Variation in medical management and neurosurgical treatment of patients with supratentorial spontaneous intracerebral haemorrhage.

INTRODUCTION: The role of surgery in spontaneous intracerebral haemorrhage (sICH) remains controversial. This leads to variation in the percentage of patients who are treated with surgery between countries. PATIENTS AND METHODS: We sent an online survey to all neurosurgeons (n = 140) and to a sample of neurologists (n = 378) in Dutch hospitals, with questions on management in supratentorial sICH in general, and on treatment in six patients, to explore current variation in medical and neurosurgical management. We assessed patient and haemorrhage characteristics influencing treatment decisions.

Practical decision-making in the treatment of unruptured cerebral aneurysm in Japan: the U-CARE study.

BACKGROUND: Decision-making during the management of unruptured cerebral aneurysms is a delicate process for both neurosurgeons and patients. Guidelines are evidence-based references that can aid in making decisions. However, neurosurgeons do not always follow guidelines in clinical practice. The purpose of this study is to verify the hypothesis that there is substantial dissociation between treatment guidelines and practical decision-making due to a bias in treatment selection for unruptured cerebral aneurysms. This bias is dependent upon clinician-driven factors such as experience and specia

Intraoperative Ionizing Radiation Exposure Awareness and Associated Morbidity in Neurosurgery: A Nationwide Survey.

BACKGROUND: Neurosurgeons often use radiation to visualize blood vessels and implants intraoperatively. However, high exposure to radiation increases one's cancer risk. This study aims to investigate intraoperative ionizing radiation exposure awareness and associated morbidity among neurosurgeons. METHODS: An anonymized 30-question survey about their intraoperative radiation exposure, protective measures, radiation knowledge, and any conditions that can arise from protracted radiation exposure was disseminated to 3344 American Association of Neurological Surgeons members. RESULTS: A total of

Management options of non-syndromic sagittal craniosynostosis.

There have been various effective surgical procedures for the treatment of non-syndromic sagittal craniosynostosis, but no definitive guidelines for management have been established. We conducted a study to elucidate the current state of practice and establish a warranted standard of care. An Internet-based study was sent to 180 pediatric neurosurgeons across the country and 102 craniofacial plastic surgeons in fourteen different countries, to collect data for primary indication for surgical management, preference for timing and choice of surgery, and pre-, peri-, and post-operative management

Defining pediatric neurosurgery in low-income countries: a cross-sectional study in Ethiopia.

OBJECTIVE: Pediatric neurosurgical practice is prevalent in most low- and lower-middle-income countries but lacks comprehensive documentation of practice patterns, demographics, and case variety. This study aimed to present the current state of pediatric neurosurgery in Ethiopia, including workforce characterization, case variety, and relevant procedures. METHODS: A survey was developed and distributed to all Ethiopian fully trained neurosurgeons (n = 50). Survey questions assessed sociodemographic variables, level of training, case variety, and neurosurgical practice. Statistical analysis wa

Emphasizing the Role of Neurosurgery Within Global Health and National Health Systems: A Call to Action.

Background: Worldwide, neurological disorders are the leading cause of disability-adjusted life years lost and the second leading cause of death. Despite global health capacity-building efforts, each year, 22.6 million individuals worldwide require neurosurgeon's care due to diseases such as traumatic brain injury and hydrocephalus, and 13.8 million of these individuals require surgery. It is clear that neurosurgical care is indispensable in both national and international public health discussions. This study highlights the role neurosurgeons can play in supporting the global health agenda, n

Survey of current neurotrauma treatment practice in Japan.

BACKGROUND: The Japanese Society of Neurotraumatology announced guidelines for management of severe traumatic brain injury (TBI) in 2000. To evaluate subsequent implementation of these guidelines, we investigated current severe TBI practices in Japan. METHODS: A questionnaire regarding management of severe TBI was sent to each of the 384 Japanese Neurosurgical Society specialist training medical centers and answers were received by mail from 233 centers (60.7%). RESULTS: Of the medical centers, 29% have neurosurgeons in their emergency department. Initial TBI treatment responsibility resided

Axial loading during MR imaging can influence treatment decision for symptomatic spinal stenosis.

BACKGROUND AND PURPOSE: Previous studies have shown that axial loading can narrow the spinal canal. However, the clinical significance is unclear. The purpose of this study was to determine whether the narrowing of the spinal canal with axial loading during MR imaging could influence treatment decision for spinal stenosis. METHODS: Two hundred patients with clinical symptoms of spinal stenosis underwent routine MR imaging and then immediately underwent axially loaded MR imaging. We selected 20 of these patients because they had narrowing of the spinal canal shown on the axially loaded images.

Present tense: a phenomenology of grief in neurosurgery.

This paper applies phenomenological insights to the neurosurgeon's lived experience of grief, approaching it not as a discrete psychological reaction, but as an enduring aspect of professional life shaped by repeated encounters with loss, complication, and responsibility. For neurosurgeons, grief is not limited to moments of patient loss but emerges gradually through the cumulative weight of adverse outcomes, moral uncertainty, and the recognition of medicine's limits. Despite its pervasiveness, grief is often treated as something to be managed, resolved, or privately endured. Drawing on pheno

Associating Surgeon Feedback With Material Physical Properties in the Development Process of a Resective Epilepsy Surgery Simulator.

BACKGROUND: Hands-on neurosurgical simulations, specifically techniques involving white matter disconnection, are underdeveloped owing to the paucity of low indentation materials that can adequately mimic brain dissection. OBJECTIVE: To describe the discovery phase of developing a resective epilepsy surgery simulator by quantifying the physical properties of 6 materials and correlating the scores with surgeon feedback data. METHODS: Six materials, silicone, TissueMatrix, gel support, Synaptive hydrogel, dry SUP706, and moist SUP706 of equal dimension, were evaluated for hardness by measuring

The Role of the Peripheral Nerve Surgeon in the Treatment of Pain.

Pain is a frequent cause of physician visits. Many physicians find these patients challenging because they often have complicated histories, emotional comorbidities, confusing examinations, difficult problems to fix, and the possibility of factitious complaints for attention or narcotic pain medications. As a result, many patients are lumped into the category of chronic, centralized pain and relegated to pain management. However, recent literature suggests that surgical management of carefully diagnosed generators of pain can greatly reduce patients' pain and narcotic requirements. This articl

A model for foramen ovale puncture training: Technical note.

BACKGROUND: Trigeminal neuralgia is a common cause of facial pain, characterized by shock-like pain affecting one or more branches of the trigeminal nerve. When conservative treatment fails and microdecompression is not indicated, percutaneous procedures are helpful. This percutaneous approach is done by a puncture up to the Gasserian ganglion, through the foramen ovale. Although simple and safe, this puncture demands some expertise from neurosurgeons. For that, a partnership between neurosurgeons and bio-engineers has developed a model for foramen ovale puncture, allowing practice for residen

Practises and controversies in the management of asymptomatic aneurysms: Results of an international survey.

BACKGROUND: Asymptomatic aneurysms that are increasingly discovered on cranial diagnostic imaging are a growing management dilemma. Large-scale studies have shown that in most instances, conservative management is appropriate for the majority of patients with aneurysms less than 7 mm in maximum diameter. It is unclear whether international practise mirrors practise in these large trials. OBJECTIVE: To determine how neurosurgeons around the world manage patients with asymptomatic aneurysms. METHODS: Electronic survey of 283 clinicians managing patients with aneurysms using a 55-item questionn

Differences between neurological and neurosurgical approaches in the management of malignant brain tumours.

The management and outcome in 205 patients diagnosed as having cerebral gliomas over five years were reviewed. Patients referred to neurologists and neurosurgeons had similar clinical features and similar results on computed tomography. Patients referred to neurologists underwent burr hole biopsy less often and had better short term morbidity than patients referred to neurosurgeons, although final outcome was the same in both groups. Few patients underwent other surgical procedures. Referral for radiotherapy was usually by neurosurgeons, although this did not significantly affect long term sur

Improving Neurosurgical Care in Afghanistan: Milestones, Challenges, and the Road Ahead.

BACKGROUND: Neurosurgical services are often lacking or nonexistent in low- and middle-income countries. Although there are limited data available on the current state of neurosurgery in Afghanistan, the existing data paint a dismal picture of inadequate clinical and research infrastructure. There are an estimated 74 neurosurgeons in the country (0.2 neurosurgeons per 100,000 people) working in 16 hospitals concentrated in urban centers, despite the fact that about 70% of the population live in rural areas. Technology seems to be a promising tool with which to enhance neurosurgical health syst
