Aflibercept Reduces Retinal Hemorrhages and Intravitreal Microvascular Abnormalities But Not Venous Beading: Secondary Analysis of the CLARITY Study.

PURPOSE: Approximately 50% of patients receiving anti-vascular endothelial growth factor (VEGF) therapy show significant improvement in diabetic retinopathy severity score (DRSS), in particular at DRSS level 47 to 53 (moderately severe to severe nonproliferative diabetic retinopathy). Level 47 to 53 consists of 3 main features: deep hemorrhages (DH), venous beading (VB), and intraretinal microvascular abnormalities (IRMAs). It is unclear whether these features respond to anti-VEGF therapies differently. DESIGN: Post hoc analysis of Intravitreal Aflibercept versus Panretinal Photocoagulation i

Thiamine Deficiency With Mashed Potatoes: A Novel Case of Wernicke Encephalopathy Manifesting With Retinal Hemorrhages in a Pediatric Patient.

We present a case report of a 17-year-old obese female with one-week onset of progressive confusion and double vision, associated with 50-pound weight loss in five months. On fundus examination, retinal hemorrhages and abnormalities of eye movements were seen. MRI showed abnormalities that were consistent with diagnosis of Wernicke encephalopathy (WE). Thiamine replacement caused gradual improvement in patient's symptoms. Peripapillary hemorrhages on fundus examination, as seen in our patient, have been rarely seen in WE. Obesity with retinal hemorrhages, diplopia, etc. can be misleading due t

Retinal and Preretinal Hemorrhages in a Patient Receiving Hyper-CVAD Chemotherapy for T-Cell Acute Lymphoblastic Leukemia.

Hyperfractionated cyclophosphamide, vincristine, adriamycin, and dexamethasone (Hyper-CVAD) is an important chemotherapeutic regimen for acute lymphoblastic leukemia (ALL) and non-Hodgkin's lymphoma. We present a case of a 23-year-old male with T-cell ALL and visual acuity of 20/20 in the right eye and 20/25 in the left eye who developed significant changes in his vision after starting Hyper-CVAD therapy. The patient initially presented with cotton wool spots in the fundus shortly after starting the regimen. After going through the induction phase of chemotherapy, he had a sudden decline in hi

Focal inner retinal hemorrhages in patients with drusen: an early sign of occult choroidal neovascularization and chorioretinal anastomosis.

PURPOSE: To present evidence that superficial retinal hemorrhage in the macula of patients with age-related macular degeneration (ARMD) may be an early sign of occult chorioretinal anastomosis (OCRA) and type 1 occult choroidal neovascularization (OCNV). METHODS: Retrospective follow-up study of 16 patients presenting with a small focal area of superficial retinal hemorrhages and drusen in the juxtafoveolar area in 24 eyes. RESULTS: OCRA and OCNV occurred in an older subset of patients with ARMD (mean age, 75 years). Of 22 eyes with the early stages of chorioretinal anastomosis (CRA), 18 had

A case of hereditary hemorrhagic telangiectasia (Rendu-Osler-Weber's disease) with dystrophic calcinosis cutis and retinal lesions.

A 65-year-old Japanese female developed a nodule on the distal interphalangealjoint of her right thumb. She also had multiple telangiectasias on her face, oral mucosa, tongue, nasal mucosa and upper extremities. A lip biopsy showed irregularly dilated capillaries and venules lined by flat endothelial cells. The nodule in the right thumb was resected. Histopathology revealed calcium deposits in the dermis. This case was diagnosed as dystrophic calcinosis cutis within hereditary hemorrhagic telangiectasia. Ophthalmologic examination showed bilateral retinal vascular lesions with dilated and tort

[Central serous retinitis or chorioretinitis (retinopathy or chorioretinopathy) and central hemorrhagic chorioretinitis (juvenile disciform macular detachment; focal hemorrhagic chorioiditis, presumed histoplasmosis) (author's transl)].

There are no clinical or etiological connections between central serous and central hemorrhagic chorioretinitis, Serous chorioretinitis is seen mostly in men; full visual acuity is regained in approx. 80% of all cases. It mainly affects subjects aged between 36 and 45. Hemorrhagic chorioretinitis affects both sexes to about the same extent and causes severe impairment of visual acuity. Both forms should be regarded as genuine inflammations (chorioretinitis centralis serosa, chorioretinitis centralis hemorrhagica), not as "pathies". Whereas the hemorrhagica disease is generally thought to be ca

Fatal pediatric collapse with bilateral subdural hematoma and retinal hemorrhage occurring in a public park: a forensic case study.

We report the death of a one-month-old infant who developed sudden neurological collapse in a public park and was later found to have bilateral subdural hematomas, extensive retinal hemorrhages, hypoxic-ischemic encephalopathy, and cerebral venous sinus thrombosis. No external or internal traumatic injuries were identified at autopsy, and the death was certified as natural. The public park setting was important because the same intracranial and ocular findings, if arising in a private setting without witnesses, might have prompted suspicion of abusive head trauma. This case illustrates the for

From Blinding Vitreous Hemorrhage to 20/20: Multimodal Management of Presumed Tubercular Retinal Vasculitis in a Tuberculosis-Endemic Setting.

Tubercular retinal vasculitis (TRV) is an ocular manifestation of Mycobacterium tuberculosis that can lead to vision-threatening complications such as recurrent vitreous hemorrhage and cystoid macular edema (CME). Even after completing a full course of anti-tubercular therapy (ATT), persistent intraocular inflammation may drive delayed recurrences of CME, highlighting the need for vigilant, escalation-based management. We report a case of a 31-year-old Indonesian woman who presented with sudden-onset hand motion vision in her left eye. Fundus examination was obscured by dense vitreous hemorrha

New Insights into the Diagnosis and Age Determination of Retinal Hemorrhages from Abusive Head Trauma: A Systematic Review.

(1) Background: Head trauma represents the first cause of death in abused children, but diagnostic knowledge is still limited. The characteristic findings of abusive head trauma (AHT) are retinal hemorrhages (RH) and additional ocular findings, including optic nerve hemorrhages (ONH). However, etiological diagnosis must be cautious. (2) Methods: The Preferred Reporting Items for Systematic Review (PRISMA) standards were employed, and the research focus was the current gold standard in the diagnosis and timing of abusive RH. (3) Results: Sixteen articles were included for qualitative synthesis.

Choroidopathy and Retinal Detachment: A Rare Sighting in a Case of Postpartum Hemorrhage Presenting With Posterior Reversible Encephalopathy Syndrome.

Posterior reversible encephalopathy syndrome (PRES) is a rare syndrome characterized by convulsions, headache, fatigue, impaired mental status, and decreased vision. It is mainly accompanied by hypertension. Although the pathophysiology of PRES is unknown, some theories revolve around cerebral autoregulation, the ability to maintain cerebral blood flow, or the brain's ability to maintain steady cerebral blood flow over a varying range of blood pressures by cerebral vaso-constriction or dilation. The presence of subcortical vasogenic edema in the posterior brain and hyperintensity lesions in th

Bilateral subdural hematomas and retinal hemorrhages mimicking nonaccidental trauma in a patient with D-2-hydroxyglutaric aciduria.

INTRODUCTION: Nonaccidental trauma (NAT) is considered when pediatric patients present with intracranial injuries and a negative history of an accidental injury or concomitant medical diagnosis. The evaluation of NAT should include the consideration of possible medical causes including coagulation, hematologic, metabolic and other genetic disorders, as well as witnessed and unwitnessed accidental injuries. CASE PRESENTATION: We present a 7-month-old male with spells and incidental findings of bilateral subdural hematomas, retinal hemorrhages, and secondary macrocephaly, leading to investigati
