[Treatment of cardiac arrhythmias in aged patients (author's transl)].

The most often causes of cardiac arrhythmias in aged patients are heart failure as a consequence of coronary or of hypertensive heart disease, hypersensitive carotis-sinus, digitalisintoxication and hypokalemia. Out of that, the different tachyarrhythmias and bradyarrhythmias and often also a tachycardia-bradycardia-syndrome may develop. Appropriately it results the most often kinds of antiarrhythmic therapy as the treatment of heart failure with digitalis, diuretics and nitrates, the handling of electrostimulation with pacemakers, the removal of digitalis and the supply of potassium. Avoiding drug problems. The safety of drugs for supraventricular tachycardia.

To minimize drug problems in the treatment of supraventricular tachycardias, it is important to understand the spectrum of adverse events and to identify patients at high risk for these problems. Adverse cardiac and non-cardiac effects are associated, to varying degrees, with currently available antiarrhythmics. Cardiac adverse events include the development of rhythm disturbances, such as ventricular tachycardia or torsades des pointes, may result in syncope or death. In a meta-analysis of six randomized trials of quinidine vs placebo for atrial fibrillation, 1.8% of quinidine-treated patient

Effects of flecainide on atrial electrophysiology in the Wolff-Parkinson-White syndrome.

The effects of intravenous flecainide and propafenone (2 mg/kg) administered in random order were compared in 16 patients with Wolff-Parkinson-White syndrome. Both agents prolonged significantly the anterograde refractory period of the pathway and caused complete anterograde block in the pathway in 5 patients. Atrial fibrillation was not inducible in 7 patients following both agents. Both drugs prolonged the minimum pre-excited RR interval, but this effect was significantly greater after flecainide than after propafenone. At a pacing cycle length of 500 msec, the atrial effective refractory pe

[Medicinal rhythm control in atrial fibrillation].

Medicinal antiarrhythmic therapy is either used in the acute setting to convert atrial fibrillation to sinus rhythm or as chronic medication to preserve sinus rhythm if a rhythm control strategy is followed. The choice of the antiarrhythmic agent is based on the presence or absence of structural heart disease. In addition, oral anticoagulation should be established according to current guidelines. In the acute setting the armamentarium comprises flecainide, propafenone, vernakalant and amiodarone. Usually, combination therapy with an atrioventricular (AV) node slowing drug (a beta blocker or v

Effectiveness of a Triple Antiarrhythmic Drug Strategy for Arrhythmia Recurrence after Persistent Atrial Fibrillation Ablation.

BACKGROUND AND OBJECTIVE: Treating recurrent atrial arrhythmias after persistent atrial fibrillation (PeAF) ablation is often challenging. This single-center, prospective study aimed to observe the effectiveness of different combinations of oral antiarrhythmic drugs (AADs) in reverting to sinus rhythm (SR) in patients with recurrent atrial arrhythmias after PeAF ablation. METHODS: Forty-five patients who experienced recurrent atrial arrhythmias after PeAF ablation were included. Based on their medication regimens, patients were divided into two groups, with the study group being a triple-drug

Using the right drug: a treatment algorithm for regular supraventricular tachycardias.

Despite the recent advent of and the successful results from catheter ablation, pharmacological therapy is still used by most clinicians as the first line therapy in patients with regular supraventricular tachycardias. Before prescribing an antiarrhythmic agent, documentation of the arrhythmia using a 12-lead electrocardiogram (ECG) is necessary to identify the type of tachycardia. The ECG diagnosis is based on the presence and polarity of the P wave, the P to QRS relationship, the presence of QRS alternation and the effect of bundle branch block on tachycardia rate. Most regular supraventricu

The effects of cardiac drugs on human erythrocyte carbonic anhydrase I and II isozymes.

Cardiovascular diseases are the leading cause of mortality worldwide. In recent years, the relationship between carbonic anhydrase inhibitors and atherosclerosis has attracted attention. In this study, we aimed to determine the in vitro effects of 35 frequently used cardiac drugs on human carbonic anhydrase I (hCA I) and II (hCA II). The inhibitory effects of the drugs on hCA I and hCA II were determined with both the hydratase and esterase methods. The most potent inhibitors observed were propafenone (hCA I: 2.8 µM and hCA II: 3.02 µM) and captopril (hCA I: 1.58 µM and hCA II: 6.25 µM). Isoso

Pharmacologic treatment of supraventricular tachycardia: the German experience.

Tachyarrhythmias that originate above the bifurcation of the bundle of His or in tissue proximal to it are classified as supraventricular tachyarrhythmias (SVTs). Primary treatment of SVT tries to influence the underlying disease. Symptomatic therapy is subdivided into drug therapy, electrotherapeutic tools (e.g., antitachycardia pacemakers, catheter ablation), and antiarrhythmic surgery. Antiarrhythmic agents that slow conduction and suppress premature beats are efficient for emergency and long-term treatment of SVTs. We evaluated some of the most relevant antiarrhythmic drugs in SVT, includi

Clinical challenge. II. Management of recent onset atrial fibrillation. PAFIT-2 Investigators.

Atrial fibrillation is a very common arrhythmia in patients with structural heart disease, but it also occurs in patients without underlying heart disease. Acute therapy for paroxysmal atrial fibrillation is very dependent on the clinical condition of the patient. Direct current cardioversion is usually the first choice whenever the arrhythmia precipitates heart failure or severe angina, while more stable patients are normally treated with drugs. Most episodes of atrial fibrillation eventually convert to sinus rhythm even in the absence of treatment. Antiarrhythmic drugs can be used to control

Factors influencing early recurrence of atrial fibrillation among elderly patients following radiofrequency catheter ablation and the impact of different antiarrhythmic regimens.

BACKGROUND: Patients with atrial fibrillation (AF) who undergo radiofrequency catheter ablation (RFCA) necessitate the administration of antiarrhythmic drugs to prevent early recurrence. The clinical outcomes among these patients may be influenced by varying antiarrhythmic regimens. OBJECTIVES: To identify the risk factors associated with early recurrence and compare the clinical outcomes among different antiarrhythmic regimens in elderly patients with AF following radiofrequency catheter ablation (RFCA) during a 3-month period. METHODS: A retrospective observational study encompassed 420 el

Nutrient Availability Dictates Cancer Metabolism-Based Therapeutic Responses to Nononcology Drugs.

UNLABELLED: Metabolic dysregulation is a major hallmark of cancer, making interventions that modify tumor nutrient availability attractive adjuvants for improving clinical outcomes for patients with cancer. Clarifying how the nutritional status of individual patients affects the metabolic vulnerability of tumors to drugs is needed to inform personalized treatment guidelines. Working toward the goal of oncometabolic precision medicine, we developed the cancer metabolism-based synthetic lethality platform (CM-SLP), a high-throughput screening platform that explores the metabolic vulnerability of

Risk factors for the development of recurrent atrial fibrillation: role of pacing and clinical variables.

Atrial fibrillation recurs in many patients treated with antiarrhythmic therapy to maintain sinus rhythm. From March 1985 to August 1991, 214 patients with recurrent symptomatic chronic or paroxysmal atrial fibrillation for which conventional antiarrhythmic agents had failed were treated with propafenone or sotalol. Baseline demographic data including the presence of pacing therapy were collected. Life-table estimates of the duration of freedom from atrial fibrillation were constructed on the basis of pacemaker status. Of 214 patients, 26 (12.1%) had pacing therapy. Patients with dual-chamber

[Dissection of the accessory conduction pathways with simultaneous mitral valve surgery].

The case of surgical treatment of a 44-years old female with a double atrio-ventricular accessory pathway and mitral stenosis was described. Fast ventricular rates due to conduction through the accessory pathways during atrial fibrillation in course of mitral stenosis caused severe heart failure, occasionally with deep hypotonia. Combined treatment with prajmaline and propafenone blocked conduction through accessory pathways. The ventricular rate slowed from approx. 200/min to 150/min. Addition of digoxin slowed the ventricular rate down to approx 80-100/min. There were no signs of preexcitati

[Current treatment of supraventricular tachycardia: drug therapy].

Recurrent supraventricular arrhythmias are associated with palpitations, weakness, syncopes, etc. Pharmacological treatment with antiarrhythmic agents aims to interrupt the tachyarrhythmia episodes (acute therapy) and to stabilize sinus rhythm thereafter (chronic therapy). Due to the newer curative options (transvenous ablation), the segment for the pharmacological therapy is shrinking. Treatment with antiarrhythmic agents is still indicated, if the recurrence of tachycardia episodes can be sufficiently controlled by a well-tolerated antiarrhythmic regimen. In the cohort of atrial fibrillation

Traditional Chinese medicine for premature ventricular contraction caused by obstructive sleep apnea: A case report and literature review.

RATIONALE: Premature ventricular contraction (PVC) is a common type of arrhythmia, and obstructive sleep apnea (OSA) is a common trigger for this condition. Some patients still have PVC, even if ventilation is improved by wearing a respirator. Traditional Chinese medicine (TCM) has a long history of arrhythmia treatment, and this is the first report of a patient with PVC caused by OSA treated with TCM. Twenty-four-hour Holter showed that the number of PVCs decreased from 8968 to 0 before and after TCM treatment, and discomfort symptoms disappeared completely. PATIENT CONCERNS: A 37-year-old m
