RapidArc, SmartArc and TomoHD compared with classical step and shoot and sliding window intensity modulated radiotherapy in an oropharyngeal cancer treatment plan comparison.

BACKGROUND: Radiotherapy techniques have evolved rapidly over the last decade with the introduction of Intensity Modulated RadioTherapy (IMRT) in different forms. It is not clear which of the IMRT techniques is superior in the treatment of head and neck cancer patients in terms of coverage of the planning target volumes (PTVs), sparing the organs at risk (OARs), dose to the normal tissue, number of monitor units needed and delivery time. The present paper aims to compare Step and Shoot (SS) IMRT, Sliding Window (SW) IMRT, RapidArc (RA) planned with Eclipse, Elekta VMAT planned with SmartArc (SA

A novel dose calculation system implemented in image domain.

PURPOSE: Modern intensity-modulated radiotherapy, aiming to deliver an accurate dose to the planning target volume while protecting the surrounding organs at risk, is regarded as the indispensable treatment for cancer in the clinic. An accurate and efficient dose calculation algorithm is essential to support and accelerate the treatment plan optimization process. METHOD: To improve the effectiveness of the dose calculation, deep learning methods are widely adopted to compute dose distributions from several variables mimicking the physical features required for dose calculation. However, these

An optimized framework for cone-beam computed tomography-based online evaluation for proton therapy.

BACKGROUND: Clinical evidence has demonstrated that proton therapy can achieve comparable tumor control probabilities compared to conventional photon therapy but with the added benefit of sparing healthy tissues. However, proton therapy is sensitive to inter-fractional anatomy changes. Online pre-fraction evaluation can effectively verify proton dose before delivery to patients, but there is a lack of guidelines for implementing this workflow. PURPOSE: The purpose of this study is to develop a cone-beam CT-based (CBCT) online evaluation framework for proton therapy that enables knowledge tran

Effect of Deformation Methods on the Accuracy of Deformable Image Registration From Kilovoltage CT to Tomotherapy Megavoltage CT.

INTRODUCTION: The registration accuracy of megavoltage computed tomography images is limited by low image contrast when compared to that of kilovoltage computed tomography images. Such issues may degrade the deformable image registration accuracy. This study evaluates the deformable image registration from kilovoltage to megavoltage images when using different deformation methods and assessing nasopharyngeal carcinoma patient images. METHODS: The kilovoltage and the megavoltage images from the first day and the 20th fractions of the treatment day of 12 patients with nasopharyngeal carcinoma w

Spiral volumetric modulated arc therapy enhances peak-to-valley dose contrast for 3D-GRID radiotherapy compared to VMAT and tomotherapy.

PURPOSE: Three-dimensional GRID radiotherapy for bulky tumours requires high peak dose while effectively preserving a low-dose intratumoural valley, but this remains challenging with current linac-based techniques. Spiral volumetric modulated arc therapy (sVMAT) is an integrated delivery technique with synchronized longitudinal couch motion and gantry rotation that generates a spiral/helical beam trajectory, combining key advantages of volumetric modulated arc therapy (VMAT) and helical tomotherapy (TOMO). This study evaluated the dosimetric feasibility of sVMAT for single-fraction 3D-GRID rad

Treatment planning parameters in intensity modulated radiotherapy for prostate cancer and their dosimetric impact: a study using Monaco TPS.

This study aimed to determine the most appropriate treatment planning solution for prostate cancer radiotherapy by analyzing the effect of planning parameters on dosimetric outcome. Four volumetric modulated arc therapy (VMAT) / intensity modulated radiation therapy (IMRT) plans were created for each of the 20 patients included in the study, using minimum segment width (MSW) of 0.5, 1.0, 1.5, and 2.0 cm. All prostate plans underwent "Fluence Smoothing" on "High", while plans using MSW = 1.0 were reoptimized with both "Low" and "Medium" settings. All plans met the planning objectives. MSW = 0.5

New energy layer reduction method for rapid intensity-modulated proton therapy (IMPT) using a ridge filter.

BACKGROUND: Efficient delivery of intensity-modulated proton therapy (IMPT) plays a pivotal role in improving the effectiveness of proton therapy. Accelerating delivery not only increases patient throughput but also improves the ability to manage dynamic target motion and reduces uncertainties, thereby enhancing treatment accuracy. Additionally, rapid delivery enables the effective use of breath-hold techniques, allowing for smaller target margins and providing better protection for healthy tissues. Since energy layer switching time constitute the majority of delivery time, reducing the number

Impact of delivery variations on 3D dose distributions for volumetric modulated arc therapy plans of various complexity.

BACKGROUND: Delivery variations during radiotherapy can cause discrepancies between planned and delivered dose distribution. These variations could arise from random and systematic offsets in certain machine parameters or systematic offsets related to the calibration process of the treatment unit. PURPOSE: The aim of this study was to present a novel simulation-based methodology to evaluate realistic delivery variations in three dimensions (3D). Additionally, we investigated the dosimetric impact of delivery variations for volumetric modulated arc therapy (VMAT) plans for different treatment

Using dose volume histogram (DVH) predictions to improve the plan quality of helical tomotherapy (HT).

This study aimed to investigate whether the RapidPlan (RP) model configured by volumetric modulated arc therapy (VMAT) plans of nasopharyngeal carcinoma (NPC) could be used to assist in the optimization of HT plans and improve their quality. An RP model was trained using 100 clinically accepted VMAT plans of NPC patients. The predicted dose constraints of the VMAT trained RP model were used to reoptimize 25 consecutive clinically accepted HT plans (HT_clinical) and perform new VMAT plans based on the same computed topography (CT). The dosimetric quality of the reoptimized HT plans (HT_reoptimi

Determining tolerance levels for quality assurance of 3D printed bolus for modulated arc radiotherapy of the nose.

Given the existing literature on the subject, there is obviously a need for specific advice on quality assurance (QA) tolerances for departments using or implementing 3D printed bolus for radiotherapy treatments. With a view to providing initial suggested QA tolerances for 3D printed bolus, this study evaluated the dosimetric effects of changes in bolus geometry and density, for a particularly common and challenging clinical situation: specifically, volumetric modulated arc therapy (VMAT) treatment of the nose. Film-based dose verification measurements demonstrated that both the AAA and the AX
