[Orthostatic hypotension in patients with Parkinson's disease].

Orthostatic hypotension (OH) is a common vegetative symptom of Parkinson's disease (PD), which is predominantly neurogenic in nature. Detection and treatment of OH is of great importance, since it affects daily activity and increases the risk of falls. In the long term it damages target organs - the heart, kidneys and brain. In this regard, the review discusses the issues of classification, pathogenesis of OH, stages of diagnosis and correction of blood pressure, as well as measures for lifestyle changing, non-drug and drug treatment of orthostasis. Strategies for the management of patients wi

Association of transient orthostatic hypotension with falls and syncope in patients with Parkinson disease.

OBJECTIVES: To assess the frequency of transient orthostatic hypotension (tOH) and its clinical impact in Parkinson disease (PD), we retrospectively studied 173 patients with PD and 173 age- and sex-matched controls with orthostatic intolerance, who underwent cardiovascular autonomic function testing under continuous noninvasive blood pressure (BP) monitoring. METHODS: We screened for tOH (systolic BP fall ≥20 mm Hg or diastolic ≥10 mm Hg resolving within the first minute upon standing) and classic OH (cOH, sustained systolic BP fall ≥20 mm Hg or diastolic ≥10 mm Hg within 3 minutes upon stan

Unconscious confusion--a literature search for definitions of syncope and related disorders.

BACKGROUND: Imprecise definitions of syncope and related conditions appear common in the medical literature. To investigate the scope of the problem we systematically searched for definitions in high-ranking medical journals. METHODS: Literature review of articles on 'syncope', 'neurocardiogenic syncope', 'neurally mediated syncope', 'orthostatic intolerance', and 'orthostatic hypotension' with these keywords in the title, mainly published in the ten journals with the highest impact in the fields of cardiology, internal medicine, and neurology. RESULTS: Syncope, neurocardiogenic syncope, neu

To stand on one's own legs.

A fundamental human expectation is to stand upright. This exposes the cardiovascular system to gravitational forces, with a fall in pressure above heart level exposing organs such as the brain to impaired perfusion if adequate adaptive mechanisms are not activated. The autonomic nervous system plays an important role in the initial response to standing upright, and can be affected by several disorders, some rare, some common. Autonomic failure can result in orthostatic hypotension with hypoperfusion of vital organs, causing a variety of symptoms including syncope. Thus, it is important to reco

Orthostatic hypotension occurs frequently in the first hour after anesthesia.

UNLABELLED: Symptoms of orthostatic intolerance are common after general anesthesia and are associated with an increased risk of postoperative morbidity. The contribution of orthostatic hypotension (OH) has not been well defined. We conducted a head-up tilt test on patients after general anesthesia for minor surgery to assess the incidence of and risk factors for OH after general anesthesia. One-hundred-four patients were enrolled and were prospectively divided into four groups: older female, older male, young female, and young male. The incidence of OH was 76.0%, 72.0%, 45.5%, and 62.5% respe

[Postural tachycardia syndrome].

Erect posture could be a source of diverse chronic symptoms usually referred as orthostasis intolerance. Increasing number of patients suffering from orthostatic syndromes is recognized. Characterization and distinction of these orthostatic syndromes are based on the clinical presentation; including the typical history of orthostasis intolerance, and the unique hemodynamic responses on assuming upright posture. The diagnosis of postural tachycardia syndrome (POTS) requires orthostatic heart rate acceleration in excess of 120 beat/min, or an absolute increase of > or = 30 beat/min, in the absen

Effect of rowing ergometry and oral volume loading on cardiovascular structure and function during bed rest.

This study examined the effectiveness of a short-duration but high-intensity exercise countermeasure in combination with a novel oral volume load in preventing bed rest deconditioning and orthostatic intolerance. Bed rest reduces work capacity and orthostatic tolerance due in part to cardiac atrophy and decreased stroke volume. Twenty seven healthy subjects completed 5 wk of -6 degree head down bed rest. Eighteen were randomized to daily rowing ergometry and biweekly strength training while nine remained sedentary. Measurements included cardiac mass, invasive pressure-volume relations, maximal

Head-up tilt table test: how far and how long?

There is great variation between laboratories in the angle and duration of tilt used in the head-up tilt table test. We compared the hemodynamic response to three sequential randomized 20-min tilts of 60 degrees, 80 degrees and 90 degrees in 20 normal subjects. We studied 66 patients to determine the duration of tilt needed to document three types of orthostatic intolerance. Blood pressure and heart rate were monitored non-invasively. Thirty-nine patients with orthostatic hypotension of diverse causes and 17 patients with orthostatic tachycardia plus were tilted to 90 degrees for 10 min. Ten p

The Dermatological Manifestations of Postural Tachycardia Syndrome: A Review with Illustrated Cases.

Postural tachycardia syndrome (POTS) is a syndrome of excessive tachycardia with orthostatic challenge, and relief of such symptoms with recumbence. There are several proposed subtypes of the syndrome, each with unique pathophysiology. Numerous symptoms such as excessive tachycardia, lightheadedness, blurry vision, weakness, fatigue, palpitations, chest pain, and tremulousness are associated with orthostatic intolerance. Other co-morbid conditions associated with POTS are not clearly attributable to orthostatic intolerance. These include chronic headache, fibromyalgia, functional gastrointesti

Autonomic neural functions in space.

Autonomic neural functions are important to regulate vital functions in the living body. There are different methods to evaluate indirectly and directly autonomic, sympathetic and parasympathetic, neural functions of human body. Among various methods, microneurography is a technique to evaluate directly sympathetic neural functions in humans. Using this technique sympathetic neural traffic leading to skeletal muscles (muscle sympathetic nerve activity; MSNA) can be recorded from human peripheral nerves in situ. MSNA plays essentially important roles to maintain blood pressure homeostasis again

Role of the sympathetic nervous system in vasovagal syncope and rationale for beta-blockers and norepinephrine transporter inhibitors.

Vasovagal or neurocardiogenic syncope is a common clinical situation and, as with other entities associated with orthostatic intolerance, the underlying condition is a dysfunction of the autonomic nervous system. This article reviews various aspects of vasovagal syncope, including its relationship with orthostatic intolerance and the role of the autonomic nervous system in it. A brief history of the problem is given, as well as a description of how the names and associated concepts have evolved. The response of the sympathetic system to orthostatic stress, the physiology of the baroreflex syst

Orthostatic blood pressure control in Marfan's syndrome.

A 33-year old male patient, with Marfan's syndrome, reported symptoms of orthostatic intolerance and fatigue as a longstanding problem. Orthostatic cardiovascular examination showed poor orthostatic tolerance, with a rise in heart rate and a fall in arterial blood pressure and cerebral blood flow velocity. Self-discovered physical counterpressure manoeuvers improved symptoms, related to a substantial increase in arterial pressure and cerebral perfusion. When orthostatic complaint are reported by patients with Marfan's syndrome, physical counterpressure manoeuvers should be advised to reduce sy

COVID-19-Induced Postural Orthostatic Tachycardia Syndrome and Dysautonomia.

Postural orthostatic tachycardia syndrome (POTS) is a disorder characterized by orthostatic intolerance and, by definition, includes clinical symptoms of lightheadedness, palpitations, and tremulousness among others. It is considered a relatively rare condition that affects approximately 0.2% of the general population, and it is estimated that between 500,000 to 1,000,000 individuals in the United States have the condition and recently has been linked to post-infectious (viral) etiologies. We present a case of a 53-year-old woman who was diagnosed with POTS following extensive autoimmune worku

Preservation of veno-arteriolar reflex in the skin following 20 days of head down bed rest in humans.

A decrease in orthostatic tolerance following exposure to microgravity or head down bed rest (HDBR) is frequently observed and is thought to be multifactorial in origin. Recently published observations by Buckey et al. indicate that the inability to regulate peripheral vascular resistance during an orthostatic stress after spaceflight could be another important mechanism for orthostatic intolerance. Previous investigations have revealed that a local veno-arteriolar reflex is present in cutaneous, subcutaneous and muscle tissue. The reflex response is elicited in response to an increase in tran

Hypovolemic intolerance to lower body negative pressure in female runners.

PURPOSE: An attenuated baroreflex response and orthostatic intolerance have been reported in endurance-trained male athletes; however, it is still unknown whether this occurs also in females. The purpose of the present study was to examine whether endurance exercise-trained women had a predisposition to orthostatic compromise, and if so, what causative factor(s) may induce orthostatic intolerance. METHODS: We studied cardiovascular and hormonal responses to graded lower body negative pressure (LBNP) (0 to -60 mm Hg) in 26 middle-distance female runners (18.6 +/- 0.1 yr) as the exercise-traine

[Does constitutional hypotension exist?].

Orthostatic hypotension is frequently related to severe insufficiency of the autonomic nervous system associated with neuropathy or systemic disease like diabetes. Inversely, pre-syncopal orthostatic symptoms associated with mild drop in orthostatic blood pressure is quite often a reason to seek medical care, but is relatively unrecognized in the literature. Recently a syndrome of mild orthostatic intolerance has been defined, and seem to be quite common among young subjects, characterized by frequent orthostatic presyncopal symptoms associated with orthostatic tachycardia and high plasma cate

Severe orthostatic hypotension in otherwise uncomplicated Plasmodium vivax infection.

Impaired autonomic control of postural homeostasis resulting in orthostatic hypotension has been described in falciparum malaria. However, severe orthostatic intolerance in Plasmodium vivax has been rarely reported. A case of non-immune previously healthy Thai woman presenting with P. vivax infection with well-documented orthostatic hypotension is described. In addition to oral chloroquine and intravenous artesunate, the patient was treated with fluid resuscitation and norepinephrine. During hospitalization, her haemodynamic profile revealed orthostatic hypotension persisting for another three

Deglutition syncope to gastronomic satiety: a paediatric case report and update on therapeutic options.

We describe a rare and severe presentation of deglutition syncope, manifesting as complete heart block, along with newer therapeutic options, including cardioneural ablation and leadless pacing. Our 15-year-old patient presented with frequent syncope with swallowing, along with symptoms of orthostatic intolerance and anxiety. When standard non-pharmacologic and pharmacologic treatments were insufficient, cardioneural ablation resulted in improvement in syncope. Subsequent standard treatment of orthostatic intolerance has significantly improved the quality of life, including allowing her a more
