Avulsion of the patellar ligament with combined fracture luxation of the proximal tibial epiphysis: case report and review of the literature.

Disruption of the extensor mechanism of the knee joint at the insertion site of the patellar tendon into bone is a somewhat common injury. Avulsion of the patellar ligament associated with fractures of the proximal tibial epiphysis is an extremely rare injury. Only three cases have been published in association with avulsion fracture of the tibial tubercle. We are reporting a case that has previously not been reported to our knowledge: an avulsion of the ligament in a closed fracture of the proximal tibial epiphysis with integrity of the tibial tubercle. We propose a three-type classification

Medial Patellofemoral Ligament Reconstruction During Primary Total Knee Arthroplasty in Patients With Pre-existing Patellar Instability: Surgical Technique.

The medial patellofemoral ligament (MPFL) is vital for patellar stability and tracking within the trochlear groove in early flexion. Although MPFL reconstruction is a well-established treatment for patellar instability in native knees, its role during primary total knee arthroplasty (TKA) remains incompletely defined. This manuscript describes a technique for concurrent MPFL reconstruction during primary TKA in select group of patients with a documented history of chronic patellar instability and persistent intraoperative maltracking despite appropriate component positioning and soft tissue ba

Anterior knee pain in anterior cruciate ligament reconstruction using bone-patellar tendon-bone graft and autologous bone grafting.

BACKGROUND: Anterior cruciate ligament (ACL) reconstruction using bone-patellar tendon-bone (BPTB) autografts remains the gold standard for young, active individuals due to its superior biomechanical strength and bone-to-bone healing. However, donor site morbidity, particularly anterior knee pain (AKP), limits its utilization despite its advantages. Various techniques have been proposed to reduce AKP, but they show variable outcomes and several limitations. AIM: To assess the incidence and severity of AKP following BPTB ACL reconstruction using an autologous bone grafting technique. METHODS:

Closing-wedge distal femoral osteotomy combined with medial patellofemoral ligament reconstruction for recurrent patellar dislocation with genu valgum.

BACKGROUND: Medial patellofemoral ligament reconstruction (MPFLR) is a well-established procedure for addressing recurrent patellar dislocation (RPD) in young patients. However, despite being a promising procedure for RPD with genu valgum, there is a scarcity of reports on simultaneous MPFLR and closing-wedge distal femoral osteotomy (CWDFO). The purpose of the present study was to observe and analyse the clinical and imaging findings of CWDFO combined with MPFLR for RPD with genu valgum. METHODS: From May 2015 to April 2018, 25 patients with RPD and genu valgum were surgically treated in our

Evaluation of Graft Tensioning Effects in Anterior Cruciate Ligament Reconstruction between Hamstring and Bone-Patellar Tendon Bone Autografts.

This article investigates the clinical, functional, and radiographic outcomes in anterior cruciate ligament (ACL) reconstruction patients over 7 years to determine the effects of initial graft tension on outcomes when using patellar tendon (bone-tendon-bone [BTB]) and hamstring tendon (HS) autografts. Ninety patients, reconstructed with BTB or HS, were randomized using two initial graft tension protocols: (1) normal anteroposterior (AP) laxity ("low-tension"; n = 46) and (2) AP laxity overconstrained by 2 mm ("high-tension"; n = 44). Seventy-two patients had data available at 7 years, with 9 e

Tibial Plateau Fracture Following Anterior Cruciate Ligament Reconstruction with a Bone-Patellar Tendon-Bone Allograft: A Case Report.

CASE: A 36-year-old woman sustained a medial tibial plateau fracture involving a tibial tunnel that had been used 4 years prior for an anterior cruciate ligament (ACL) reconstruction with a bone-patellar tendon-bone (BPTB) allograft in the same knee. At 26 months following open reduction and internal fixation of the tibial plateau fracture (6 years following the index ACL reconstruction), the patient returned to full activity. CONCLUSION: To our knowledge, this is the first report of a tibial plateau fracture following ACL reconstruction with a BPTB allograft, which adds to the paucity of lit

Patellar Redislocation Rates and Clinical Outcomes After Medial Patellofemoral Ligament Reconstruction: Suture Anchor Versus Double Transpatellar Tunnel Fixation.

BACKGROUND: Few studies to date have compared clinical outcomes in patients who have undergone medial patellofemoral ligament (MPFL) reconstruction using the suture anchor and double transpatellar tunnel fixation methods. This meta-analysis therefore compared the clinical results, including the patellar redislocation rate and improvement in functional scores, of suture anchor and double transpatellar tunnel fixation. HYPOTHESIS: The recurrence rate and improvement in functional outcomes after surgery would be similar using the suture anchor and double transpatellar tunnel fixation methods. S

Surgical medial patellofemoral ligament reconstruction versus non-surgical treatment of acute primary patellar dislocation: a prospective controlled trial.

PURPOSE: The acute primary (first-time) lateral patellar dislocation is associated with a high rate of functional disability. There is no consensus as to the choice of surgical or non-surgical treatment for these patients. The aim of this study is to compare the clinical results between the surgical (reconstruction of the medial patellofemoral ligament [MPFL]) and non-surgical treatments for acute primary patellar dislocations. METHODS: Sixty-nine skeletally mature patients (69 knees) were included in this prospective non-randomized controlled trial. At least one predisposing factor for patel

Impaired Quadriceps Rate of Torque Development and Knee Mechanics After Anterior Cruciate Ligament Reconstruction With Patellar Tendon Autograft.

BACKGROUND: Rate of torque development (RTD) measures the ability of a muscle to produce torque quickly. Decreased quadriceps RTD may impair performance of sporting tasks after surgery. Currently, little is known about variations in quadriceps RTD between anterior cruciate ligament (ACL)-reconstructed and noninjured limbs. PURPOSE: To determine the differences in RTD of the quadriceps, the rate and timing of knee extensor moment (KEM) development, and knee flexion excursion during running after ACL reconstruction with patellar tendon autograft. Patellar instability factors in isolated medial patellofemoral ligament reconstructions--what does the literature tell us? A systematic review.

BACKGROUND: Many studies have demonstrated good results after medial patellofemoral ligament (MPFL) reconstruction for patients with patellar instability. The applicability of published studies to the clinical decision-making process for the individual patient with patellar instability, however, is not well elucidated. HYPOTHESIS: There is inconsistency in the reporting of preoperative and postoperative variables, which limits the applicability of current studies to patients with patellar instability. STUDY DESIGN: Systematic review. METHODS: A systematic review of the literature was conduc

Patellofemoral chondral status after medial patellofemoral ligament reconstruction using second-look arthroscopy in patients with recurrent patellar dislocation.

BACKGROUND: Most patients with recurrent patellar dislocation show cartilage damage in the patellofemoral joint. Medial patellofemoral ligament reconstruction has become one of the most important surgical techniques for treating recurrent patellar dislocation. However, patellofemoral chondral status after this reconstruction has not been elucidated. The purpose of this study was to investigate the effects of medial patellofemoral ligament reconstruction on articular cartilage in the patellofemoral joint by comparing the arthroscopic chondral status at the time of reconstruction with that at se

Compact platelet-rich fibrin scaffold to improve healing of patellar tendon defects and for medial collateral ligament reconstruction.

BACKGROUND: Platelets are one of the most biocompatible and cost-effective sources of growth factors. Attention is being paid to autologous platelets and platelet-rich plasma. We developed a novel compact platelet-rich fibrin scaffold (CPFS) that was produced from blood and calcium gluconate only. The objective of this study was to investigate the potential of CPFS as a provisional scaffold in two rabbit models. METHODS: In the first rabbit model, the central half of the patellar tendon was resected bilaterally. Allogenic CPFS was attached to the defect in the right knee, while the left knee
