Digital approach to rehabilitating partially edentulous arch using advanced fabrication techniques for precision attachments in cast partial dentures: a case report.

The integration of digital technologies in removable prosthodontics has significantly enhanced the precision, efficiency, and predictability of cast partial denture (CPD) fabrication. Conventional methods, often involving multiple manual steps, are prone to inaccuracies and technical errors that may compromise the prosthesis fit, patient comfort, and long-term clinical success. The advent of computer-aided design and computer-aided manufacturing (CAD/CAM), along with direct metal laser sintering (DMLS), has enabled the fabrication of high-precision frameworks and extracoronal precision attachm

Functional and aesthetic rehabilitation with maxillary prosthesis supported by two zygomatic implants for maxillary defect resulting from cancer ablative surgery: a case report/technique article.

We report herein a case of functional and aesthetic rehabilitation with maxillary prosthesis supported by only two zygomatic implants for a patient with severe maxillary defect resulting from subtotal maxillectomy for malignant melanoma of the upper gingiva. A 76-year-old woman was referred to our facility with non-painful discoloration of the upper gingiva. After several examinations, a clinical diagnosis of malignant melanoma of the upper gingiva (cT3N1M0 stage IVA, American Joint Committee on Cancer) was made. Subtotal maxillectomy utilizing Le Fort I osteotomy and functional neck dissectio

The milled implant bar: an alternative to spark erosion.

Patients who cannot tolerate total coverage of the hard palate or whose maxillary arches are poorly formed, because of congenital, developmental or surgical defects, may be unable to wear a conventional complete denture. These patients can be successfully treated with implant-supported prostheses that cover only a minimal amount of palatal tissue. With spark-eroded castings, very precise restorations can be constructed to fit such implant supports. However, these castings are so expensive that cost precludes their use for many patients. This article presents an alternative approach, developed

Outcome of debridement and retention in prosthetic joint infections by methicillin-resistant staphylococci, with special reference to rifampin and fusidic acid combination therapy.

The management of prosthetic joint infections remains a clinical challenge, particularly infections due to methicillin-resistant staphylococci. Previously, this infection was considered a contraindication to debridement and retention strategies. This retrospective cohort study examined the treatment and outcomes of patients with arthroplasty infection by methicillin-resistant staphylococci managed by debridement and retention in conjunction with rifampin-fusidic acid combination therapy. Over an 11-year period, there were 43 patients with infection by methicillin-resistant staphylococci manage

A Qualitative Study on Patients' Perceptions of Two Types of Attachments for Implant Overdentures.

The aim of this qualitative study was to gain a deeper understanding of patient perceptions of wearing implant-retained overdentures with ball-shaped or cylindrical attachment systems. Twenty-two wearers of implant-supported overdentures participated in this qualitative study based on a randomized crossover clinical trial that aimed to compare a cylindrical attachment and a ball attachment. In phase I of the study, group A experienced ball attachments (n = 11) and group B Locator attachments (n = 11) for 1 year. Afterward, in phase II, the attachments were changed; group A received Locator att

Radiographic evaluation of marginal bone levels around implants supporting splinted fixed bridges: A retrospective study on 412 implants.

OBJECTIVES: The effect of the implant position within the prosthesis on bone remodeling is scarcely documented so far. Thus, the aim of the present study was to investigate whether central implants may suffer higher peri-implant marginal bone levels (MBL) compared to laterals in case of fixed splinted bridges supported by ≥ three implants. MATERIALS AND METHODS: Partially edentulous subjects rehabilitated with at least one fixed bridge supported by ≥ three dental implants were enrolled. MBL was assessed radiographically by means of intraoral radiographs acquired with phosphor plates and impor

Antibiotic resistance in orthopaedic surgery: acute knee prosthetic joint infections due to extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae.

The aim of this study was to describe the prevalence and characteristics of knee prosthetic joint infections due to extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae. From 2000 to 2007, 132 infections out of 5,076 arthroplasties (2.6%) were registered. Seven out of 132 infections (5.3%) were due to ESBL-producing Enterobacteriaceae, Escherichia coli in six cases and Klebsiella pneumoniae in one. Open debridement and retention of the implant was the first surgical approach and all patients received intravenous carbapenems. Relapse was documented in four cases and remission in

The Management of Inflatable Penile Prosthesis Erosion and Infection following Iatrogenic Aspiration.

This case report presents a unique and previously unreported case of malfunction, infection, and erosion of an inflatable penile prosthesis (IPP) resulting from iatrogenic injury during a priapism aspiration procedure performed by an emergency medicine physician. The patient, a 75-year-old male with a history of IPP placement for erectile dysfunction, presented with urinary retention and priapism, leading to inadvertent deflation of the IPP during aspiration. Subsequent evaluation revealed a pinhole opening on the scrotum, indicating infection and erosion of the prosthesis tubing. The patient

The Soft Palate Friendly Speech Bulb for Velopharyngeal Insufficiency.

Velopharyngeal insufficiency is an anatomic defect of the soft palate making palatopharyngeal sphincter incomplete. It is an important concern to address in patients with bilateral cleft lip and palate. Speech aid prosthesis or speech bulbs are best choice in cases where surgically repaired soft palate is too short to contact pharyngeal walls during function but these prosthesis have been associated with inadequate marginal closure, ulcerations and patient discomfort. Here is a case report of untreated bilateral cleft lip and palate associated with palatal insufficiency treated by means of pal

Functional assessment of dental implant osseointegration.

Functional ankylosis of dental implants in alveolar bone is the current criterion to assess implant osseointegration from a biomechanical standpoint. In this literature review, the clinical significance and current available assessments of implant stability are discussed. However, these assessments demonstrate a variety of correlations to peri-implant structures and as such are difficult to translate to the clinical arena. Calculating the effective stiffness from homogenization of peri-implant tissues appears to be a more reliable approach to predict implant stability in preclinical studies, b

Technique to restore unfavorably inclined implants.

Unfavorable inclination of implants is a common problem that may compromise esthetics, phonetics, and function of the implant-supported fixed prosthesis. Several methods have been reported to compensate for malaligned implants; however, most techniques are complicated and expensive. A technique that combines a custom abutment from a hexed UCLA-type plastic burn-out pattern and a manually threaded setscrew hole is a relatively inexpensive and uncomplicated approach to eliminate problems caused by unfavorable implant inclination. This article describes the procedures for fabrication of an implan

Is Resonance Frequency Analysis a Reliable Evaluation for Primary Stability of Implants Without Apical Contact?

PURPOSE: The primary stability of dental implants is one of the most crucial factors for providing long-term success of osseointegration. Vertical deficiencies, such as those due to maxillary sinus pneumatization, may cause a severe vertical limitation to residual bone height. This study aimed to examine the primary stabilization of implants without apical contacts. MATERIALS AND METHODS: Eighty bone-level implants (4.1-mm diameter/10-mm length) were placed into polyurethane test blocks without apical contacts. According to coronal bone-to-implant contact, groups were set as 4, 6, 8, and 10 m

Esthetic 7/8 crown--the Tucker technique.

As restorative dentists, one of the largest challenges we face is to restore proper form and function while considering the impact on the esthetic outcome. One must consider the clinical strengths and weaknesses of various materials and their applicability in different situations. For each clinical situation, there is always a most favorable choice coordinating material and design for optimum physiological, functional and esthetic results. The cast gold 7/8 crown with esthetic buccal margin, as designed and advocated by Dr Richard V Tucker, is an extremely valuable procedure, providing a very
