Physical therapy, corticosteroid injection, and extracorporeal shock wave treatment in lateral epicondylitis. Clinical and ultrasonographical comparison.

The aim of this study was to compare--clinically and ultrasonographically--the therapeutic effects of physical therapy modalities (hot pack, ultrasound therapy, and friction massage), local corticosteroid injection, and extracorporeal shock wave treatment (ESWT) in lateral epicondylitis (LE). Fifty-nine elbows of 59 patients with LE were randomized into three treatment groups receiving either physical therapy, a single corticosteroid injection, or ESWT. Visual analogue scale (VAS) was used to assess pain intensity, Jamar hydraulic dynamometer for grip strength, finger dynamometer for pinch str

Comparison of Thumb Metacarpophalangeal Arthrodesis to Volar Plate Capsulodesis for Metacarpophalangeal Hyperextension in the Setting of Basal Joint Arthritis-A Case-Control Study.

INTRODUCTION: Surgical treatment of metacarpophalangeal (MCP) joint hyperextension in the treatment of thumb basal joint arthritis can be categorized as motion preserving and arthrodesis. We compared minimum 2-year results between nonsurgical thumbs and thumbs undergoing ligament reconstruction with tendon interposition (LRTI) alone, thumb MCP arthrodesis, and volar plate capsulodesis as adjunct procedures to LRTI. METHODS: Single surgeon patients with thumb MCP hyperextension >30° underwent arthrodesis or volar plate capsulodesis with LRTI. Thirty-one LRTI thumbs were compared with 22 LRTI w

Principal component and cluster analysis of functional parameters in rheumatic patients: Identifying the most efficient assessment tool.

BACKGROUND: Rheumatic diseases significantly impact hand function, leading to disability and reduced quality of life. Various tools assess upper limb function, including the DASH questionnaire, grip strength measurement, and range of motion (ROM) evaluation. However, the most efficient parameters for assessing functional impairment remain unclear. OBJECTIVE: To determine the most relevant anthropometric and functional measurements for evaluating hand function impairment in patients with rheumatic diseases, using principal component analysis (PCA) and cluster analysis. METHODS: We conducted a

Radial to Median Sensory Nerve Transfers to Restore Sensate Key Pinch: A Cadaveric Assessment of Size Match and Implications for Transfer.

BACKGROUND: Loss of key-pinch sensation after median nerve injury poses significant functional detriment. Nerve transfers are utilized to improve function after nerve injury and size matching of donor and recipient nerves is important to optimize success. This anthropometric study investigates the anatomy of the superficial branch of the radial nerve (SBRN) to the thumb and index finger and explores radial to median sensory nerve transfers, a necessary but not heavily discussed facet of nerve transfers for the hand. METHODS: Anatomic dissection of SBRN in 15 cadaver hands was performed to ass

Ulnar collateral ligament injuries of the thumb: a comprehensive review.

Injuries to the thumb ulnar collateral ligament (UCL) are common. Failure to address the ensuing laxity of the metacarpophalangeal joint can lead to compromised grip and pinch, pain, and ultimately osteoarthritis. Instability to valgus stress with the lack of a firm end point is a strong indicator of complete rupture of the UCL. Nonoperative treatment is reserved for incomplete ruptures of the thumb UCL. Operative intervention is typically performed for complete ruptures. Repair of acute ruptures and reconstruction for chronic injuries yield excellent results. Complications are rare and most p

Long-term function after pseudo-pollicization of the little finger.

This article presents a technique for facilitating use of pinch between the ring and little fingers for use in rare cases of congenital absence, or severe hypoplasia, of the thumb in which pollicization of the index finger is impossible, or not advised because the child is already pinching by scissor action between the ring and little fingers. The technique avoids drawing attention to the hand, as is the case after true pollicization of the little finger. The technique was used in five hands in five children. Three of the patients could only be followed for under 2 years. Two patients were ava

Effects of tendon transfer to restore index finger abduction for severe cubital tunnel syndrome.

BACKGROUND: Severe cases of cubital tunnel syndrome do not always result in functional recovery after surgical decompression of the ulnar nerve. A combined operation of tendon transfer to restore index finger abduction and decompression of the ulnar nerve was performed for patients with severe cubital tunnel syndrome who required powerful pinch strength and whose preoperative compound muscle action potential of the abductor digiti minimi muscle was not recordable or almost non-recordable. The purpose of this study was to assess the efficacy of tendon transfer to restore index finger abduction

Functional Reconstruction of Thumb with Tendons from an Amputated Finger in Median Nerve Territory-Oriented Macrodactyly.

Nerve territory-oriented macrodactyly (NTOM) of the hand, particularly involving the median nerve, results in severe enlargement and dysfunction of the thumb and index finger, compromising grasp and pinch functions. We present two paediatric cases managed by ray amputation of the index finger and debulking of the thumb, with a novel approach utilizing tendons from the amputated finger to restore thumb flexion, abduction, and opposition in a single-stage procedure. This technique avoids additional donor-site morbidity and optimizes the use of available structures. Both patients achieved improve

Reconstruction of lateral pinch in an isolated paralysis of the first dorsal interosseous muscle - A new surgical technique.

Lack of function of the first interosseous muscle (FDI) might be responsible for insufficient stabilisation of the index finger during lateral pinch, and may induce disability in hand function. The first cause of FDI palsy is ulnar nerve palsy. We describe a new tendon transfer to reanimate the FDI muscle, using the extensor indicis proprius tendon. The tendon is sectioned at its distal insertion and rerouted in the first extensor tendon compartment. We report one case of isolated first interosseous muscle palsy secondary to direct trauma. Preoperatively, the patient complained of a severe lac

The results of carpal tunnel release for carpal tunnel syndrome diagnosed on clinical grounds, with or without electrophysiological investigations: a randomized study.

We compared the results of carpal tunnel release in patients with the diagnosis of carpal tunnel syndrome based on only clinical grounds and those diagnosed on clinical and electrophysiological grounds. Ninety-three patients, 83 women (89%) and ten men (11%), meeting the criteria of 'typical' carpal tunnel syndrome, were randomly assigned to receive carpal tunnel release with (n = 45, 48%) or without (n = 48, 52%) nerve conduction studies. Patients were followed-up at 1 and 6 months, by assessments that included the Levine scores, filament tests, grip and pinch strength. No significant differe

Direct end-to-end repair of flexor pollicis longus tendon lacerations.

Between 1976 and 1986, 38 consecutive acute isolated flexor pollicis longus lacerations were repaired. This study excluded all replanted or mutilated digits and all lacerations with associated fracture. Average follow-up was 26 months. Tendon rehabilitation was standardized. Range of motion and pinch strength were measured postoperatively. Seventy-four percent (28/38) of the flexor pollicis longus injuries occurred in zone II. Neurovascular injury occurred in 82% of the lacerations, and this correlated with the zone of tendon injury. In 21% of the patients (8/38) both digital nerves and arteri

Significance of finger forces and kinematics during handwriting in writer's cramp.

Muscular hyperactivity during handwriting, irregular and jerky scripts, as well as awkward and slowed pen movements are the cardinal symptoms of writer's cramp. Accordingly, impaired kinematics and increased force have been reported in writer's cramp. However, the relationship between these symptoms has rarely been investigated. In addition, measurements of finger forces have been restricted to the vertical pen pressure. In the present study, the pen of a graphic tablet was equipped with a force sensor matrix to measure also the grip force produced against the pen barrel despite highly variabl

Late results in replanted digits. Is replantation of a single digit worthwhile?

During the period 1980-1989 35 patients with successfully replanted digits could be selected. Only 17 patients with 32 long fingers and 2 thumbs replanted were prepared to cooperate for this retrospective study, looking for the late functional results. Mobility of the replanted digits was measured by the TAM-value. Power was indicated by grip strength and tip pinch. Weber two-point discrimination test and Von Frey test were used to investigate sensibility. Most patients show cold-intolerance. Mobility was worse for the more proximal replantations. Replantations distal to PIP do functionally ve
