BACKGROUND: Taiwan, one of the fastest-aging countries in the world, started implementing version 1.0 of its long-term care (LTC) plan in 2008. In 2017, LTC Plan 2.0 began a new era with its goal to integrate Taiwan's fragmented LTC service system. LTC Plan 2.0 also aims to establish an integrated community-based LTC system incorporating both health care and disability prevention. This three-tier model consists of the following: two LTC services with a day-care center as their base and case management (Tier A), a day-care center and a single LTC service (Tier B), and LTC stations that provide

BACKGROUND: Sleep deprivation can decrease parental well-being and degrade mental and physical health in parents of children with chronic illness. The aim of this study was to explore the associations of sleep quality, psychological stress perception, and evening salivary cortisol concentration with self-esteem, optimism and happiness in parents of children with type 1 diabetes and developmental disorders compared to parents of healthy, typically developing children.
METHODS: We studied 196 parents of children with chronic conditions, including autistic spectrum disorder (N = 33), cerebral pa

The prevalence of human immunodeficiency virus (HIV) infection is increasing in Singapore. The surgical experience, however, remains limited. A retrospective review of 13 HIV-positive patients requiring abdominal surgery within Singapore was done. There were 4 females and 9 males with age ranging from 21 to 44 years. Operations included appendicectomy, colectomy, splenectomy, intestinal bypass, gastrostomy and exploratory laparotomy. Pathologic findings directly related to HIV infection were found in two-fifths (5 out of 13) of these patients. A low CD4+ count or signs of full-blown acquired i

INTRODUCTION: Caring for a child with obsessive-compulsive disorder (OCD) can be extremely difficult, yet evidence-based support strategies for parents/carers are limited. A detailed understanding of parent support needs is an important first step in intervention development and qualitative research with this focus is currently lacking. In this study, the viewpoints of parents and professionals were used to understand support needs and preferences when caring for a child with OCD. This qualitative descriptive study formed part of a wider UK-based project aimed at developing better support for

AIM: This study aimed to explore the lived experiences and support needs of parents caring for children with type 1 diabetes mellitus (T1DM) in Zabol, Iran, a rural and resource-scarce region.
METHODS: A qualitative phenomenological design was employed with 36 parents (25 mothers and 11 fathers) of children aged 4-17 years who had lived with T1DM for at least 6 months. Semistructured interviews were conducted in Persian, transcribed, translated, and thematically analyzed using Braun and Clarke's six-step framework, in accordance with Consolidated Criteria for Reporting Qualitative Research (C

Standard therapy for advanced Prostate Cancer (PCa) consists of antiandrogens, which provide respite from disease progression, but ultimately fail resulting in the incurable phase of the disease: mCRPC. Targeting PCa cells before their progression to mCRPC would greatly improve the outcome. Combination therapy targeting the DNA Damage Response (DDR) has been limited by general toxicity, and a goal of clinical trials is how to target the DDR more specifically. We now show that androgen deprivation therapy (ADT) of LNCaP cells results in increased expression of TLK1B, a key kinase upstream of NE

BACKGROUND AND OBJECTIVES: American Indian and Alaska Native (AIAN) experience disproportionately high rates of Alzheimer's disease and related dementias (ADRD), yet little research examines caregiving experiences and service needs within these communities. Guided by the Sociocultural Stress and Coping Model, this study explored dementia caregiving experiences, cultural contexts, and service needs among American Indian community members, tribal health professionals, and leaders in a Northern Plains tribal community using a community-based participatory research approach.

BACKGROUND: Spinal muscular atrophy (SMA) is a rare neurodegenerative disease characterized by progressive muscular weakness, which occurs in one in 6,000 to 10,000 live births. The burden of SMA on Canadian patients and caregivers is not known.
OBJECTIVE: To characterize the burden of SMA in Canada as reported by patients and caregivers, including disease and treatment impacts, indirect costs, and caregiver burden.
METHODS: Surveys were distributed by Cure SMA Canada and Muscular Dystrophy Canada to individuals with SMA and their caregivers. The online surveys were anonymous and completed b

BACKGROUND: Tuberous sclerosis complex (TSC) is a complex multisystem genetic disorder. Approximately 84% of people with TSC have epilepsy. However, there is little literature available regarding families' experiences with TSC and seizure management. Therefore, the aim of the current study was to explore families' positive and negative experiences, and attitudes towards TSC, epilepsy and medical management of seizures.
METHODS: Framework analysis informed an open exploration of families' experiences with TSC, epilepsy and medical management of seizures. Using structured interviews, 11 parents

BACKGROUND: The developmental outcomes and life course trajectories of young children with or at-risk for fetal alcohol spectrum disorders (FASD) can be optimized when individual and family needs are identified early and met with family-centered early intervention (EI) services. However, little is known about access to and quality of EI services with this high-needs population.
METHOD: Twenty-five biological or adoptive parents of children with or at high risk for FASD, living in the greater area of Seattle, Washington participated in this qualitative study. Three focus groups were conducted

BACKGROUND: There is little evidence of patient acceptability for drug-resistant tuberculosis (DRTB) care in the context of new treatment regimens and HIV co-infection. We aim to describe experiences of DRTB-HIV care among patients in KwaZulu-Natal province, South Africa.
METHODS: In this qualitative study using Bury's framework for chronic illness, we conducted 13 focus groups at a tertiary hospital with 55 patients co-infected with DRTB and HIV (28 women, 27 men) who were receiving new bedaquiline-based treatment for DRTB, concurrent with antiretroviral therapy. A retrospective cohort study was performed of the school-aged siblings of children with severe learning difficulties. Index siblings demonstrated restricted contact with friends, behaviour difficulties, increased anxiety and increased acceptance towards disability compared with comparison siblings. Boys older than the disabled child demonstrated increased acceptance, whereas older girls demonstrated high rates of behaviour difficulty. Younger siblings demonstrated higher levels of anxiety, and those near in age demonstrated restricted contact with friends and behaviour difficulties. Those with

Background and Objectives: Paediatric palliative care in Romania is underdeveloped and unevenly distributed, which creates major difficulties in accessing services for children with life-limiting illnesses and their families. The lack of a dedicated national strategy, the shortage of specialised staff, and socio-economic barriers exacerbate the vulnerability of these groups. This study aimed to explore parents' and caregivers' experiences, to analyse the perspectives of public institutions and NGOs involved in supporting these children, and to identify the main barriers and facilitators in acc

BACKGROUND: Raising a child with 22q11.2DS presents significant psychosocial and financial challenges for caregivers for various reasons. Research on the quality of life (QoL) of caregivers for patients with rare diseases highlights significant challenges, with caregivers reporting lower health-related QoL compared to the general population. Long-term care impacts social, health, and economic aspects of life, with the burden on caregivers inversely correlating with their QoL, affecting mobility and daily activities. Parents often manage their child's complex medical care, underscoring the need

Child's challenging behaviours can pose complex, highly stressful demands on parents. Such chronic stress may affect autonomic regulation, particularly vagal balance, and increase oxidative stress and the risk of developing chronic diseases. This study investigated the association between the child's challenging behaviour and heart rate variability, psychological and oxidative stress in parents of children with chronic conditions. We included parents of children with autism spectrum disorder, cerebral palsy, Down syndrome, diabetes type 1, and parents of healthy children. Multivariate linear r
