Objectifying the Pivot Shift Test.

The pivot shift test is utilized for assessment of rotatory instability in the anterior cruciate ligament (ACL) deficient knee. There are multiple reports of the pivot shift maneuver, and there is a lack of consensus among clinicians as to a standardized maneuver. Measurement devices are a feasible option to evaluate rotatory knee instability, objectively or quantitatively. Traditionally, measurement systems have been invasive systems. More recently, electromagnetic system, inertial sensor, or imaging analysis systems, specifically with the utilization of a tablet computer, have emerged as non

Common Running Injuries: Evaluation and Management.

Running is a common form of exercise but predisposes athletes to several running-related injuries. Most running injuries are due to overuse and respond to conservative treatment. Tendinopathies in the patellar, Achilles, and hamstring tendons are common, and are primarily treated with eccentric exercise. Iliotibial band syndrome and patellofemoral pain syndrome are less common than patellar tendinopathy and are treated by strengthening exercises for the core and legs in addition to flexibility exercises. Acute hamstring strains and medial tibial stress syndrome require a period of relative res

Kinesiophobia and Return to Sports After Anterior Cruciate Ligament Reconstruction.

Anterior cruciate ligament (ACL) reconstruction is typically recommended for patients who wish to return to aggressive athletic activity. Unfortunately, reconstructive knee surgery is not a guarantee that all patients will return to their preinjury level of function. A recent meta-analysis including 48 studies showed that after a mean follow-up of 41 months, 82% of participants had returned to some kind of athletic activity but only 63% returned to their preinjury level of participation and a disappointing 44% returned to competitive sports. The reasons why some athletes have been unsuccessful

Occupational repetitive strain injury.

Repetitive strain injuries include a group of disorders that most commonly develop in workers using excessive and repetitious motions of the neck and upper extremity. A careful occupational history, physical examination and specific diagnostic maneuvers can help distinguish these musculoskeletal injuries from rheumatologic diseases, psychologic disorders, acute joint or tendon inflammation from other causes and single-event traumatic sprains and strains. Repetitive strain injury may be manifested by cervical syndrome, tension neck syndrome, thoracic outlet syndrome and frozen shoulder syndrome

Operative treatment of acute and chronic knee problems.

Attention to detail and anatomic precision is essential to accurate diagnosis. Accurate diagnosis is the keystone of care for the acute or chronic knee problem. The arthroscope is an essential diagnostic tool. Uniplanar instability can usually be treated nonoperatively with good results providing that secondary supporting structures are not anatomically disrupted, the ligaments are protected during their plastic healing phase, and the patient is committed to a complete rehabilitation program. Rotatory instabilities require surgery if a high functional level is desired by the patient, and the s

Return to Sport After Revision Anterior Cruciate Ligament Reconstruction: A Systematic Review and Meta-analysis.

BACKGROUND: Revision anterior cruciate ligament (ACL) reconstruction (ACLR) is a well-established procedure to restore knee stability and improve function after a failed primary ACLR. In active individuals, patient, injury, and operative variables influencing successful return to sport (RTS) after revision ACLR remain poorly understood. PURPOSE: To evaluate RTS outcomes in patients undergoing revision ACLR. STUDY DESIGN: Meta-analysis, Level of evidence, 4Methods:A systematic review was conducted in accordance with the 2020 PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-An

Association of Insurance Type and PROMIS Scores Following Ankle Fracture Surgery: A Retrospective Comparative Study.

BACKGROUND: It remains unclear whether insurance as a proxy for socioeconomic status and social determinants of health is associated with functional outcomes after ankle fracture surgery. This study assesses the association between insurance and Patient-Reported Outcomes Measurement Information System (PROMIS) measures of physical function (PF) and pain interference (PI) after ankle fracture surgery. METHODS: All patients who underwent ankle fracture surgery (2016-2021) were reviewed retrospectively. Multiple injuries, open fractures, pilon variants, and nonanatomically reduced fractures were

Return to Sports Rates in Pediatric Patients After Bilateral Versus Unilateral Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: Patellofemoral instability commonly occurs during sports activities. The return to sports (RTS) rate for pediatric patients after bilateral medial patellofemoral ligament reconstruction (MPFLR) is unknown. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate RTS outcomes for pediatric patients undergoing bilateral MPFLR. It was hypothesized that (1) fewer pediatric patients would RTS after bilateral MPFLR compared with unilateral MPFLR and that (2) for those in the bilateral cohort who were able to RTS, fewer patients would attain the same level of play as or higher level

Augmentation of Primary ACL Reconstruction With a Modified Ellison Lateral Extra-articular Tenodesis in High-Risk Patients: A Pilot Study.

BACKGROUND: Lateral extra-articular tenodesis (LET) has been used to augment primary anterior cruciate ligament (ACL) reconstruction to reduce the risk of reinjury. Most LET procedures result in a construct that is fixed to both the femur and the tibia. In a modified Ellison procedure, the construct is only fixed distally, reducing the risk of inadvertently overconstraining the lateral compartment. PURPOSE: To evaluate the use of the modified Ellison procedure in a cohort of patients deemed to be at a high risk of further ACL injury after primary ACL reconstruction. STUDY DESIGN: Case series

Collateral Ligament Knee Injuries in Pediatric and Adolescent Athletes.

BACKGROUND: The majority of research on medial (MCL) and lateral (LCL) collateral ligament injuries has focused on adults and combined collateral/cruciate injuries. The purpose of this study was to determine characteristics associated with isolated collateral ligament injuries in adolescents, and assess timing for return to sports. METHODS: Electronic medical records were queried to identify patients aged below 17 years who sustained a magnetic resonance imaging-confirmed isolated MCL or LCL injury over an 8-year period. Retrospective review then documented patient and injury characteristics

Selective bundle reconstruction for symptomatic partial anterior cruciate ligament tears demonstrates good functional scores, high return to sport rates and a low re-tear rate.

BACKGROUND: The treatment of symptomatic partial anterior cruciate ligament (ACL) tears remains controversial. This study investigated the clinical and functional outcome of patients undergoing selective posterolateral (PLB) or anteromedial (AMB) bundle reconstruction. METHODS: 55 consecutive patients underwent partial bundle reconstruction, of which 45 (AMB = 17, PLB = 28) were included in the current analysis (62% male, mean age of 29.1 years). Patients were assessed at 12 and 24 months with patient-reported outcome measures (PROMs), graft laxity (KT-1000), knee extensor and flexor torque a

Greater rate of return to play and re-injury following all-inside meniscal repair compared to the inside-out technique: a systematic review.

INTRODUCTION: Inside-out and all-inside arthroscopic meniscal repairs are widely performed. However, it remains unclear which method promotes greater clinical outcomes. This study compared inside-out versus all-inside arthroscopic meniscal repair in terms of patient-reported outcome measures (PROMs), failures, return to play, and symptoms. METHODS: This systematic review was conducted in accordance with the PRISMA guidelines. Two authors independently performed the literature search by accessing the following databases: PubMed, Google Scholar, and Scopus in February 2023. All clinical studies

Serratus Anterior Muscle Injury in an Overhead-Throwing Athlete: A Report of a Rare Case With Serial Ultrasonographic Observations.

Serratus anterior muscle injuries in overhead athletes are extremely rare, and no consensus exists regarding treatment strategies or return-to-play timelines. We report the case of a high school baseball pitcher who sustained a serratus anterior injury. Ultrasonography revealed swelling and a hypoechoic area in the lower fibers of the serratus anterior at the level of the seventh and eighth ribs. Based on these findings, conservative management with localized compression was initiated, without the use of any medications, including nonsteroidal anti-inflammatory drugs. Serial ultrasonographic m

The Structure, Function, and Adaptation of Lower-Limb Aponeuroses: Implications for Myo-Aponeurotic Injury.

The aponeurosis is a large fibrous connective tissue structure within and surrounding skeletal muscle and is a critical component of the muscle-tendon unit (MTU). Due to the lack of consensus on terminology and the heterogeneous nature of the aponeurosis between MTUs, there are several questions that remain unanswered. For example, the aponeurosis is often conflated with the free tendon rather than being considered an independent structure. This has subsequent implications when interpreting data regarding the structure, function, and adaptation of the aponeuroses from these studies. In recent

Human pluripotent stem cell-derived myogenic progenitors undergo maturation to quiescent satellite cells upon engraftment.

Human pluripotent stem cell (hPSC)-derived myogenic progenitor cell (MPC) transplantation is a promising therapeutic approach for a variety of degenerative muscle disorders. Here, using an MPC-specific fluorescent reporter system (PAX7::GFP), we demonstrate that hPSC-derived MPCs can contribute to the regeneration of myofibers in mice following local injury and in mice deficient of dystrophin (mdx). We also demonstrate that a subset of PAX7::GFP MPCs engraft within the basal lamina of regenerated myofibers, adopt a quiescent state, and contribute to regeneration upon reinjury and in mdx mouse

Editorial Commentary: Indications for Lateral Extra-Articular Tenodesis in Primary Anterior Cruciate Ligament Reconstruction.

Lateral extra-articular tenodesis and other anterolateral procedures improve patient outcomes when combined with anterior cruciate ligament reconstruction. Failure rates are >50% lower in young patients at high risk of reinjury. We defined patients at high risk as those aged 14 to 25 years with 2 or more factors of the following criteria: 1) returning to contact pivoting sport, 2) high-grade anterolateral rotatory laxity, as measured by pivot shift test (grade 2 or higher), and 3) generalized ligamentous laxity (Beighton score greater than 4) or knee hyperextension recurvatum of greater than 1

Knotless Repair of Achilles Tendon Rupture in an Elite Athlete: Return to Competition in 18 Weeks.

Rupture of the Achilles tendon is an increasingly common injury, particularly in physically active males, and current evidence favors minimally invasive surgical repair. We describe the case of a 36-year-old male elite bobsled athlete with complete rupture of the Achilles tendon. He was treated with surgical repair of the ruptured tendon using an innovative, minimally invasive procedure based on an internal bracing concept and was able to undergo early mobilization and aggressive physiotherapy rehabilitation. His recovery was such that he returned to training at 13 weeks postoperatively and pa

National Athletic Trainers' Association position statement: conservative management and prevention of ankle sprains in athletes.

OBJECTIVE: To present recommendations for athletic trainers and other allied health care professionals in the conservative management and prevention of ankle sprains in athletes. BACKGROUND: Because ankle sprains are a common and often disabling injury in athletes, athletic trainers and other sports health care professionals must be able to implement the most current and evidence-supported treatment strategies to ensure safe and rapid return to play. Equally important is initiating preventive measures to mitigate both first-time sprains and the chance of reinjury. Therefore, considerations fo

Psychological and Physical Readiness Profiles for Return to Sport in Patients After Anterior Cruciate Ligament Reconstruction: An Analysis of 822 Patients.

BACKGROUND: Psychological factors influence return to sport (RTS) and reinjury rates following anterior cruciate ligament (ACL) reconstruction (ACLR). However, many RTS decisions are made without considering psychological factors, or consider psychological and physical factors in isolation, leading to suboptimal RTS and reinjury rates. PURPOSE: To identify distinct profiles of psychological and physical readiness to RTS following ACLR using strength- and performance-based measures of physical readiness and to examine the extent to which these readiness domains align.
