving persistent interest in alternative neuromodulation approaches. Among the neuromodulation strategies under investigation, low-frequency (predominantly 1 Hz), non-neuronavigated repetitive transcranial magnetic stimulation (rTMS) represents a surface-coil protocol class distinct from the FDA-cleared deep TMS approach for OCD, ye

OBJECT: The purpose of this study was to identify the anatomy of pineal region venous complex using neuronavigation software when distorted by the presence of a space-occupying lesion and to describe the anatomical relationship between lesion and veins. Moreover we discuss its influence on the choice of the surgical strategy.
METHODS: Of the 33 patients treated at our Institute for pineal region tumors between 2003 and 2008 we used the neuronavigation software to depict the venous system of the pineal region in 14 patients. We focused on depiction of the basal vein of Rosenthal (BV), the inte

BACKGROUND: For glial tumor management, the extent of resection (EOR) is the key to enhance tumor control and improve patient outcomes. Intraoperative MRI (IoMRI) neuronavigated microsurgery emerged as a useful neuroimaging tool for performing optimal and safe tumor resection.
METHOD: Here, we present the different steps of the microsurgical resection of a challenging deeply located right fronto-temporal glioma, using intraoperative MRI in an integrated IoMRI imaging platform.
CONCLUSION: Intraoperative MRI neuronavigated microsurgery helps to enhance the tumor resection, while reducing unin

BACKGROUND: Solitary fibrous tumors (SFTs) are uncommon in the central nervous system (CNS) and tend to mimic meningioma, based on the symptoms and imaging findings. Falcotentorial location is rare. There is almost no evidence regarding resection using a 3D-4K exoscope.
METHODS: We present the case of a 59-year-old female patient with a tumor adjacent to the falx and tentorium, with extension to the pineal region. Falcotentorial meningioma was considered the first diagnostic possibility. A parieto-occipital interhemispheric transtentorial approach was chosen, guided by neuronavigation and usi

BACKGROUND: Subependymal giant cell astrocytoma (SEGA) is a rare pediatric tumor that most commonly occurs in the setting of tuberous sclerosis complex (TSC). The deep anatomical location of these lesions makes the selection of an appropriate neurosurgical approach critical. However, minimally invasive techniques have emerged as a safe and effective alternative for the surgical resection of tumors located in deep regions.
OBSERVATIONS: A 7-year-old male with a history of TSC presented with a large intraventricular mass located in the frontal horn of the right lateral ventricle, associated wit

About one-third of patients with schizophrenia are classified as treatment-resistant schizophrenia (TRS) experiencing persistent symptoms despite adequate antipsychotic trials. Non-invasive brain stimulation (NIBS) emerged as a promising alternative treatment for TRS, yet its comparative efficacy remains unclear. We conducted a systematic review and network meta-analysis (NMA) to investigate the efficacy of NIBS for TRS patients (PROSPERO: CRD42023442563). Randomized trials comparing NIBS with sham or other active interventions in TRS patients were included. NMA with frequentist random-effects

BACKGROUND: Basilar invagination (BI) is a complex condition characterized by prolapse of the odontoid into the brain stem/upper cervical cord. This lesion is often associated with Chiari malformations, and rheumatoid arthritis (RA). Treatment options for BI typically include cervical traction, an isolated anterior transoral odontoidectomy, anterior endonasal odontoidectomy, an isolated posterior fusion, or combined anterior/ posterior surgical approach.
CASE DESCRIPTION: A 45-year-old female with a Chiari I malformation and RA underwent a combined posterior C0-C5 posterior decompression/fusi
