A proximal tibial skeletal traction in the treatment of femoral shaft fractures in the growing child has been reported to result in complications such as knee pain, knee joint subluxation, and growth disturbance. For the last 15 years, the authors used a single screw (instead of the Kirschner wire) inserted in the tibia perpendicular to the surface and well below the tibial tubercle physis. With the traction applied to this screw and the leg in a 90-90 position or resting on a Braun splint, none of the previously described complications were observed. This modification of the proximal tibial s

BACKGROUND: The optimal surgical treatment for patients presenting with (impending and complete) pathological proximal femoral fractures is predicated on prognosis. Guidelines recommend a preoperative biopsy to exclude sarcomas, however no evidence confirms a benefit.
OBJECTIVE: This study aimed to describe the diagnostic accuracy, morbidity and sarcoma incidence of biopsy results in these patients.
MATERIAL AND METHODS: All patients (n = 153) presenting with pathological proximal femoral fractures between 2000 and 2019 were retrospectively evaluated. Patients after inadvertent surgery (n =

BACKGROUND: To assess outcomes after uncemented hemiarthroplasty stems in the treatment of intracapsular femoral neck fractures over an 11-year period. Mortality rates were assessed, and whether proximal femoral geometry and stem alignment were factors in intraoperative or postoperative periprosthetic fracture (PPF) needs to be identified.
MATERIALS AND METHODS: A retrospective single-center observational study was conducted of all patients who underwent intracapsular femoral neck fracture treatment using an uncemented prosthesis between January 2008 and December 2018. Primary endpoints inclu

BACKGROUND: The treatment for intertrochanteric femoral fractures (IFF) among the elderly has been a controversial topic. Hemiarthroplasty (HA) and proximal femoral nail antirotation (PFNA) have their own advantages in the management of IFF. Hence, this study aims to compare and analyze differences in the effectiveness of both procedures on IFF among the elderly.
METHODS: Overall, 99 patients (81.09 ± 8.29 years; 68 women) underwent HA or PFNA from January 2016 to May 2020. IFF were classified according to the Arbeitsgemeins für Osteosynthesefragen (AO) classification. The difference in under

PURPOSE: Proximal femoral fractures (PFF) are a global health burden in aging populations. Evidence on the prevalence and clinical relevance of concomitant fractures in this population remains limited. This study investigated the impact of concomitant fractures on mortality, complications, transfusion requirements, and postoperative mobility in patients with geriatric PFF.
METHODS: This single-center cohort study included patients ≥ 65 years with a positive geriatric screening, who underwent surgery for a PFF after low-energy trauma between January 2019 and December 2023. Patients with concom

Purpose: Surgical treatment is the main treatment method for femoral intertrochanteric fractures (FIFs), however, there are lots of implant-related complications after surgery. Our team designed a new intramedullary nail (NIN) to manage such fractures. The purpose of this study was to introduce this new implant and compare it with proximal femoral nail antirotation (PFNA) and InterTAN for treating FIFs. Methods: An AO/OTA 31-A1.3 FIF model was built and three fixation models were created via finite element method, comprising PFNA, InterTAN, and the NIN. Vertical, anteroposterior (A-P) bending,

BACKGROUND: Proximal femoral fracture is a common condition in the elderly but very little is known about fracture-related hip pain in these patients after discharge from stationary treatment.
AIMS: To identify risk factors associated with persistent hip pain in elderly hip-fracture patients.
METHODS: We analysed data from a large observational study, evaluating the health care situation of hip-fracture patients between January 2002 and September 2003 in Germany. For this analysis, we focused on a sub-sample of patients who were 65 years or older, had sustained an isolated proximal femoral f

BACKGROUND/OBJECTIVE/INTRODUCTION: Hip fractures in older patients are associated with substantial morbidity and in-hospital mortality. Despite the clinical importance of this condition, few tools specifically estimate in-hospital mortality or account for clinical deterioration during hospitalization, particularly in middle-income settings. This study aimed to develop a dynamic prognostic score to estimate the risk of in-hospital mortality among older patients undergoing surgical treatment for hip fractures.
MATERIALS AND METHODS: A retrospective cohort study was conducted with 1,509 patients

BACKGROUND: With population aging, intertrochanteric fractures (ITFs) in elderly individuals are increasingly common, and the choice between proximal femoral nail antirotation (PFNA) and hemiarthroplasty (HA) remains under debate, particularly regarding the optimal balance between early mobilization and the risk of long-term implant failure or revision. This study aimed to investigate this trade-off through a retrospective cohort comparison of PFNA and HA in elderly ITF patients stratified by fracture stability. METHODS: This retrospective cohort study reviewed 372 elderly patients with ITF tr

Lower limb amputation is associated with progressive loss of bone mineral density (BMD) in the residual limb, particularly at the amputated-side proximal femur. However, proximal femoral fractures in working-age individuals with transfemoral amputation who are independently ambulatory with a prosthesis remain insufficiently described. We report the cases of two working-age men who sustained ipsilateral proximal femoral fractures after minor falls at home while not wearing their prostheses. Case 1 sustained a left femoral neck fracture classified as Garden grade I six years after amputation, wh

Case series summaryFour domestic cats treated surgically for complex fractures of the proximal femur were included in this retrospective case series. Medical records from October 2021 to January 2023 were reviewed for cats presenting with high-energy trauma resulting in comminuted proximal femoral fractures. All cats underwent surgical stabilisation using a lateral approach extended by complete release of the vastus lateralis muscle to improve visualisation and access. Follow-up included physical and radiographic evaluation and completion of the Feline Musculoskeletal Pain Index (FMPI). The mo
