[Isolated surgery of tricuspid insufficiency in valvular reoperation].

BACKGROUND AND AIMS OF THE STUDY: Development of late tricuspid regurgitation (TR) following prior operation on left heart valves is associated with an important impairement of functional status and adverse prognosis. Indications and outcomes of surgery in this setting are not well defined. Our Department experience in isolated TR in valve reoperation is evaluated. METHODS: Twenty-four patients underwent isolated tricuspid valve surgery because of severe TR following a prior operation on left heart valves between January/1990 and July/2003. Patients with congenital heart disease, traumatic TR

[Reoperation after aortic valve replacement and root reconstruction].

Between 1984 and 1997, 127 patients in our institution underwent single aortic valve replacement (AVR) with the St. Jude Medical valve (group S) and 11 patients with a porcine pericardial valve (group T). In the same period, 45 patients underwent aortic root reconstruction, among which 39 patients underwent the Bentall procedure using Carrel patch coronary reanastomosis (group B). The other 6 patients were treated with valve-sparing aortic root reconstruction (group V), among which the reimplantation method was used in 4 patients and the remodeling method in 2. The mean and total follow-up per

[Reoperation after corrective surgery for tetralogy of Fallot].

Thirty-six patients (7.6%) underwent reoperation after total correction of tetralogy of Fallot in our institution from 1955 to 1997. Eight patients underwent a second reoperation in the same follow-up period. The indications for surgical repair were recurrent or residual lesions alone (Qp/Qs > or = 2.0, right ventricule-pulmonary artery pressure gradient > or = 50 mmHg, tricuspid regurgitation: (TR) > or = grade 3, pulmonary regurgitation (PR) > or grade 3), or in combination with other lesions. The reoperation consisted of closure of a residual VSD in 12 patients (33.3%), relief of right vent

Early localization and reoperation for persistent primary hyperparathyroidism.

BACKGROUND: Reoperation for persistent primary hyperparathyroidism is often performed after a delay of 4-6 months. Success can be expected in over 90 per cent of cases but exploration is technically difficult and there is the possibility of creating permanent hypoparathyroidism and vocal cord paralysis. This is a study of early localization and reoperation. METHODS: In a consecutive series of 273 patients who had surgery for primary hyperparathyroidism, three remained hypercalcaemic and in three the abnormal parathyroid was not found at initial exploration. They underwent early (6-48 h) singl

Reoperation Due to Complications after Gastric Restriction Operation.

BACKGROUND: In the past 4 years we performed operations on 90 patients who suffered from morbid obesity. Five different operative techniques were available, vertical banded gastroplasty (VBG), silastic ring vertical gastroplasty (SRVG), gastric banding operation, adjustable silicone gastric banding (ASGB) and biliopancreatic diversion (BPD). METHODS: Two of these operations were mainly utilized. The ASGB was done routinely. The SRVG was used particularly for patients with hiatal hernia. Only one patient, who had a deformed pylorus, underwent BPD. RESULTS: Eleven patients had to be reoperated d

[Reoperation after corrective surgery for tetralogy of Fallot].

Thirty-two of 460 patients (6.9%) who had corrective surgery for tetralogy of Fallot in our institute between January, 1955 and March, 1994 required intracardiac reoperation. The indications for reoperation included residual lesions alone or combination of other lesions. The reoperation consisted of a new patch or direct closure of a residual ventricular septal defect in 28 (73%), reconstruction of a residual pulmonary stenosis in 18 (47%) and pulmonary regurgitation in 6 (16%), repair of tricuspid regurgitation in 11 (29%), recurrent infective endocarditis in 5 (13%), repair of a left ventric

Reoperation in patients with the short bowel syndrome.

The clinical courses of 53 adult patients with the short bowel syndrome (SBS) were evaluated to determine the incidence of and indications for reoperation. Mesenteric vascular disease (23 patients) and malignancy/irradiation (18 patients) were the most frequent causes of resection. Early reoperation was necessary in nine (17%) patients, primarily for intestinal complications. Twenty (53%) of the 38 patients leaving the hospital required a later abdominal procedure during the mean follow-up of 30 months (range: 2 to 108 months). Three (33%) of nine patients with ulcer disease had gastric resect

Complications and reoperation after Nissen fundoplication in childhood.

Over a 10 year period, 429 Nissen fundoplications were performed on children with gastroesophageal reflux. Postoperative complications occurred in 69 children (16 percent), including wrap herniation or breakdown in 29; postoperative bowel obstruction in 18; stricture in 10; intraabdominal abscess and enterocutaneous fistula in 3 patients each; and wound infection, wound dehiscence, and inadvertent splenectomy in 2 patients each. The postoperative mortality rate was 0.9 percent (4 of 429 patients) and was related to sepsis in 1 patient, a metabolic disorder in 1 patient, and underlying pulmonar

Reoperation by anterior perineal approach for missed puborectalis.

Poor postoperative continence in patients operated for high imperforate anus may result from an inadequate bowel pull-through behind the puborectal muscle. Between 1979 and 1983, we used the anterior perineal approach for reoperations in five such cases (3 males, 2 females). In all of them, preoperative continence was socially unacceptable. Each time it was possible to identify a partially or totally missed puborectal muscle. Although some complications occurred postoperatively, all patients acquired a socially acceptable level of continence with a mean follow-up of 4.5 years. Preoperative and

Results of reoperation for persistent and recurrent hyperparathyroidism.

Between August 1975 and January 1981, 106 patients thought to have persistent or recurrent hyperparathyroidism underwent a total of 108 parathyroid re-explorations at the National Institutes of Health. These 106 patients had a total of 175 previous operations for hyperparathyroidism (156 cervical and 19 mediastinal). Nephrolithiasis (54% of patients) and bone disease (24% o patients) were the predominant symptoms. Arteriographic examination and selective venous sampling provided highly accurate localizing results in 33% of the patients, and were of some help in 64%. The final diagnoses after r

Foregut Surgery: Innovations and Reoperative Techniques.

This article reviews current and emerging surgical and endoscopic approaches to foregut disease, building on more than a century of innovation. For GERD, laparoscopic fundoplication remains the gold standard, complemented by magnetic sphincter augmentation, the novel RefluxStop device, and endoscopic options such as TIF, Stretta, and antireflux mucosectomy. In achalasia and motility disorders, laparoscopic and robotic Heller myotomies are equivalent, while POEM offers a noninferior endoscopic alternative limited mainly by postprocedural reflux. Management of recurrent dysphagia and myotomy "bl

Reoperations Following Cervical Disc Replacement.

Cervical disc replacement (CDR) has emerged as an alternative surgical option to cervical arthrodesis. With increasing numbers of patients and longer follow-ups, complications related to the device and/or aging spine are growing, leaving us with a new challenge in the management and surgical revision of CDR. The purpose of this study is to review the current literature regarding reoperations following CDR and to discuss about the approaches and solutions for the current and future potential complications associated with CDR. The published rates of reoperation (mean, 1.0%; range, 0%-3.1%), revi

Amniotic membrane in the management of strabismus reoperations.

PURPOSE: To reduce postoperative scar formation and to improve duction using amniotic membrane (AM) in strabismus reoperations. METHODS: A prospective study of interventional case series comprised of 14 patients with restrictive strabismus. Objective clinical findings (visual acuity, angle of deviations and degree of duction deficits) were recorded in both the pre- and post-operative periods. Strabismus surgery included the excision of adhesions and scar tissue, repositioning of extraocular muscles (according to the degree of deviations) and placement of two sheets, one between muscle and ten

Frequency of reoperations in patients with Marfan syndrome.

BACKGROUND: We undertook a retrospective study of the pattern of reoperations in surgical patients with Marfan disease. METHODS: Between 1985 and 2008, 83 Marfan patients (60 males, 23 females) underwent 155 aortic operations in our institution. Twenty-eight patients had acute dissection (22 type A, 6 type B), and two had aortic rupture. Mean age at initial operation was 32±13 years. Operations included valve-sparing or Bentall aortic root repair, and ascending aorta, arch, descending thoracic, thoracoabdominal aorta, and infrarenal aortic replacement. Reoperative treatment of obstructive sleep apnea.

Over the last several decades, various surgical treatments have been advocated and used to treat obstructive sleep apnea. Treatments ranging from aggressive procedures, such as tracheostomy, to the least-invasive procedures, such as radioablation, have not yielded satisfactory results. One of the major causes for surgical failures and lower success rates lies in the inadequate understanding and appreciation of the anatomic and pathophysiologic factors that contribute to upper airway obstruction. In some cases, combinations of various surgical techniques may help improve the conditions. This ar

The dilemma with multiply reoperated knee instabilities.

The treatment of instability of the multiply reoperated knee is a complex problem. The causes for failure are numerous and include repeated trauma, insufficient fixation and non-anatomic placement of the graft, inadequate replacement material, isolated anterior cruciate ligament (ACL) reconstruction in complex knee instability or the use of a ligament prosthesis. With every surgical procedure, however, the anatomical and technical conditions become worse. Problems like degenerative changes, joint stiffness and gait abnormalities occur and often become a more focal point than the instability it
