Choosing Appropriate Nutritional Therapy for Patients With Anorexia Nervosa Exhibiting Liver Dysfunction: A Case Report.

Anorexia nervosa (AN) presents with a variety of physical complications such as hypoglycemia, electrolyte abnormalities, and dehydration associated with starvation, requiring rapid weight gain through nutritional therapy. However, despite nutritional therapy, patients are at risk of many serious medical complications, including hypoglycemia, hypophosphatemia, edema, and liver damage. Starvation has been found to cause hepatocyte injury with mild-to-severe increases in liver enzyme levels, and distinguishing between autophagy and refeeding syndrome is important for treatment strategies. Herein,

Rapid refeeding does not worsen anxiety in adolescents with anorexia nervosa: a pilot study.

The study aimed to describe the progression of state anxiety in adolescents with anorexia nervosa (AN) hospitalized on a high calorie refeeding (HCR) protocol. Participants, 12-21 years, admitted for malnutrition due to AN were placed on a HCR protocol in which calories were advanced by 300 kcal/day. The State-Trait Anxiety Inventory for Children (STAIC) was given to participants within 24 hours of hospitalization and the state anxiety component of the STAIC was administered daily immediately before and after breakfast until discharge. Of 22 patients enrolled, 86% were female, mean age was 14.

Marked elevation of transaminases and pancreatic enzymes in severe malnourished male with eating disorder.

We report a case of a 45 year old Caucasian malnourished male with an history of eating disorder who developed severe liver and pancreatic damage and multiorgan disfunction. At admission to our department, his body mass index (BMI) was 11.1. Biochemical evaluation showed elevated serum levels of transaminases (AST= 2291 U/L, ALT= 1792 U/L), amylase (3620 U/L), lipase (4102 U/L), CPK= 1370 U/L, LDH= 2082 U/L. No other cause of acute liver and pancreatic damage was evidenced. Haematological disorders (anemia, thrombocytopenia, leukopenia) found on admission seem related to bone marrow hypoplasia

Anorexia nervosa and heart disease: a systematic review.

Anorexia nervosa (AN) is an eating disorder that most frequently afflicts females in adolescence. In these subjects, cardiovascular complications are the main cause of morbidity and mortality. Aim of this review is to analyze the hemodynamic, pro-arrhythmic and structural changes occurring during all phases of this illness, including re-feeding. A systematic literature search was performed on studies in the MEDLINE database, from its inception until September 2017, with PUBMED interface focusing on AN and cardiovascular disease. This review demonstrated that the most common cardiac abnormaliti

Assessment and Treatment of Eating Disorders.

Eating disorders are serious psychiatric conditions in terms of chronicity and have the highest mortality rate among psychiatric disorders. The assessment and treatment of eating disorders are also challenging, due to patients' denial of their illness and reluctance for change. Despite a large number of previous assessment and treatment studies, new strategies to overcome these difficulties are still needed. This study casts light on four aspects; involvement of the brain's reward system, stages of change in relationship with motivation, refeeding syndrome during renourishment, and gut microbi

A clinical audit of changes in urine pH during the nutritional rehabilitation of adolescent and young adult patients hospitalised with a restrictive eating disorder.

BACKGROUND: Alkaline urine pH has previously been reported in patients with restrictive eating disorders (EDs) over 30 years ago, however its prevalence is not routinely reported. This study carries out a preliminary investigation of the change in urine pH in malnourished adolescent and young adult patients hospitalised with restrictive EDs during an initial two weeks of higher energy nutritional rehabilitation. METHODS: A retrospective review was carried out of the medical charts of malnourished adolescent and young adult patients (aged 14-21) hospitalised with restrictive EDs for ≥ 14 days

Hyperphosphatemia during nutrition recovery in patients with severe anorexia nervosa.

INTRODUCTION: Anorexia nervosa (AN) is a disorder associated with many medical complications. Regarding phosphorus metabolism, the only recognized alteration is hypophosphatemia associated with refeeding syndrome. However, in our clinical practice, we have observed a high frequency of hyperphosphatemia in late phases of nutrition therapy in severely undernourished AN patients, which has barely been described. MATERIALS AND METHODS: We carried out a retrospective study of patients with AN hospitalized for severe decompensation of the disease. RESULTS: Eleven patients were included, all women,

Severe gastric dilatation due to superior mesenteric artery syndrome in anorexia nervosa.

Forty-seven year old female, with a history of anorexia nervosa, was admitted to a medical stabilization unit (ACUTE) complaining of abdominal pain exacerbated by oral intake, associated with nausea, and relieved by emesis. Admission body mass index was 10.6. Labs were notable for hepatitis and hypoglycemia. On her progressive oral refeeding plan, she suddenly developed severe abdominal pain. Computed tomography (CT) revealed gastric dilatation and superior mesenteric artery (SMA) syndrome. SMA syndrome is a rare complication of severe malnutrition resulting from compression of the duodenum be

Neonatal short bowel syndrome: 'rectal feeding' in order to stimulate intestinal growth.

UNLABELLED: Short bowel syndrome (SBS) is characterized by a state of malabsorption following extensive resection of the small bowel, resulting in insufficient nutritive supply requiring artificial nutrition with long-term parenteral nutrition. Here we present an illustrative case report of a premature infant born with gastroschisis and SBS, who was treated with enteral refeeding via rectum. The infant developed during the period of rectal feeding with jejunostomy loses bowel lengthening and could be fed orally within a few months after birth. Rectal feeding with ostomy loses could stimulate b

Severe anorexia nervosa in males: clinical presentations and medical treatment.

The clinical presentation and medical complications of severe anorexia nervosa among males were examined to further the understanding of this increasingly prevalent condition. Fourteen males were admitted to a medical stabilization unit over the study period. Males with severe anorexia nervosa were found to have a multitude of significant medical and laboratory abnormalities, which are in need of treatment via judicious, nutritional rehabilitation and weight restoration to prevent additional morbidity and to facilitate transfer and admission to traditional eating disorder programs.

A Missed Opportunity: Severe Malnutrition in an Infant Despite Routine Care Access.

Severe malnutrition of an infant is rare in Canada but poses a major public health concern. This case report describes a 5-month-old male presenting with a rash, found to be severely malnourished with hypoglycemia, bradycardia, hypothermia, ketosis, and metabolic acidosis. Malnutrition was likely due to inadequate breastmilk supply and underfeeding. The infant was treated with intravenous fluids and carefully managed feeding plans while closely monitoring biochemical markers. His nutritional status gradually improved, normalizing symptoms and lab values. This case highlights the importance of

[Review of nutritional and dietary management of anorexia nervosa].

While numerous studies have highlighted the need to approach anorexia nervosa from a multidisciplinary perspective, the dietary and nutritional aspects of the disorder are rarely considered in depth. Basic guidelines to monitor food intake of patients, are available. A literature review was performed in Medline, searching for articles related to the dietary and nutritional management of anorexia nervosa and published over the last five years. The final analysis focused on 102 articles. This review summarizes the different therapeutic contexts and objectives, nutritional support, the use of die
