Natural and experimental prion diseases of humans and animals.

Prions cause transmissible and genetic neurodegenerative diseases. Infectious prion particles are composed largely, if not entirely, of an abnormal isoform of the prion protein (PrPSc), which is encoded by a chromosomal gene. Although the PrP gene is single copy, transgenic mice with both alleles of the PrP gene ablated develop normally. A post-translational process, as yet unidentified, converts the cellular prion protein (PrPC) into PrPSc. Scrapie incubation times, neuropathology and prion synthesis in transgenic mice are controlled by the PrP gene. Mutations in this gene are genetically lin

Antiphospholipid antibodies in acute and post-treatment Lyme disease.

Acute Lyme disease is caused by a Borrelia infection and typically responds to antibiotic treatment, but post-infectious chronic symptoms are common. The precise origin of these post-treatment symptoms is unknown; dysregulation of immune responses raised during the initial infection is likely to contribute. Antilipid antibodies are associated with several autoimmune diseases and have been shown to arise in both acute and post-treatment Lyme disease. To assess the potential contribution of antilipid antibodies to Lyme disease pathology and their possible use as biomarkers of both acute and chro

Extracorporeal Photopheresis as an Immunomodulatory Bridge in Kidney Transplant Recipients With Infections and Concomitant Donor-Specific Antibodies: A Case Series.

INTRODUCTION: Kidney transplant recipients with severe infections often require immunosuppression minimization, increasing the risk of donor-specific antibody (DSA) persistence and antibody-mediated injury. Extracorporeal photopheresis (ECP) may provide immunomodulation while limiting alloimmune activation. METHODS: We retrospectively analyzed four kidney transplant recipients with severe infectious complications and concomitant DSAs who required immunosuppression reduction. Clinical scenarios included BK viremia, BK virus nephropathy, Epstein-Barr virus-associated post-transplant lymphoproli

EBV-PTLD in a patient after haploidentical stem-cell transplantation with post-transplant cyclophosphamide.

Here, we describe the case of a male patient with Epstein-Barr virus post-transplantation lymphoproliferative disorder (EBV-PTLD), which developed 18 months after a haploidentical hematopoietic stem cell transplantation (haplo-HSCT) and the administration of post-transplant cyclophosphamide (PTCy). Of note, no anti-thymoglobulin was used in the entire clinical course. Prior to the onset of EBV-PTLD, the patient had pulmonary chronic graft-versus-host disease and was treated with prednisolone and tacrolimus. After stopping immunosuppressive therapy, he was diagnosed with EBV-positive infectious

Autoimmunity in visual loss.

There are a number of autoimmune disorders which can affect visual function. There are a very large number of mechanisms in the visual pathway which could potentially be the targets of autoimmune attack. In practice it is the retina and the anterior visual pathway (optic nerve and chiasm) that are recognised as being affected in autoimmune disorders. Multiple Sclerosis is one of the commonest causes of visual loss in young adults because of the frequency of attacks of optic neuritis in that condition, however the basis of the inflammation in Multiple Sclerosis and the confirmation of autoimmun

Hieronymus Bosch (1450-1516): paleopathology of the medieval disabled and its relation to the Bone and Joint Decade 2000-2010.

BACKGROUND: At the start of the Bone and Joint Decade 2000-2010, a paleopathologic study of the physically disabled may yield information and insight on the prevalence of crippling disorders and attitudes towards the afflicted in the past compared to today. OBJECTIVE: To analyze "The procession of the Cripples," a representative drawing of 31 disabled individuals by Hieronymus Bosch in 1500. METHODS: Three specialists--a rheumatologist, an orthopedic surgeon and a neurologist--analyzed each case by problem-solving means and clinical reasoning in order to formulate a consensus on the most lik

Gastric motility disorders and their endoscopic and surgical treatments other than bariatric surgery.

Gastroparesis is the most common gastric motility disorder. The cardinal symptoms are nausea, vomiting, gastric fullness, early satiety, or bloating, associated with slow gastric emptying in the absence of mechanical obstruction. Delayed gastric emptying is demonstrated by a gastric emptying scintigraphy or by a breath test. Gastroparesis can be idiopathic, post-operative, secondary to diabetes, iatrogenic, or post-infectious. Therapeutic care must be multidisciplinary including nutritional, medical, endoscopic and surgical modes. The complications of delayed gastric emptying must be sought an

How competing needs after incarceration lead to adverse health outcomes among people who use criminalized drugs.

In the USA, people with a history of criminalized drug use and drug use disorders reentering the community after incarceration frequently experience adverse health outcomes including overdose, suicide, and infectious disease acquisition. This review presents a conceptual model for understanding risk pathways for these outcomes related to post-release psychosocial needs. We first summarize the literature on post-release needs experienced by people who use criminalized drugs during reentry in multiple domains, including basic needs and those related to relationships as well as medical, mental he

Pathological and bacteriological findings in sows, finisher pigs, and piglets, being culled for lameness.

BACKGROUND: Lameness is a common pig disorder. In the herd, a recurring, consistent aetiology is often suspected based on the age of the pig and herd-specific predisposing factors. The aetiology may be difficult to establish especially in larger animals, since the primary pathogenesis is obscured by delayed diagnosis, chronic changes and sequelae, and thorough investigation usually requires culling and transport to a pathological laboratory for necropsy. Thus, treatments are mostly based on a tentative diagnosis, commonly including antibiotics to eliminate infectious agents. The study aimed to

Vesicoureteral reflux and elimination disorders.

Two kinds of elimination disorders can be associated with Vesico Ureteral Reflux (VUR): pure bladder elimination disorders or combination of bladder and bowel elimination disorders. An elimination disorder is always a factor which worsens the prognosis of VUR, as it increases the risk of infectious complications and thus presents a threat for the upper urinary tract. Regarding pure bladder elimination disorders, a chronic urine residue is observed in four clinical situations: the syndrome megacystis-mega ureter; the mega bladder without mega ureter, but with VUR; high grade massive VUR without

Association Between Post-COVID-19 Herpes Zoster Reactivation and Peripheral Nervous System Disorders: Multinational Real-World Evidence from TriNetX.

BACKGROUND: The increased incidence of herpes zoster (HZ) following coronavirus disease 2019 (COVID-19) suggests a period of immune dysregulation, but the associated long-term neuro-immunological risks remain unclear. METHODS: Utilizing de-identified electronic health records from the TriNetX Global Collaborative Network, we established 1:1 propensity score-matched cohorts of COVID-19 survivors, comparing HZ-exposed and unexposed groups over a three-year follow-up. Primary endpoints included the incidence of peripheral nervous system disorders: Bell's palsy, Guillain-Barré syndrome (GBS), and

Development of restrictive eating disorders in children and adolescents with long-COVID-associated smell and taste dysfunction.

BACKGROUND: Absent or abnormal senses of smell and taste have been frequently reported during both acute and long COVID in adult patients. In contrast, pediatric patients who test positive for SARS-CoV-2 are often asymptomatic and the loss of smell and/or taste has been infrequently reported. After observing several young patients with COVID-associated anosmia and ageusia at our clinic, we decided to investigate the incidence of subsequent eating disorders in these patients and in SARS-CoV-2 positive patients who did not experience anosmia and ageusia during the same period. Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections (PANDAS): An Evolving Concept.

Pediatric autoimmune neuropsychiatric disorders associated with streptococcus infections (PANDAS) originated from the observational work of Swedo and collaborators, who formalized their definition in 1998 in a set of operational criteria. The application of these criteria, which focuses on tics and obsessive-compulsive symptoms as core symptoms, has encountered difficulties, eventually leading to a high rate of misdiagnosis. In particular, the core feature represented by the association between newly diagnosed infections and neuropsychiatric symptom relapses in youths with this diagnosis could

Managing bronchiolitis obliterans syndrome (BOS) and chronic lung allograft dysfunction (CLAD) in children: what does the future hold?

The success of pediatric lung transplantation continues to be limited by long-term graft dysfunction. Historically this has been characterized as an obstructive spirometric defect in the form of the bronchiolitis obliterans syndrome (BOS). It is recognized, however, that this does not reflect many of the other acknowledged etiologies of chronic lung dysfunction-noting it is the sum of the parts that contribute to respiratory morbidity and mortality after transplant. The term chronic lung allograft dysfunction (CLAD) has been coined to reflect these other entities and, in particular, a group of

SQSTM1/p62 Post-Translational Modifications and Reverse Processes Modulate Disease Pathogenesis via KEAP1-NRF2 Signaling and Selective Autophagy.

Significance: Sequestosome 1 (SQSTM1/p62, hereafter referred to as p62) is a multifunctional ubiquitin-binding autophagy receptor that acts as a critical bridge between the kelch-like ECH-associated protein 1 and nuclear factor erythroid 2-related factor 2 (KEAP1-NRF2) pathway and selective autophagy through diverse post-translational modifications (PTMs) and their reverse processes. Recent Advances: As a selective autophagy receptor, p62 facilitates the degradation of ubiquitinated substrates while functioning as a signaling hub to orchestrate cellular responses to oxidative stress. Given its

Transgender people in clinical trials of drugs and biologics: An analysis of ClinicalTrials.gov from 2007 to 2023.

AIMS: Transgender people have unmet health needs related to chronic conditions such as dementia, osteoporosis and hypertension. Community-driven advocacy increased transgender representation in phase III trials for pharmacological prevention of HIV, but the extent to which drug trials for other conditions have included transgender people is unknown. We investigated the extent to which trials of drugs and biologics represented transgender people across therapeutic areas on ClinicalTrials.gov. METHODS: Cross-sectional analysis of trials of drugs and biologics registered on ClinicalTrials.gov fr
