Transaxillary thoracodorsal artery perforator flap: a versatile new technique for hypopharyngeal reconstruction.

INTRODUCTION: Reconstruction of the hypopharynx and cervical esophagus after tumor resection remains a challenging problem. Different techniques of reconstruction have been described for such defects. Although free flap reconstruction, including jejunum and fasciocutaneous flaps, has gained popularity for this complex defect, regional flaps are still in use worldwide. PATIENTS AND METHODS: We present our experience using the pedicled thoracodorsal artery perforator (TDAP) flap whereby only a small cuff of latissimus dorsi muscle around the perforators is harvested. The remaining muscle with i

Angular artery perforator-based transposition flap for the reconstruction of midface defect.

BACKGROUND: Midface defect is often seen due to trauma, neoplasm, and infection. It is important to avoid tension and distortion of the local structures and to provide skin color and texture match. We present the technique of angular artery perforator-based transposition flap for small to moderate size defect of the midface. MATERIALS AND METHODS: Between 2002 and 2009, 17 patients underwent operations for the reconstruction of midface defect. After tumor excision, perforators were found at the medial aspect of the defect along the nasolabial fold. Considering the change of the defect shape a

The deep inferior epigastric artery perforator (DIEAP) flap for total glossectomy reconstruction.

PURPOSE: Total or subtotal glossectomy following the resection of intraoral tumors causes significant morbidity. However, which reconstructive technique is the most successful remains controversial. PATIENTS AND METHODS: After approval by the Ethics Committee, charts were reviewed retrospectively for patients treated at the Oral and Maxillofacial Surgery Department, University Hospital La Paz (Madrid, Spain), during a 3-year period (2005-2008). All were reconstructed with a deep inferior epigastric artery perforator (DIEAP) flap after total glossectomy. Data collected included affiliation dat

An ideal method for pressure sore reconstruction: a freestyle perforator-based flap.

BACKGROUND: Pressure sore reconstruction is quite difficult for plastic surgeons because of long-term high recurrence rates. We designed a freestyle perforator-based flap for pressure sore reconstruction considering pressure sore recurrence and further reconstruction. METHODS: We used a handheld Doppler device to locate a perforator position just adjacent to the pressure ulcer. In a series of 34 patients, we used 37 perforator-based flaps to reconstruct 25 sacral, 5 ischial, and 4 trochanteric ulcers. RESULTS: Twenty-eight of 37 flaps healed uneventfully without complication. One patient had

Medial plantar artery perforator flap for soft-tissue reconstruction of the heel.

Soft-tissue wounds of the foot and especially the heel are challenging problems for reconstructive surgeons. An important principle that guides heel reconstruction is to provide sensate skin with a similar thickness to resurface the weight-bearing heel and avoid late flap ulceration. Among various techniques to achieve this result, the sensate medial plantar perforator flap is an excellent option, which provides durability to friction, a cushioning effect, and sensation. An anatomic study was performed to clarify the anatomy of the cutaneous perforators of the medial plantar artery and to dete

Application of various compositions of thoracodorsal perforator flap for craniofacial contour deformities.

BACKGROUND: Craniofacial contour defects are challenging to restore because they may involve multiple tissues and span several aesthetic subunits in a non-contiguous manner. Some of these deformities may be associated with significant dead space in the region of sinus and orbit. The numerous subtle contours of the craniofacial regions must be preserved or restored to achieve a pleasing outcome. PATIENT AND RESULTS: We managed six patients with various craniofacial contour deformities as a result of hemifacial microsomia, infection, post excision of venous malformation, lipodystrophy, craniect

Pre-expanded pedicled thoracodorsal artery perforator flap for postburn axillary contracture reconstruction.

INTRODUCTION: The axillary region is one of the sites most frequently affected by postburn contractures. In this clinical study, we used pre-expanded pedicled thoracodorsal artery (TDA) perforator flaps for release of postburn contracture of the axillary region. PATIENT AND METHODS: Five patients with severe axillary burn contractures were reconstructed with six pre-expanded pedicled TDA perforator flaps between 2008 and 2010. All were men ranging in age from 20 to 26 years (mean, 22 years). Mean time of follow-up was 12 months. Flap and donor site complications, preoperative, and postoperati

Peroneal artery perforator chimeric flap for reconstruction of composite defects in extremities.

Large bone defects of extremities, especially those associated with soft tissue defects, represent difficult reconstructive problems. Chimeric flap is a suitable option for reconstruction of complex bone and soft-tissue defects. In this report, we present the experience on use of the peroneal artery perforator chimeric flap for the reconstruction of complex bone and soft tissue defects in the extremities in 16 patients. The bone defects were located in the tibia in 8 patients, in both tibia and fibula in 1 patient, in the ulna in 2 patients, in both ulna and radius in 2 patients, and the metat

[Free peroneal artery perforator sural neurocutaneous flap for hand and foot defects].

OBJECTIVE: To report the operative techniques and clinical results of the free peroneal artery perforator sural neurocutaneous flaps. METHODS: Free sural neurocutaneous flap with a single unit of perforating veins and artery arising from the peroneal artery was designed to repair skin defects of hand or from the ankle to distal end of the foot. After the flap was transposed to the defect, the perforating artery was anastomosed with a branch of a nearby artery, and the small saphenous vein was anastomosed with the cephalic vein or the great saphenous vein to establish the flap's circulation. Inferior gluteal artery perforator flap: a viable alternative for ischial pressure sores.

The ischial area is by far the most common site for pressure sores in wheelchair-bound paraplegic patients, because most of the pressure of the body is exerted on this area in the seated position. Even after a series of successful pressure sore treatments, the site is very prone to relapse from the simplest everyday tasks. Therefore, it is crucial to preserve the main pedicle during primary surgery. Several surgical procedures, such as myocutaneous flap and perforator flap, have been introduced for the treatment of pressure sores. During a 4-year time period at our institute, we found favourab

Thoracodorsal artery perforator flap: a versatile alternative for various soft tissue defects.

Thoracodorsal artery perforator (TDAP) flap is a relatively new member of the perforator flap family. The objective of this study is to describe the use of pedicled and free TDAP flaps for various soft tissue defects. Fifteen patients underwent soft tissue reconstruction using 16 TDAP flaps. Twelve pedicled flaps were used for axillary, breast, and shoulder regions. Four free flaps were used for cheek, popliteal, hand, and foot reconstruction. The flaps were harvested based on the perforators, which were preoperatively located at or close to a point 8 cm below the posterior axillary fold and 2

Muscle herniation: a complication at the anterolateral thigh perforator flap donor site.

The anterolateral perforator flap is becoming the flap of choice for a wide variety of complex defects. It has been known to provide excellent donor site morbidity even in cases where vastus lateralis is included within the flap. We report a case of herniation of the vastus lateralis and rectus femoris muscles through the overlying fascia, following ALT perforator flap harvest. In this case muscle herniation at the donor site required surgical repair.

Medial sural artery perforator flap for tongue and floor of mouth reconstruction.

BACKGROUND: The radial forearm flap is frequently considered the first choice for tongue reconstruction, but the disadvantages of donor site morbidity are well known. The search for another thin skin flap as an alternative has led to the application of the medial sural artery perforator flap. METHODS: We used 12 medial sural artery perforator flaps to reconstruct tongue and floor of mouth following cancer ablation. We paid attention to the major perforator (vein >or= 1 mm) as the vascular relay. RESULTS: Most flaps were raised with a single perforator. The size of the skin paddle varied from

Latissimus dorsi muscle and its short perforator-based skin compound free flap.

Some authors have described the latissimus dorsi muscle and its short perforator-based skin compound flap based on the same thoracodorsal vessels. This flap procedure involves separating the skin island from the underlying latissimus dorsi muscle and rotating the skin island over the musculocutaneous perforator emerging from the latissimus dorsi muscle. As with all surgical procedures, there are various advantages and disadvantages. However, to the best of the authors' knowledge, there are no reports on the use of the flap in a consecutive series. Between 1997 and 2005, the flap was used to re

The in-the-crease inferior gluteal artery perforator flap for breast reconstruction.

BACKGROUND: Perforator free flaps harvested from the abdomen or buttock are excellent options for breast reconstruction. They enable the reconstructive surgeon to recreate a breast with skin and fat while leaving muscle at the donor site undisturbed. The gluteal artery perforator free flap using buttock tissue was first introduced by the authors' group in 1993. Of the 279 gluteal artery perforator flaps, the authors have performed for breast reconstruction, 220 have been based on the superior gluteal artery and 59 have been based on the inferior gluteal artery. The authors have found that for

Radiation effects on breast reconstruction with the deep inferior epigastric perforator flap.

Immediate breast reconstruction has important advantages over delayed reconstruction, including a shorter operative time and decreased psychological distress for the patient. However, the authors' experiences with the deep inferior epigastric perforator flap demonstrate variable aesthetic outcomes among patients who undergo radiation postoperatively. To establish an association between radiation and poor postoperative results, a matched-pairs analysis was conducted. Thirty irradiated patients were paired with 30 nonirradiated patients, according to age and body mass index. For each group, the

Perforator mapping and clinical experience with the superthin profunda artery perforator flap for reconstruction in the upper and lower extremities.

INTRODUCTION: Thin skin flaps have gained popularity for extremity reconstruction. However, the use of the profunda artery perforator (PAP) flap has not been explored as much. With its concealed donor site on the medial thigh and bulk, the PAP has become popular for reconstruction in the breast, head, and neck. The thickness of the subfascial PAP flap is decreased through elevation on the thin or superthin plane, making it more suitable for extremity reconstruction. METHODS: A consecutive series of 28 patients with 29 flaps who received a thin or superthin single perforator PAP flap for upper

Keystone Design Perforator Island Flap for the treatment of recurrent pilonidal disease.

Pilonidal disease is commonly managed with surgical excision; however, recurrence remains frequent, particularly following midline closure. Off-midline flap techniques are increasingly favoured for complex or recurrent disease, as they are associated with reduced wound healing duration and lower recurrence rates. This report describes the case of a 37-year-old obese male with chronic pilonidal disease refractory to multiple prior excisions, who underwent wide local excision and reconstruction with a Keystone Design Perforator Island Flap (KDPIF). Initial recovery was satisfactory, although api
