Personality disorders and their association with adverse childhood events.

PURPOSE OF REVIEW: Theoretical models of personality disorders (PDs) highlight the role of adverse childhood experiences (ACEs) in the development and maintenance of personality pathology. This review presents recent studies on the associations between childhood adversity and PDs, while discussing mechanisms and factors that impact these relationships. RECENT FINDINGS: Studies point to small to moderate associations between childhood adversity and cluster A, cluster B, and cluster C PDs, and some research suggests that childhood adversity could be a predictor for the onset and development of

The evolution of personality disorders: A review of proposals.

Personality disorders (PDs) are currently considered dysfunctions. However, personality differences are older than humanity and are ubiquitous in nature, from insects to higher primates. This suggests that a number of evolutionary mechanisms-other than dysfunctions-may be able to maintain stable behavioral variation in the gene pool. First of all, apparently maladaptive traits may actually improve fitness by enabling better survival or successful mating or reproduction, as exemplified by neuroticism, psychopathy, and narcissism. Furthermore, some PDs may harm important biological goals while f

Personality traits and anxiety in patients with temporomandibular disorders.

BACKGROUND: Temporomandibular disorders (TMD) have long been suggested to result from psychological factors. Recent studies, however, tend to consider TMD a chronic psychosomatic illness. The present study was designed to explore the association between TMD and personality profile. The Minnesota Multiphasic Personality Inventory-2-Reconstructed form (MMPI-2-RF) was used to evaluate the association for the first time. METHODS: A total of 258 subjects participated in this case-control study. TMD cases as detected by the Helkimo index were questioned regarding their personality characteristics a

Prosociality in personality disorders: Status quo and research agenda.

Personality disorders (PDs) are characterized by interpersonal dysfunction and deficits in prosociality are theorized to contribute to this. We review studies linking categorical PDs to prosocial and antagonistic traits and highlight studies that assessed prosocial behavior in PDs via economic games. We structure our review based on the recently proposed affordance framework of prosocial behavior, summarizing the evidence for exploitation, reciprocity, temporal conflict, and dependence under uncertainty as situational affordances that allow the expression of personality in prosocial behavior.

Application of the ICD-11 classification of personality disorders.

BACKGROUND: The ICD-11 classification of Personality Disorders focuses on core personality dysfunction, while allowing the practitioner to classify three levels of severity (Mild Personality Disorder, Moderate Personality Disorder, and Severe Personality Disorder) and the option of specifying one or more prominent trait domain qualifiers (Negative Affectivity, Detachment, Disinhibition, Dissociality, and Anankastia). Additionally, the practitioner is also allowed to specify a Borderline Pattern qualifier. This article presents how the ICD-11 Personality Disorder classification may be applied i

Interpersonal dysfunction in personality disorders: A meta-analytic review.

Personality disorders are defined in the current psychiatric diagnostic system as pervasive, inflexible, and stable patterns of thinking, feeling, behaving, and interacting with others. Questions regarding the validity and reliability of the current personality disorder diagnoses prompted a reconceptualization of personality pathology in the most recent edition of the psychiatric diagnostic manual, in an appendix of emerging models for future study. To evaluate the construct and discriminant validity of the current personality disorder diagnoses, we conducted a quantitative synthesis of the ex

Narcissistic Personality Disorder and suicidal behavior in mood disorders.

The relationship of Narcissistic Personality Disorder (NPD) to suicidal behavior is understudied. The modest body of existing research suggests that NPD is protective against low-lethality suicide attempts, but is associated with high lethality attempts. Mood-disordered patients (N = 657) received structured interviews including Axis I and II diagnosis and standardized clinical measures. Following chi-square and t-tests, a logistical regression model was constructed to identify predictors of suicide attempt. While there was no bivariate relationship of NPD on suicide attempt, in the logistic r

Review of pharmacologic treatment in cluster A personality disorders.

INTRODUCTION: A personality disorder is a pervasive and enduring pattern of behaviors that impacts an individual's social, occupational, and overall functioning. Specifically, the cluster A personality disorders include paranoid personality disorder, schizoid personality disorder, and schizotypal personality disorder. Patients with cluster A personality disorders tend to be isolative and avoid relationships. The quality of life may also be reduced in these individuals, which provokes the question of how to treat patients with these personality disorders. The purpose of this review is to evalua

Phemenological aspects of personality disorders in adult psychiatric patients.

BACKGROUND: Many empirical studies give evidence of co-occurrence of mental and personality disorders (PDs). On the other hand theoretical models explain the relationship between personality and mental disorders from different perspectives. This research studied the phemenological aspects of PDs in adult psychiatric patients with different mental disorders according to cognitive and psychoanalytic criteria for personality pathology. SUBJECTS AND METHODS: In order to study personality pathology in different diagnostic groups we constructed a self-report Questionnaire of Personality Disorders (

Have personality disorders been overdiagnosed among eating disorder patients?

BACKGROUND: There is persuasive evidence for a relationship between eating disorders (EDs) and personality disorders (PDs). Research studies over the last three decades have used various tools to explore PDs in EDs with differing results. We investigated PDs derived from an interview--the International Personality Disorder Examination. METHODS: 132 female inpatients with restrictive anorexia nervosa (AN-R), binge-purging AN, bulimia nervosa (BN) and ED not otherwise specified were interviewed. MANCOVA was used to test for differences in dimensional PD scores for the ED diagnostic and behaviou

Clusters of personality disorder cognitions in the eating disorders.

This study examined whether comorbid personality disorder pathology in the eating disorders clusters into broader patterns, and whether those clusters have clinical validity in terms of levels of eating pathology and axis 1 comorbidity. The sample consisted of 214 eating-disordered women who completed measures of personality disorder cognitions, eating pathology and axis 1 pathology at assessment. Three clusters of eating disorder patients emerged-low levels of personality pathology overall, high levels of cognitions underpinning anxiety-based personality pathology, and high levels of all of t

The plight of personality disorders in the DSM-5.

This article provides a brief history of the development of the Diagnostic and Statistical Manual of Mental Disorders published by the American Psychiatric Association, and how it relates to the proposed fifth edition to be published in 2013. Of particular emphasis is Axis II and how this axis is likely to be restructured. The reconceptualization of the nosology for personality disorders has been controversial since the publication of the DSM-III-R (Wilson, 1993). In both the clinical and academic communities, ongoing debate about diagnostic classification of personality disorders has been com

Off-label use of atypical antipsychotics in personality disorders.

INTRODUCTION: Personality disorders are among the most persistent and challenging disorders to treat within psychiatry. There is emerging evidence that some personality disorders, particularly borderline personality disorder and, to a lesser extent, schizotypal personality disorder, may benefit from treatment with atypical antipsychotics as well as mood stabilizers. This review examines the evidence for atypical antipsychotics for personality disorders and discusses strengths and limitations of this approach. AREAS COVERED: Searches of Medline and PsycInfo yielded 57 articles related to use o

Personality disorders: review and clinical application in daily practice.

Personality disorders have been documented in approximately 9 percent of the general U.S. population. Psychotherapy, pharmacotherapy, and brief interventions designed for use by family physicians can improve the health of patients with these disorders. Personality disorders are classified into clusters A, B, and C. Cluster A includes schizoid, schizotypal, and paranoid personality disorders. Cluster B includes borderline, histrionic, antisocial, and narcissistic personality disorders. Cluster C disorders are more prevalent and include avoidant, dependent, and obsessive-compulsive personality d

[Impulse control disorders in borderline and antisocial personality disorder].

A borderline personality disorder is associated with highly impulsive acts that cannot be controlled by cognitive inhibition. In a psychopathic/antisocial personality disorder emotional inhibition of hostile acts is lacking. The patient has a high proclivity for risk-seeking, and is incapable of responding appropriately to punishment. In both disorders, the result is (auto)aggressive behavior. The family doctor must refer such patients to a specialist, when there is an acute danger of self-harm or when a grave functional limitation in the areas of work or interpersonal relationship has persist

Eating disorders and personality: a methodological and empirical review.

Methodological approaches utilized to evaluate models of the relationship between personality and eating disorders, as well as empirical support for each model, are reviewed. Limited prospective research suggests that negative emotionality, perfectionism, drive for thinness, poor interoceptive awareness, ineffectiveness, and obsessive-compulsive personality traits are likely predisposing factors. Limited family study research suggests that obsessive-compulsive personality disorder (OCPD) and anorexia nervosa share a common familial liability. Potential pathoplastic personality factors include

Skills training as an adjunctive treatment for personality disorders.

Personality disorders are usually treated with either psychodynamic or supportive psychotherapy, with psychotropic medication often used as an adjunctive treatment. However, patients with personality disorders pose special treatment issues, because their problems are pervasive and long-standing and they have entrenched deficits in many areas of functioning. In this article, the authors consider the role of skills training in the treatment of personality disorders. They describe a two-pronged approach to the treatment of personality disorders based on a model of deficit compensation. Because sk
