Otopathology of Egyptian mummy Pum II: final report.

Otoscopic examination of the ears of Egyptian mummy PUM II with the operating microscope revealed a small oval perforation of the right tympanic membrane in the posteroinferior quadrant. The smooth and concise margins suggested an etiology of middle ear infection. A method which allows decalcification of ancient specimens without disintegration of the eardrum and other soft tissues within the temporal bone was described. There was evidence of histological repair at the margins of the perforation consistent with sequela of otitis media. No other abnormalities were observed in the temporal bones

Audiological investigation of otitis media in children with atopy.

Otitis media (OM) describes an inflammatory process within the middle ear space that is generally associated with accumulation of fluid and that may lead to hearing loss, learning difficulties, and delays in language development. The pathogenesis of OM is multifactorial, involving the adaptive and native immune system, eustachian tube dysfunction, viral and bacterial load, and genetic and environmental factors. The involvement of IgE-mediated allergic reactions in the pathogenesis of OM has been suggested by clinical observations of a high prevalence of OM among patients with allergies. Eviden

Chondroma of the bony external auditory canal attached to the short process of the malleus.

A chondroma located in the external auditory canal is a rare finding. Here, we report the case of a 12-year-old girl who attended our clinic with complaints of a rustling sound in her left ear. Otoscopic examination revealed a tiny bone-like tumor protruding from the anterior wall of the bony external auditory canal, attached to the short process of the malleus. An audiogram revealed a conductive hearing loss in her left ear, although she did not notice it. After excision of the tumor through the ear canal, her complaints disappeared and her hearing recovered. Histologically, the tumor proved

[Aberrant intra-petrous internal carotid artery: apropos of a case].

This article illustrates a case of an aberrant pathway of the petrous portion of an internal carotid artery presenting as a cholesteatoma. This is a case of a 53 y.o man presenting with unilateral otalgia and vertigo. Otoscopic examination revealed a whitish retro-tympanic mass accompanied by a conduction hearing loss mimicking a cholesteatoma. An exploratory tympanotomy was performed. The surgical procedure was interrupted after needle aspiration of the mass suggesting an aberrant course of the petrous portion of the internal carotid artery. Authors report a review of the literature with an e

Influence of repetitive diving in saltwater on pressure equalization and Eustachian tube function in recreational scuba divers.

INTRODUCTION: We investigated in a prospective, observational trial the feasibility of using the Eustachian tube function test (ETFT) to measure the effect of repetitive pressure exposure during open seawater dives on Eustachian tube function. METHODS: The study included 28 adult divers during six consecutive days of diving in the Red Sea. Participants underwent otoscopy and ETFT before the first dive, between each dive and after the last dive. ETFT included regular tympanometry (R-tymp), tympanometry after Valsalva (V-tymp) and after swallowing (S-tymp). The R-tymp was obtained as 'baseline'

Eustachian tube balloon dilation surgery.

BACKGROUND: Eustachian tube dysfunction (ETD) is a common condition without an effective, accepted treatment. Balloon dilation Eustachian tuboplasty (BDET) has been recently proposed for the management of ETD. Previous studies of Eustachian tube function and treatment have not included a validated assessment of patient symptoms. We report for the first time the outcomes of BDET using a validated disease-specific symptom score, the 7-item Eustachian Tube Dysfunction Questionnaire (ETDQ-7). METHODS: Consecutive patients undergoing BDET were prospectively enrolled from an otolaryngology practice

Aberrant internal carotid artery causing objective pulsatile tinnitus and conductive hearing loss.

Aberrant internal carotid artery (ICA) in the middle ear is a rare, dangerous vascular anomaly and conservative follow-up was usually adopted in most reported cases. Here we report the case of an 8-year-old girl with symptoms of objective pulsatile tinnitus and conductive hearing loss in the right ear. Otoscopic examination, computed tomography, and conventional angiography were performed. An aberrant ICA combined with a 'third mobile window' was suspected preoperatively and confirmed at exploratory surgery of the middle ear. The aberrant ICA was treated, and the pulsatile tinnitus disappeared

Treatment of canine otitis externa using video otoscopy.

Otitis externa in 27 toy poodles and 40 miniature dachshunds was treated using a video otoscope. A distinct concavity (external tympanic concavity) was observed at the junction between the ventral part of the external surface of the tympanum and the ear canal to which a considerable amount of hair and debris had adhered. All hair and debris adhering to the external tympanic concavity were removed, and systemic antibiotic and antifungal agents were administered, after which all of the dogs recovered. The pattern of hair growth observed in the external tympanic concavity could be characterized a

Pediatric otosclerosis: case report and literature review.

Otospongiosis is an osteodystrophy of the temporal bone, characterized by disordered neoformation and deposition of bone, characterized by the presence of a progressive conductive, sensorineural or mixed hearing loss and tinnitus. Typically, otospongiosis presents as a slowly progressive conductive hearing loss in the third to fourth decade of life. Uncommonly children and adolescents may also have conductive or sensorineural hearing loss caused by otosclerosis. We describe a case of an 11-year-old patient, with progressive unilateral conductive hearing loss for 5 years. The otoscopic examinat

AI-Assisted Detection Support for Middle Ear Diseases Using Multimodal Large Language Models.

Middle ear diseases, such as otitis media and middle ear effusion, are difficult to accurately detect in primary care. We developed an AI-powered system using Azure OpenAI's GPT-4 Vision, the first multimodal large language model (LLM) applied to analyze otoscopic images and detect ten middle ear conditions. Built with a Model-View-Controller (MVC) architecture, the system processes images in under 5 seconds and provides bilingual (English and Chinese) detection reports with confidence scores and treatment recommendations. This scalable solution integrates advanced image analysis and natural l

Repair of a malleus-handle fracture using calcium phosphate bone cement.

We report a case of a 60-year-old woman who reported a popping sound in her left ear, induced by changes in pressure, since she pulled her index finger out of her ear canal after accidentally inserting it while washing her hair. Otoscopic examination revealed a wrinkled tympanic membrane and irregular malleus handle. An isolated malleus-handle fracture resulting from barotrauma was suspected. The fracture was repaired surgically using calcium phosphate bone cement. Postoperatively, the problem disappeared and her hearing recovered. This case is the first to demonstrate the efficacy of calcium

The application of acoustic impedance measurements to pediatric clinical practice.

This article provides background information on acoustic impedance testing and illustrates utilization of test results in pediatric clinical practice. Acoustic impedance measurement devices are presently being used successfully by hearing screening programs and by otorhinolaryngologists in clinical and hospital settings. As a diagnostic tool, the acoustic impedance bridge can have particular applicability in dealing withe young children and other difficult-to-test populations. Impedance measurements can provide physicians with objective information about the condition of the middle ear, oftent

[Exposed superior bulb of the internal jugular vein. Differential hearing loss diagnosis with pulse synchronous tinnitus].

A case report is presented of a 54-year-old woman with tinnitus synchronous with her pulse and an ipsilateral conductive hearing loss. Otoscopy showed a pulsatile structure behind the right eardrum. Imaging with high-resolution computed tomography of the temporal bone showed an enlarged jugular vein bulb. Magnetic resonance imaging could not be performed because of an implanted pacemaker. Since a glomus jugulare tumor could not be excluded right diagnostic tympanoscopy was performed and revealed a high jugular bulb that filled nearly a third of the tympanum.
