Xanthogranulomatous pyelonephritis (XGP) is a rare, severe form of chronic kidney infection that mimics renal malignancies, often leading to diagnostic challenges. We present the case of a 36-year-old woman with a 2-month history of progressive lower back pain, fever, and weight loss. Imaging revealed left renal enlargement with multiple low-attenuation areas and large staghorn calculi, suggestive of XGP. Xanthogranulomatous pyelonephritis is a rare variant of pyelonephritis that often requires radical surgery. We report a case of a 51-year-old female patient who was managed with prolonged placement of nephrostomy tubes prior to surgery. This case illustrates the importance of surgical optimization of a poor operative candidate and the use of nephrostomy tubes as a temporizing management strategy.

Xanthogranulomatous pyelonephritis (XGP) is a rare histological subset of pyelonephritis characterized by being a chronic destructive granulomatous inflammation of the renal parenchyma. XGP is classified according to the extent of disease into two entities: within the renal cortex (focal or segmental XGP) or diffuse spread with pelvic communication (diffuse XGP). We present the first case, up to our knowledge, of XGP in pregnant woman, in solitary kidney (related living kidney donor). Therapeutic abortion was not resolutive and nephron sparing surgery (espeleostomy) was successfully performed. Fourteen years later the renal function is still normal and 2/3 of renal parenchima are preserved. Literature review is pointed out.

Focal xanthogranulomatous pyelonephritis is an unusual form of chronic renal infection that is difficult to diagnose prior to surgery. Association of renal cell carcinoma (RCC) with renal calculi is very rare. Herein we report a case of RCC developing in a non-functioning kidney containing multiple renal and ureteric calculi. Histologically, the kidney, in addition, revealed changes of xanthogranulomatous pyelonephritis (XGP). XGP can be confused clinically, grossly and microscopically with RCC. In the present case both were coexistent and thus make it interesting.

Replacement lipomatosis (RPL) is seen in patients in whom renal parenchyma is destroyed due to chronic calculous disease and inflammation. The triggering mechanism for xanthogranulomatous pyelonephritis (XGP) is also the same. We report a case in which RPL and XGP coexist in the same kidney. To our knowledge, this coexistence has not been previously reported.

We present herein a case of superior vena cava (SVC) syndrome caused by localized fibrosing mediastinitis (FM), which had histological features similar to xanthogranulomatous pyelonephritis (XGP). The association of xanthogranulomatous pyelonephritis (XPN) and systemic amyloidosis is extremely rare. This association has been described in only six cases.
