Predicting long-term treatment with short-acting opioids: the role of general practice consultations and sociodemographic factors.

BACKGROUND: Long-term treatment with short-acting opioids is not recommended for patients with chronic nonmalignant pain. General practitioners should assess patient's risk of developing long-term treatment prior to prescribing. AIM: To identify predictors of long-term opioid treatment among adults with nonmalignant pain initiating treatment with short-acting opioids, focusing on general practice consultations and sociodemographic factors. METHODS: A national register-based repeated cross-sectional linear probability model was employed. The study population comprises adult, nonmalignant pain

Hybrid Hospital-at-Home Program in Singapore: Ethnographic Study.

BACKGROUND: Hospital-at-Home (HaH) programs deliver hospital-level care in the home setting, traditionally emphasizing high-touch, in-person clinical visits. In recent years, especially during the COVID-19 pandemic, HaH models have rapidly evolved to incorporate digital technologies, enabling remote consultations and monitoring as part of care delivery. While both in-person and remote HaH modalities have individually shown positive outcomes with patient experience and clinical safety, less is known about how hybrid models, combining both modalities, are experienced by patients and caregivers.

Sociodemographic variation in the association between modes of access and patient experience of primary care: a retrospective cross-sectional patient-level analysis of the General Practice Patient Survey in England in 2023.

BACKGROUND: Patient experience is a key indicator of healthcare quality. Access to general practice (GP) has shifted with increasing use of digital tools for contacting practices and more remote consultations. These changes may improve patient experience, but unequally across sociodemographic groups. This study examines how patient experience of GP varies by access mode and sociodemographic characteristics. METHODS: Retrospective cross-sectional analysis of 759 149 responses from the 2023 GP Patient Survey in England. Mixed-effects logistic regression was used to examine associations between

Optimisation of Remote Monitoring Programmes in Heart Failure: Evaluation of Patient Drop-Out Behaviour and Healthcare Professionals' Perspectives.

Heart failure (HF) is a growing epidemic, affecting millions of people worldwide, and is a major cause of mortality, morbidity, and impaired quality of life. Traditional cardiac rehabilitation is a valuable approach to the physical and quality-of-life recovery of patients with cardiovascular disease. The innovative approach of remote monitoring through telemedicine offers a solution based on modern technologies, enabling continuous collection of health data outside the hospital environment. Remote monitoring devices present challenges that could adversely affect patient adherence, resulting in

The development of a culturally appropriate school based intervention for Australian Aboriginal children living in remote communities: A formative evaluation of the Alert Program® intervention.

BACKGROUND/AIM: Although previous research has demonstrated the benefits of targeting self-regulation in non-Aboriginal children, it is unclear whether such programs would be effective for Aboriginal children attending school in remote communities. Some of these children have been diagnosed with a fetal alcohol spectrum disorder (FASD) impairing their ability to self-regulate. The aim of this article is to describe a three phase formative process to develop and pilot a curriculum version of the Alert Program® , a promising intervention for improving self-regulation that could be used in remote

A randomised trial of point-of-care tests for chlamydia and gonorrhoea infections in remote Aboriginal communities: Test, Treat ANd GO- the "TTANGO" trial protocol.

BACKGROUND: High prevalence rates of Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) have been reported in Aboriginal people in remote and regional areas of Australia for well over two decades, and repeat positivity rates are high. To interrupt disease transmission and reduce the risk of complications, early diagnosis and treatment is important. However in many remote and regional areas there are long delays between testing for these curable sexually transmissible infections and providing treatment, due to both physical distance from laboratories and difficulties when recalling patie

Electronic Referrals and Digital Imaging Systems in Ophthalmology: A Global Perspective.

Ophthalmology departments face intensifying pressure to expedite sight-saving treatments and reduce the global burden of disease. The use of electronic communication systems, digital imaging, and redesigned service care models is imperative for addressing such demands. The recently developed Scottish Eyecare Integration Project involves an electronic referral system from community optometry to the hospital ophthalmology department using National Health Service (NHS) email with digital ophthalmic images attached, via a virtual private network connection. The benefits over the previous system in

Attitudes towards telemedicine in ophthalmology: a population-based online survey in Germany.

BACKGROUND: Despite the increasing impact of telemedicine services on global healthcare provision, little data on acceptance and preference for telemedicine services at the population level are available to date. This is especially important for telemedicine in ophthalmology, where commercial telemedical applications are regularly offered outside of the state-funded health care system. Our study investigates the acceptability of tele-ophthalmology to the German population and associated sociodemographic factors. METHODS: We conducted a cross-sectional survey via an online panel to assess attit

Training of Non-expert Users Using Remotely Delivered, Point-of-Care Tele-Ultrasound: A Proof-of-Concept Study in 2 Canadian Communities.

Many physicians, particularly those practicing in remote regions, lack training opportunities to develop point-of-care ultrasound (POCUS) skills. This pretest-posttest study quantified the skill improvement of learners after participating in a virtual training program that used real-time, remotely delivered point-of-care tele-ultrasound (tele-POCUS) for teaching and learner feedback provision. Ten physicians practicing in an urban tertiary (Kingston, Ontario, Canada, n = 6) or remote care center (Moose Factory, Ontario, Canada, n = 4) completed a 3-week educational program that consisted of e-
