During pathological examinations of amyotrophic lateral sclerosis (ALS) patients, we noticed that bundles of dense small myelinated fibers were relatively well preserved in the anterior marginal areas of the cervical cords in a few ALS patients with diffuse myelin pallor. AIMS: In optic nerve hypoplasia (ONH), the extent of functional loss of retinal ganglion cells cannot be determined by ophthalmoscopic examination. The prognostic value of visual electrodiagnostic tests in infants and toddlers with ONH was assessed by comparison with visual outcome.
METHODS: 85 participants with ONH had electroretinogram (ERG) and visual-evoked potential (VEP) testing to flash and to pattern-reversal checks and ocular fundus photography prior to 36 months of age. These initial measures were compared with visual acuity outcomes at 5 years of age in the better-seeing eye.

Histopathological diffuse pallor of the cerebral white matter on myelin staining is a characteristic feature of progressive subcortical vascular encephalopathy of the Binswanger type (PSVE). A previously healthy 9-month-old boy presented to the emergency department with sudden onset of pallor and reduced activity. A detailed history and initial blood tests revealed the diagnosis of oxidant stress-induced hemolysis caused by glucose-6-phosphate dehydrogenase deficiency. However, the child also had a second diagnosis that was revealed with additional testing. This case is described to illustrate the dual diagnosis and management guidelines for both potentially serious conditions.

The terminology of the acro-ischaemic states of the upper limb is muddled. It would be desirable for a strict definition of Raynaud's "phenomenon", "syndrome" and "disease" to be established and accepted by all angiologists. In the author's view, Raynaud's phenomenon is an intermittent circulatory crisis, leaving after it a normal finger. When set off by a detectable organic lesion, the phenomenon then belongs to the category of Raynaud's syndrome. When there is no organic lesion to explain it, it becomes a symptom of Raynaud's disease.

Visceral Leishmaniasis (also known as Kala Azar) is a parasitic infection caused by Leishmania Donovani in the East and Leishmania Infantum in the west. It is prevalent in many countries including India, Bangladesh, Nepal, Africa and part of America. The disease follows chronic course and is usually lethal if left untreated. It has also been reported from different parts of Pakistan, including Northern areas, Districts Mansehra & Abbottabad and Hilly areas of Khyber Pukhtoonkhawah.
