Favorable Rates of Cardiovascular Events with Stringent Cardiovascular Monitoring and a Lowered Dose of Ponatinib as Second-Line Treatment in Chronic Phase Chronic Myeloid Leukemia Patients Failing or Intolerant to First-Line Second-Generation Tyrosine Kinase Inhibitor Treatment: Results of the Prospective PONS Trial.

INTRODUCTION: In chronic myeloid leukemia patients, second-line treatment requires careful consideration of response and tolerability. As most patients need a more efficient tyrosine kinase inhibitor, ponatinib at a lowered dose should be evaluated in this setting. METHODS: We studied a lowered dose of 30 mg ponatinib in second line in patients selected toward a low cardiovascular risk. In 22 screened patients, ponatinib was started in 18 patients previously treated with imatinib (n = 3), dasatinib (n = 9), or nilotinib (n = 6). Patients were frequently monitored for cardiovascular toxicities

Gene delivery of paraoxonase-1 inhibits neointimal hyperplasia after arterial balloon-injury in rabbits fed a high-fat diet.

Paraoxonase-1 (PON-1) is a high-density lipoprotein (HDL)-associated enzyme that hydrolyzes oxidized phospholipids, thereby preventing the oxidative modification of low-density lipoproteins (LDL). A high-fat diet reduces PON-1 activity, enhancing LDL oxidation. Thus, PON-1 is a candidate for anti-atherogenic gene therapy. In the present study, we investigated the effect of local PON-1 overexpression on the development of atherosclerotic lesions using the Sendai virus-mediated transgenic technique. One-month-old rabbits (n=11) were fed a high-fat diet for 8 weeks and then subjected to balloon i

Ponicidin ameliorates Alzheimer's disease through dual inhibition of RIPK1-mediated neuroinflammation and necroptosis.

Ponicidin (Pon), a diterpenoid isolated from Rabdosia rubescens, exhibits a broad range of pharmacological activities, including anti-inflammatory effects. However, its therapeutic potential in Alzheimer's disease (AD), particularly in modulating receptor-interacting protein kinase 1 (RIPK1)-mediated neuroinflammation and necroptosis, remains underexplored. This study aims to investigate the mechanism through which Pon targets RIPK1 to alleviate AD pathogenesis. The interaction between Pon and RIPK1 was confirmed using bio-layer interferometry (BLI) and drug affinity responsive target stabilit

[CLinico-morphologic correlations in cases of fatal outcome following removal of acoustic nerve neuromas].

The clinical findings were compared with the results of morphological examination of the brain in 32 patients who had died in different periods after surgery for removal of neurinoma of the acoustic nerve. In all cases vessels supplying the brain stem were injured during removal of the tumor. In 5 cases all three groups of arteries of the pons varolii were injured, which led to the development of infarction of half of the pons. The short circumflex pons arteries and the anterior inferior cerebellar artery were injured in 18 patients, which caused the formation of an infarct in the lateral part

BMP-7 Treatment Ameliorates PTEN-Akt Mediated Apoptosis and Adverse Cardiac Remodeling in Ponatinib-Induced Cardiotoxicity.

Background/Objectives: Ponatinib (PON) is a potent anticancer drug widely used to treat chronic myeloid leukemia (CML). Although many cancer survivors benefit from such therapies, managing drug-induced side effects, especially cardiotoxicity, remains a major challenge. Despite its prevalence, the exact mechanisms underlying PON-induced cardiotoxicity have not been thoroughly investigated. Additionally, the potential of Bone Morphogenetic Protein 7 (BMP-7) to alleviate these cardiotoxic effects has yet to be explored. Methods: To address these essential questions, we conducted a study using C57

The human paraoxonase gene cluster as a target in the treatment of atherosclerosis.

The paraoxonase (PON) gene cluster contains three adjacent gene members, PON1, PON2, and PON3. Originating from the same fungus lactonase precursor, all of the three PON genes share high sequence identity and a similar β propeller protein structure. PON1 and PON3 are primarily expressed in the liver and secreted into the serum upon expression, whereas PON2 is ubiquitously expressed and remains inside the cell. Each PON member has high catalytic activity toward corresponding artificial organophosphate, and all exhibit activities to lactones. Therefore, all three members of the family are regard

[Sustaining molecular response with very low dose ponatinib and further response after hemodialysis initiation in a patient with chronic myeloid leukemia].

A 46-year-old man was diagnosed with chronic myeloid leukemia (CML) in chronic phase. He was treated with imatinib, nilotinib, and dasatinib, but failed to achieve a complete cytogenetic response (CCyR). After tyrosine kinase inhibitor therapy, F317L BCR-ABL1 kinase domain mutation was detected. At age 66, the patient started ponatinib (PON) at 45 mg/day, and achieved CCyR within three months. Subsequently, PON was tapered to 15 mg once weekly due to arterial-occlusive events. PON was discontinued after a 3-year deep molecular response (≥ MR4.5). However, the patient lost MR4.0 within two mont

HMGB1 Modulates Angiogenic Imbalance and Cardiovascular Complications in Preeclampsia through Decorin and VEGF Regulation.

BACKGROUND: Type 1 diabetes mellitus (T1DM) is a chronic autoimmune condition that can lead to long-term complications due to oxidative stress and metabolic dysregulation. Paraoxonase-1 (PON-1), an enzyme associated with high-density lipoprotein (HDL), has dual activities: arylesterase and lactonase. These activities protect lipids from oxidative damage. The functional status of PON-1 in children with T1DM may provide insights into the relationship between oxidative stress and the enzyme's protective role. This study aims to assess the arylesterase and lactonase activities of PON-1 in Iraqi ch

Empagliflozin mitigates ponatinib-induced cardiotoxicity by restoring the connexin 43-autophagy pathway.

BACKGROUND: Empagliflozin (EMPA), a selective sodium-glucose cotransporter type 2 (SGLT2) inhibitor, has been shown to reduce major adverse cardiovascular events in patients with heart failure of different etiologies, although the underlying mechanism still remains unclear. Ponatinib (PON) is a multi-tyrosine kinase inhibitor successfully used against myeloid leukemia and other human malignancies, but its cardiotoxicity remains worrisome. Cardiac connexins (Cxs) are both substrates and regulators of autophagy and responsible for proper heart function. Alteration in connexin expression and loca

Serum paraoxonase-1 activity is inversely related to free thyroxine in euthyroid subjects: The PREVEND Cohort Study.

BACKGROUND: Low-normal thyroid function within the euthyroid range has been suggested to enhance atherosclerosis susceptibility. Paraoxonase-1 (PON-1) may protect against atherosclerotic cardiovascular disease development by attenuating oxidative stress. We evaluated relationships of PON-1 with thyroid stimulating hormone (TSH), free T4 , free T3 , lipids and apolipoprotein (apo)A-I in euthyroid subjects, and assessed whether such relationships are modified in the context of the metabolic syndrome (MetS). MATERIALS AND METHODS: Serum PON-1 activity (arylesterase activity), TSH, free T4 , free
