Persistent foetal vasculature: Developmental mechanisms, contemporary management and evolving therapeutics.

Persistent foetal vasculature (PFV) is a spectrum of congenital malformations arising from incomplete regression of the hyaloid-tunica vasculosa lentis complex. Since Reese's original description of persistent hyperplastic primary vitreous and Goldberg's seminal reinterpretation of PFV as a unified developmental spectrum, our understanding has expanded through advances in genetics, imaging, and vitreoretinal surgery. PFV remains one of the most challenging causes of unilateral childhood visual loss. Although modern microsurgical techniques now deliver consistent anatomical success and globe pr

Visual prognosis of posterior and combined persistent fetal vasculature.

INTRODUCTION: The persistent fetal vasculature refers to congenital anomalies of the globe resulting from the abnormal persistence of the hyaloid vascular system. It can present as anterior, posterior, or combined form. The aim of this study was to report the visual outcomes of posterior and combined forms of persistent fetal vasculature. METHODS: This retrospective, single-center study included every patient referred to our outpatient clinic with a posterior or combined form of persistent fetal vasculature. The primary endpoint was the visual acuity of the impaired eye, or of the best eye if

Morning glory syndrome. A functional analysis including fluorescein angiography, ultrasonography, and computerized tomography.

The Morning Glory syndrome (MGS) is a congenital optic nerve anomaly that has received scant attention since first described by Handmann in 1929. We studied ten eyes and eight patients displaying variations of the syndrome. We found that the syndrome may be bilateral, hereditary, and associated with other congenital anomalies. When bilateral, visual function was only moderately disturbed. The anomaly is usually limited to the eye, and the retrobulbar nerve and brain appear normal, as documented by ultrasound and computed tomography. Surprisingly, increasing fibrovascular tissue produces tracti

A male with polysyndactyly, linear skin defects and sclerocornea. Goltz syndrome versus MIDAS.

Focal dermal hypoplasia (FDH) or Goltz syndrome is a rare clinical syndrome presenting with cutaneous, skeletal, dental, ocular, central nervous system and soft-tissue defects. We report on a male infant with characteristic skin defects of the face, trunk and extremities, polysyndactyly and unusual ocular and brain findings. He had sclerocornea of the right eye, anterior persistent hyperplastic primary vitreous of the left eye and hydrocephalus. Clinical findings support the diagnosis of Goltz syndrome. The clinical picture of Goltz syndrome is compared with that of MIDAS syndrome.

Molecular and Cellular Regulations in the Development of the Choroidal Circulation System.

Disorders in the development and regulation of blood vessels are involved in various ocular disorders, such as persistent hyperplastic primary vitreous, familial exudative vitreoretinopathy, and choroidal dystrophy. Thus, the appropriate regulation of vascular development is essential for healthy ocular functions. However, regulation of the developing choroidal circulation system has not been well studied compared with vascular regulation in the vitreous and the retina. The choroid is a vascular-rich and uniquely structured tissue supplying oxygen and nutrients to the retina, and hypoplasia an

[Excentric lentiglobus posterior].

BACKGROUND: The posterior lentiglobe is a rare anomaly of the lens shape. Two cases of excentric protuberance of the posterior capsule are presented. To our knowledge, only one description of a similar case exists so far. PATIENTS: An eight-year-old girl (diagnosis by routine examination, V.A. 20/20) and a six-year-old boy (V. A. 20/200, esotropia) are presented. Apart from a persistent hyperplastic primary vitreous and a posterior pole cataract a vitreous cyst adherent to the posterior surface of the lens has to be considered as differential diagnosis. The etiology remains unclear. CONCLUSI

Critical period for retinoic acid-induced developmental abnormalities of the vitreous in mouse fetuses.

To elucidate the underlying developmental mechanisms of persistent hyperplastic primary vitreous (PHPV) in humans, we investigated a mouse model for PHPV induced by retinoic acid. We treated C57BL/6NJcl mice at various stages of pregnancy (gestation days 7, 8, 9, 10, 11, or 12) with the teratogen retinoic acid, which affects the migration of neural crest cells. Untreated pregnant mice served as a control group. The eyes of the fetuses were examined histologically on day 18 on gestation. Developmental abnormalities of the vitreous were defined as the presence of excessive mesenchymal tissue in

A novel NDP mutation in an infant with unilateral persistent fetal vasculature and retinal vasculopathy.

BACKGROUND: Mutations in the Norrie Disease gene, Norrie Disease Pseudoglioma (NDP) lead to a phenotypically heterogeneous group of retinopathies. We report a novel mutation in the NDP gene identified in a patient whose clinical presentation was suggestive of unilateral persistent fetal vasculature (PFV). MATERIALS AND METHODS: Ophthalmic examinations, ocular ultrasounds and sequence analysis of the exons of the NDP gene on peripheral blood DNA were performed. RESULTS: A four-month-old boy was referred to our institution for presumed unilateral retinoblastoma. The clinical and ultrasonograph

Unilateral persistent fetal vasculature coexisting with anterior segment dysgenesia.

CASE REPORT: A case is presented of a 4 week-old female neonate with Peters anomaly (PA) and unilateral persistent foetal vasculature (PFV) referred to our centre due to esotropia. At 12 weeks of age, a penetrating keratoplasty and vitrectomy were performed without major complications in the immediate post-operative period. The patient is currently under an intensive treatment for amblyopia and secondary glaucoma. DISCUSSION: Surgical treatment of PFV is controversial, with prevention of amblyopia, phthisis, and glaucoma being the main reasons for it. Patients with unilateral PFV and type II
