Sudden appearance of new upper extremity motor function while performing neurophysiologic intraoperative monitoring during tethered cord release: a case report.

UNLABELLED: : Tethered cord syndrome occurs when the distal spinal cord or filum adheres to adjacent structures resulting in progressive sensorimotor deficits in the lower extremities, fecal and/or urinary incontinence, and musculoskeletal deformities. Tethering of the distal cord may be idiopathic, may be associated with an intraspinal abnormality such as a lipoma, but most commonly the distal spinal cord remnant is adherent to the area of the original dysraphism repair in patients with myelodysplasia. Surgery to untether the cord is indicated in patients with worsening pain symptoms, progres

Multimodal intraoperative neurophysiological monitoring may better predict postoperative distal upper extremities' complex-functional outcome than spinal and muscular motor evoked potentials alone in high-cervical intramedullary spinal cord tumor surgery.

OBJECTIVE: D-wave can safely monitor the corticospinal-tract (CST)-function regarding gross-motor outcome of lower extremities, but it is still unknown whether i)D-wave can also safely monitor the gross-motor outcome of distal upper extremities in those patients undergoing high-cervical intramedullary-spinal-cord-tumor (IMSCT)-resection (enabling epidural D-wave-placement below C5) and ii)multimodal IONM can also predict fine-motor/complex hand function. METHODS: We prospectively assessed 20 patients undergoing IMSCT-surgery above the C4/5-level with multimodal IONM (D-wave/mMEPs/EMG/SSEPs).

Below the belt: sensory mapping and monitoring in the sacral-pudendal region.

Originally described in 1982, scalp somatosensory evoked potential responses can be recorded after stimulation of multiple different pudendal nerve- and sacral root-supplied structures. The resulting P40 response is usually the highest amplitude at Cz. Responses are generally easy to resolve and therefore should be of equivalent ease to follow for neurophysiologic intraoperative monitoring versus lower limb peripheral nerve somatosensory evoked potentials (e.g., tibial or fibular [peroneal] nerves), but sizeable reports of pudendal somatosensory evoked potential monitoring are few. Pitfalls in intraoperative nerve monitoring during vestibular schwannoma surgery.

Despite the widespread acceptance of intraoperative neurophysiological monitoring in skull base surgery over the last 2 decades, surgeon training in the technical and interpretive aspects of nerve monitoring has been conspicuously lacking. Inadequate fundamental knowledge of neurophysiological monitoring may lead to misinterpretations and an inability to troubleshoot system errors. Some surgeons perform both the technical and interpretive aspects of monitoring themselves while others enjoin coworkers (surgical residents, nurses, anesthetists, or a separate monitoring service) to perform the te
