Impact of a multidisciplinary pain management team on acute and chronic pain management after total knee arthroplasty.

PURPOSE: The impact of multidisciplinary team (MDT) interventions on chronic postsurgical pain remains unclear. This study evaluated the effects of MDT on acute and chronic pain outcomes in total knee arthroplasty (TKA) patients. METHODS: This retrospective cohort study included 324 patients who underwent unilateral TKA between April 2017 and March 2023. The patients were divided into pre-MDT (n = 147) and post-MDT (n = 177) groups. The MDT, comprising anesthesiologists, pain nurses, and pharmacists, conducted daily rounds from postoperative day (POD) 1 to 4. The acute (duration of nerve bloc

Retrospective assessment of cancer pain management in an outpatient palliative radiotherapy clinic using the Pain Management Index.

CONTEXT: The Pain Management Index (PMI) is a simple index linking the usual severity of cancer pain with the category of medication prescribed to treat it. Medication categories are derived from the World Health Organization's "analgesic ladder" approach to cancer pain, and the PMI is an indicator of the extent to which the medication prescribed corresponds to the recommended categories for mild, moderate, and severe pain. OBJECTIVES: The aim of this study was to assess prevalence of inadequate pain management in an outpatient palliative radiotherapy clinic using the PMI. METHODS: All patie

Pain Management in Neurosurgery: Back and Lower Extremity Pain, Trigeminal Neuralgia.

Interventional anesthetic techniques are an integral component of a biopsychosocial approach and multidisciplinary treatment. Injection techniques are often used to diagnose disorders, decrease the need for surgery, or increase the time to surgery. The role of neural blockade techniques using local anesthetics and steroids in the assessment and treatment of pain continues to be refined. With the current opioid crisis and an aging population with increasing medical comorbidities, there is an emphasis on the use of nonopioid, nonsurgical, and multimodal therapies to treat chronic pain. This arti

Ongoing pain despite aggressive opioid pain management among persons with HIV.

BACKGROUND: Chronic pain is a common problem among persons living with HIV and opioids are frequently used in its treatment. However, data on the variables associated with opioids use and the efficacy of this practice are lacking. METHODS: We performed a cross-sectional cohort study of self-reported pain during the year 2005 in our clinic. Patients were grouped into 3 cohorts: those receiving daily opioid therapy for chronic pain (cohort 1, n=115), those with a chronic pain diagnosis but not on daily opioid therapy (cohort 2, n=209), and those without a chronic pain diagnosis (cohort 3, n=796

Comparison of a pain management program with usual care in a pain management center for people with spinal cord injury-related chronic pain.

OBJECTIVE: To evaluate the effectiveness of a specifically modified group-based, multidisciplinary cognitive-behavioral pain management program (PMP) in comparison to usual care in a clinical sample of people with spinal cord injury (SCI)-related chronic pain. METHOD: Changes over time on a range of measures, including usual pain intensity, pain catastrophizing and self-efficacy, mood, SCI acceptance and self-efficacy, and interference due to pain were examined in 2 groups with SCI-related chronic pain after a multidisciplinary pain assessment. One group (n=19) attended the PMP, and the other

Acute Pain Management of Chronic Pain Patients in Ambulatory Surgery Centers.

PURPOSE OF REVIEW: With the widespread growth of ambulatory surgery centers (ASCs), the number and diversity of operations performed in the outpatient setting continue to increase. In parallel, there is an increase in the proportion of patients with a history of chronic opioid use and misuse undergoing elective surgery. Patients with such opioid tolerance present a unique challenge in the ambulatory setting, given their increased requirement for postoperative opioids. Guidelines for managing perioperative pain, anticipating postoperative opioid requirements and a discharge plan to wean off of

Pain management after cardiac surgery: are we underestimating post sternotomy pain?

OBJECTIVE: Good pain management can improve the outcome of patient care after cardiac surgery. The intensity of pain after cardiac surgery is often underrated. Inadequate pain control can result in increased morbidity and length of hospital stay as well as lead to chronic pain. Therefore, the authors conducted a study to identify the prevalence and risk factors of moderate to severe pain after cardiac surgery including treatment and complication. MATERIAL AND METHOD: The present study was prospectively performed in the patients undergoing cardiac surgery with median sternotomy in Siriraj Hosp

Focused review of interdisciplinary pain rehabilitation programs for chronic pain management.

Interdisciplinary pain rehabilitation programs (IPRPs) are based on a functional restoration approach to treating complex chronic pain conditions. With a greater appreciation for a biopsychosocial approach to more effectively manage patients with chronic pain has come the development of more comprehensive treatment programs with less of a biomedical emphasis (i.e., interventional therapy, unimodal physical therapy, and passive modalities) and more of a biopsychosocial one. Interdisciplinary programs involve the use of multiple disciplines such as physical and occupational therapy, pain psychol

Retrospective evaluation of donor pain and pain management after laprascopic nephrectomy.

PURPOSE: The purpose of this study was to evaluate donor pain and pain management beginning immediately postoperatively until hospital discharge. METHODS: All kidney donors were included from 2008 and 2009. Demographic data, operative data, pain scores in the postanesthesia care unit, and visual analog pain scale (VAS) scores were collected for each patient. Standardization for comparison was made by converting doses to intravenous morphine equivalents (ME). RESULTS: Eighty-five patients were identified as donors, all of which underwent laparoscopic nephrectomy. Daily analgesic requirement w

Managing the neuroinflammatory pain of endometriosis in light of chronic pelvic pain.

INTRODUCTION: Endometriosis affects 5% to 10% of reproductive age women and may be associated with severely painful and debilitating symptoms as well as infertility. Endometriosis involves hormonal fluctuations, angiogenesis, neurogenesis, vascular changes and neuroinflammatory processes. The neuroinflammatory component of endometriosis makes it a systemic disorder, similar to other chronic epithelial inflammatory conditions. AREAS COVERED: Inflammatory mediators, mast cells, macrophages, and glial cells play a role in endometriosis which can result in peripheral sensitization and central sen

Psychiatry and Pain Management: at the Intersection of Chronic Pain and Mental Health.

PURPOSE OF REVIEW: Chronic pain impacts millions of people in the USA. At the heart of the problem of chronic pain remains the complex psychosocial aspects associated with living with chronic pain. Given the overlap between chronic pain and mental health, a promising treatment approach is to improve how we integrate psychiatry into pain management. RECENT FINDINGS: Treatment of chronic pain and comorbid mental health issues requires a multidisciplinary approach. Advancements in how pain is understood, especially centralized pain, have helped inform both pharmacological and behavioral interven

Pain and pain management in hospitalized patients before and after an intervention.

BACKGROUND AND AIM: Studies have shown that pain is common among hospitalized patients and that there is a lack of compliance with pain management guidelines. Improving pain management does not only involve developing new drugs or technology; even more important is an effective organisation that utilises existing expertise. The aim of this study was to investigate whether pain in hospitalized patients can be reduced by implementing evidence-based pain management guidelines, providing education for staff and an organisation that includes pain responsibility nurses. METHODS: A cross-sectional s

Assessment and management of pain syndromes and arthritis pain in children and adolescents.

Chronic musculoskeletal pain, whether it is idiopathic or disease-related, is common in childhood. Pediatric rheumatologists and other pediatric health care providers must understand the epidemiology of musculoskeletal pain as part of childhood, diagnose pain syndromes in children and rule out rheumatic disease, and be willing to initiate treatment of pain in children and adolescents. Practitioners' ability to carry out these tasks is enhanced by an awareness of the biopsychosocial model of pain, which integrates biologic, environmental, and cognitive behavioral mechanisms in describing the ca

[Optimization of perioperative pain management: a role for a Transitional Pain Service].

There is a concern about the increasing use of opioids for non-cancer-related pain. Persistent pain after surgery may contribute to the prevalence of increasing (long-term) opioid use. Identification of patients at increased risk of acute and chronic postsurgical pain and long-term opioid use is suboptimal. This results in a lack of early prevention and optimal treatment of acute and subacute pain in this group. Furthermore, responsibility for postoperative pain after discharge takes places fragmented by both the general practitioner and surgeon. The implementation of a Transitional Pain Servi
