Exploring sociodemographic factors and coping strategies before and during the COVID-19 pandemic among paramedics working in a Middle Eastern country.

BACKGROUND: Paramedics often work under pressure, making them vulnerable to burnout and depression, but the Coronavirus disease (COVID-19) pandemic was an unprecedented crisis in modern times. This study examines the coping strategies employed by paramedics in Qatar's national Ambulance Service prior to and during the COVID-19 pandemic. METHODS: In early 2021, we conducted an anonymous online survey using a modified version of Carver's Brief COPE scale, a validated tool measuring coping mechanisms. The survey was shared with 1,100 paramedics. RESULTS: The data from 272 valid responses reveal

What do community paramedics in Germany do regarding the care of older people? A retrospective, descriptive analysis of low-acuity cases.

BACKGROUND: Non-life-threatening cases treated by emergency services have been increasing in recent years, especially in older people. In a region in Germany with approximately 600,000 inhabitants, the role of a specially trained community paramedic (Gemeindenotfallsanitäter, G-NFS) was introduced in 2019. The G-NFS is dispatched to low-acuity requests, attends the assignment alone and is allowed to treat patients at home. AIM: The aim of this study was to analyse the assignments attended by the G-NFS relating to the suspected diagnoses, with a focus on older people (≥ 65 years) according to

Efficacy and safety of intravenous sufentanil administration in low-severity acute trauma as a competence of paramedics: a follow-up observational study.

BACKGROUND AND IMPORTANCE: In the Czech Republic, paramedics are required to consult a physician before administering intravenous opioids, which may delay effective prehospital pain management. As paramedic competencies expand in Europe, it is important to evaluate the safety and efficacy of independent opioid administration in prehospital emergency care settings. OBJECTIVES: To assess the safety and effectiveness of intravenous sufentanil administered independently by trained paramedics compared to administration following remote physician consultation in adult trauma patients. DESIGN: Pros

Clinical evaluation of the use of laryngeal tube versus laryngeal mask airway for out-of-hospital cardiac arrest by paramedics in Singapore.

INTRODUCTION: It remains unclear which advanced airway device has better placement success and fewer adverse events in out-of-hospital cardiac arrests (OHCAs). This study aimed to evaluate the efficacy of the VBM laryngeal tube (LT) against the laryngeal mask airway (LMA) in OHCAs managed by emergency ambulances in Singapore. METHODS: This was a real-world, prospective, cluster-randomised crossover study. All OHCA patients above 13 years of age who were suitable for resuscitation were randomised to receive either LT or LMA. The primary outcome was placement success. Per-protocol analysis was

Trends in the Application of Invasive Prehospital Interventions by Paramedics: A Six-Year Descriptive Analysis of the German Emergency Medical Services System.

BACKGROUND: The Emergency Paramedic Act (EPA; Notfallsanitätergesetz), implemented in 2014, expanded the legal authority of German paramedics to independently perform defined invasive and pharmacological interventions under specific conditions. However, real-world implementation of these competencies within the physician-based German Emergency Medical Services (EMS) system remains insufficiently described. OBJECTIVE: The aim of this study was to evaluate the frequency, type, temporal development, and regional variation of invasive and pharmacological/therapeutic interventions performed indepe

Identifying and profiling prearrival characteristics of avoidable emergency department visits transported by paramedics: a cohort study using linked prehospital and hospital data.

BACKGROUND: Increasing demand and crowding in emergency departments (EDs) remain persistent challenges for healthcare systems worldwide. A portion of these visits is avoidable, indicating they could have been effectively managed in non-ED care settings. There has been increased attention on paramedics redirecting avoidable visits to community-based care before ED transport. However, limited evidence exists to identify which patients might be suitable for non-ED care models, particularly based on prehospital clinical presentations. This study aimed to examine the patient characteristics associa

Posttraumatic stress disorder symptomology as measured by PCL-5 and its relationships to resilience, hostility and stress among paramedics and social professionals.

This survey study aimed to contribute to the extensive debate on the dimensionality of the Posttraumatic Stress Disorder Checklist for the DSM-5 (PCL-5) questionnaire by examining the psychometric properties and construct validity of its Hungarian version and relying on the inspection of a conceptual network of related variables, that is, perceived stress, hostility, and resilience. Confirmatory factor analysis (CFA), exploratory structural equation modelling (ESEM) and path analysis were applied on data collected from 177 paramedics and 66 professionals from the social field (58.4% male; Mage

Comparison of Prehospital Assessment by Paramedics and In-Hospital Assessment by Physicians in Suspected Stroke Patients: Results From 2 Prospective Cohort Studies.

BACKGROUND: It is unknown if ambulance paramedics adequately assess neurological deficits used for prehospital stroke scales to detect anterior large-vessel occlusions. We aimed to compare prehospital assessment of these stroke-related deficits by paramedics with in-hospital assessment by physicians. METHODS: We used data from 2 prospective cohort studies: the LPSS (Leiden Prehospital Stroke Study) and PRESTO study (Prehospital Triage of Patients With Suspected Stroke). In both studies, paramedics scored 9 neurological deficits in stroke code patients in the field. Trained physicians scored t

Mental health acts and out-of-hospital care: Legislative boundaries and the paramedic role in Australian mental health legislation.

BACKGROUND: Changes to Australian mental health legislation have expanded the role of paramedics in managing mental illness. Paramedics in most Australian states and territories can now temporarily detain and transport people suspected of being mentally ill for further assessment if these people are viewed as being at risk or as posing a risk to others and as lacking capacity for decision making. AIM: This paper examines current mental health legislation from the eight Australian states and territories. We explore the conditions for transport to involuntary care and additional powers granted

Prevalence of anxiety, depression, and post-traumatic stress disorder among paramedic students: a systematic review and meta-analysis.

PURPOSE: There are elevated mental health concerns in paramedic students, but estimates vary between studies and countries, and no review has established the overall prevalence. This systematic review addressed this by estimating the global prevalence of common mental health disorders, namely anxiety, depression, and post-traumatic stress disorder (PTSD), in paramedic students internationally. METHODS: A systematic search of six databases, including MEDLINE, EMBASE, PsycINFO, CINAHL, Scopus, and medRxiv, was conducted to identify studies relating to mental health among paramedicine students.

Impact of cardiopulmonary resuscitation duration on survival from paramedic witnessed out-of-hospital cardiac arrests: An observational study.

BACKGROUND: Resuscitation guidelines often recommend ongoing cardiopulmonary resuscitation (CPR) efforts to hospital for out-of-hospital cardiac arrests (OHCA) witnessed by emergency medical service (EMS) personnel. In this study, we examine the relationship between EMS CPR duration and survival to hospital discharge in EMS witnessed OHCA patients. METHODS: Between January 2003 and December 2011, 1035 adult EMS witnessed arrests of presumed cardiac aetiology were included from the Victorian Ambulance Cardiac Arrest Registry. CPR duration was defined as the total sum of prehospital CPR time in

Protection of medical and paramedical university students in Lebanon against measles, mumps, rubella and varicella: active measures are needed.

OBJECTIVE: In many countries, universities require students to either show a physician-certified proof of immunity or to get vaccinated against measles, mumps, rubella and varicella, prior to their registration in medical and paramedical majors. The objective of this study was to evaluate the need to implement this policy in Lebanon. DESIGN: A cross-sectional study was performed on students of the Lebanese University (LU), faculties of Medicine, Dentistry, Pharmacy and Public Health. METHODS: The serological immunity status was assessed by determining specific antibody titer and the disease

Accuracy of final-year paramedic students' ECG interpretation and referrals for transient loss of consciousness, per the NICE CG109 guidelines.

Paramedics in the United Kingdom independently assess patients with transient loss of consciousness (TLOC) in community settings. The UK's National Institute for Health and Care Excellence publishes clinical guidelines on assessment of TLOC (NICE CG109), recommending ECG interpretation to identify red-flag abnormalities that warrant referral. This study assessed whether final-year university paramedic students meet these recommendations uniformly across different ECG patterns. Our hypothesis was that participants will identify the Brugada syndrome (a required but difficult-to-recognize ECG pat

Development and Evaluation of the Advanced Joint Airway Management System for Educational Utility in Endotracheal Intubation, as Assessed by Expert Paramedic Instructors.

INTRODUCTION: Between 2011 and 2014, The Combat Casualty Training Consortium research study sought to evaluate all aspects of combat casualty care, including mortality, with a special focus on the incidence and causes of potentially preventable deaths among U.S. combat fatalities. This study identified a major training gap in critical airway management. To address this gap, the Advanced Joint Airway Management System (AJAMS) was designed and assessed for physical fidelity and educational utility in a population of paramedic instructors. MATERIALS AND METHODS: Paramedic instructors served as p

The association of ketamine induction with blood pressure changes in paramedic rapid sequence intubation of out-of-hospital traumatic brain injury.

INTRODUCTION: Rapid sequence intubation (RSI) is used to secure the airway of traumatic brain injury (TBI) patients, with ketamine frequently used for induction. Studies show that ketamine-induction RSI might cause lower blood pressures when compared to etomidate. It is not clear if the results from that research can be extrapolated to systems that use different dosing regimens for ketamine RSI. Ambulance Victoria authorized the use of 1.5 mg/kg ketamine in January 2015 for head injury RSI induction by road-based paramedics. This study aims to examine whether systolic blood pressure changed wh
