Post-resuscitation pre-hospital emergency anaesthesia for exchange of a supraglottic airway device with an endotracheal tube - a multicentre observational study.

BACKGROUND: Post-resuscitation pre-hospital emergency anaesthesia (PHEA) for changing a supraglottic airway device (SGA) to an endotracheal tube (ETT) after out-of-hospital cardiac arrest may improve airway management but carries potential risks, particularly related to haemodynamic instability. This study investigates its association with complications during post-resuscitation care. METHODS: All Emergency Medical Service missions in the districts Gütersloh, Dresden and Lippe from 2019 to 2021 were analysed for out-of-hospital cardiac arrest and supplemented with data from the German Resusci

External validation and comparative performance of ACLS, SARICA, NULL-PLEASE, BDR scores for in-hospital mortality after out-of-hospital cardiac arrest: a multicenter retrospective cohort study.

BACKGROUND: Prognostic scores are used to estimate outcomes after out-of-hospital cardiac arrest (OHCA), but their comparative performance in external cohorts remains unclear. We externally validated the ACLS, SARICA, NULL-PLEASE, and BDR scores for predicting in-hospital mortality after return of spontaneous circulation. METHODS: We conducted a retrospective multicenter cohort study using the OHCA Research Database. Adult patients with emergency medical services-attended OHCA who achieved return of spontaneous circulation were included. Patients without data required for score calculation, i

[Case report: resuscitation of a hypothermic patient - "Nothing's like it seems!"].

We report about a prolonged resuscitation of a hypothermic patient (26.2°C) in stadium HT III of the Swiss Staging System (SSS). A mechanical chest compression with the Lund University Cardiac Assist System (LUCAS) and the rewarming with a haemodialysis device were implemented. After a resuscitation time of 200min and a body temperature of 32.1°C ventricular fibrillation occurred. After the defibrillation a return of spontaneous circulation (ROSC) was established. The patient achieved a very good cognitive-mnestic result after the resuscitation but he suffered a neurological deficit in the mea

Echocardiographic detection of coronary air embolism as the cause of cardiac arrest in a neonate with transposition of the great arteries.

A hemodynamically stable neonate with transposition of the great arteries suddenly went into cardiac arrest during preparation for transport to the operating room. Emergency echocardiography during cardiac massage detected coronary air embolism as the presumed cause of arrest. After about 15 minutes of resuscitation, it was possible to establish return of spontaneous circulation. Since the most likely source of air was the neonate's peripheral venous line, we have introduced inline microfilters for all neonates and all patients with right-to-left shunt lesions and did not encounter proven air

Venoarterial Extracorporeal Membrane Oxygenation for "Protected" Percutaneous Coronary Intervention Secondary to Refractory Polymorphic Ventricular Tachycardia and Cardiac Arrest.

We present a case of cardiogenic shock secondary to refractory polymorphic ventricular tachycardia associated with coronary ischemia resulting in cardiac arrest. Following the return of spontaneous circulation, the patient was cannulated for peripheral venoarterial extracorporeal membrane oxygenation (V-A ECMO) in anticipation of high-risk "protected" percutaneous coronary intervention (PCI). Under full V-A ECMO support, inotropes and vasopressors were weaned off, and the patient underwent uneventful PCI of left circumflex and obtuse marginal lesions. After 48 hours, the patient was decannulat

Massive mass embolism detected by transesophageal echocardiography in bone cement implantation syndrome: a case report.

Bone cement implantation syndrome (BCIS) is a known complication in patients undergoing cemented orthopedic surgeries; however, the etiology and pathophysiology of BCIS are not fully understood. We report the case of a patient who developed pulseless electrical activity (PEA) due to BCIS after cemented femoral head replacement. Transesophageal echocardiography (TEE) during PEA revealed a massive embolus extending from the main pulmonary artery to the inferior vena cava. Of note, this embolus disappeared completely and rapidly after return of spontaneous circulation. TEE proved to be useful in

Prehospital Pericardiocentesis Using a Pneumothorax Needle.

Case: We discuss a patient with a penetrating knife wound to the chest who lost pulses from cardiac tamponade. Prehospital ultrasound was able to quickly identify the tamponade and a pericardiocentesis was performed using a Simplified Pneumothorax Emergency Air Release (SPEARTM) Needle (North American Rescue, LLC, USA) with subsequent return of spontaneous circulation. Discussion: Penetrating chest trauma carries significant morbidity and mortality. In traumatic cardiac arrest due to a penetrating mechanism, it is paramount that the patient be transported to a trauma center as quickly as possi

Therapeutic hypothermia after prolonged cardiopulmonary resuscitation for pulseless electrical activity.

We report an 18-year-old female patient with cardiac arrest due to pulseless electrical activity caused by a massive pulmonary embolism. Cardiopulmonary resuscitation was continued for more than one hour. Although the initial clinical signs and symptoms suggested poor outcome, immediate intravenous thrombolysis was instituted. After return of spontaneous circulation (75 minutes) the patient was still comatose and mild therapeutic hypothermia (32.5 degrees C) was instituted for brain protection during the first 24 hours. She recovered uneventfully without neurological deficit. Therapeutic hypot

Spectral Characteristics and Parameters of Pulse Oximetry Waveforms for Assessing Resuscitation Status.

BACKGROUND: Real-time assessment of physiological resuscitation status is critical for guiding personalised cardiopulmonary resuscitation (CPR) and avoiding unnecessary harm from prolonged chest compressions. Non-invasive, objectively trackable tools for this purpose remain limited. METHODS: This multicentre retrospective study (2013-2018) included 451 adult patients with cardiac arrest (497 CPR episodes) from 14 Chinese tertiary hospitals, comprising 158 Out-of-Hospital Cardiac Arrest (OHCA) episodes with prehospital SpO2/ECG monitoring continued upon ED arrival and full pulse oximetry pleth

[Successful use of an AED following anterior myocardial infarction].

A participant of the annual Hamburg marathon collapses on the finish line. Medics at the scene find a conscious patient and prepare transport to the finish area medical center. During transport the patient becomes unconscious and pulseless. The medics immediately perform basic life support and cardiopulmonary resuscitation (CPR). An automated external defibrillator (AED) is attached and after analysis of the patient}s heart rhythm the patient is defibrillated twice. The ambulance service reach the scene with a delay. The emergency physician}s ECG shows ventricular fibrillation (VF) and two mor

Unmasking the Hidden Culprit: A Coma Mimicry in a Child Bitten by Cobra.

Snake bite is a significant public health concern, particularly in tropical regions. Individuals who are bitten by neurotoxin snake commonly present with ptosis, ophthalmoparesis, muscle weakness, and diminished or absent of deep tendon reflexes. However, accurately determining the occurrence of a snakebite can sometimes be challenging, potentially leading to misdiagnosis. We present the case of a 2-year-old boy with sudden cardiac arrest. Following a brief resuscitation, he had return of spontaneous circulation. Despite normal electroencephalography results, the patient continued to have abse

Efficacy of laryngeal mask epinephrine in neonatal resuscitation; an ovine study.

BACKGROUND: The efficacy of Laryngeal Mask Airway (LMA) epinephrine during neonatal resuscitation has not been studied. We hypothesize that LMA epinephrine is as effective as endotracheal tube (ETT) epinephrine. METHODS: Sixteen fetal lambs were randomized in ETT or LMA group for ventilation and airway epinephrine administration after cord occlusion to induce complete cardiac arrest. Lambs were delivered and instrumented to continuously record pulmonary and systemic hemodynamics. After 5 min of cardiac arrest, lambs were resuscitated per NRP guidelines. Blood gases and plasma epinephrine leve

Cardiac arrest following arteriovenous fistula manipulation: a cautionary note.

Arteriovenous fistulas can lead to a number of different chronic complications. We describe a case where a patient developed a thrombosis within her brachiobasilic arteriovenous fistula, which was manually manipulated in order to restore fistula flow. This resulted in a pulseless electrical activity cardiac arrest within a few minutes. After ten minutes of chest compressions and intubation, there was return of spontaneous circulation. No epinephrine was given nor shocks administered. Patient was extubated within minutes and was alert, orientated and haemodynamically stable. CT pulmonary angiog

Thrombolytics in Cardiac Arrest from Pulmonary Embolism: A Systematic Review and Meta Analysis.

BACKGROUND: During cardiopulmonary resuscitation, intravenous thrombolytics are commonly used for patients whose underlying etiology of cardiac arrest is presumed to be related to pulmonary embolism (PE). METHODS: We performed a systematic review and meta-analysis of the existing literature that focused on the use of thrombolytics for cardiac arrest due to presumed or confirmed PE. Outcomes of interest were return of spontaneous circulation (ROSC), survival to hospital discharge, neurologically-intact survival, and bleeding complications. RESULTS: Thirteen studies with a total of 803 patient

A unified hypothesis of SUDEP: Seizure-induced respiratory depression induced by adenosine may lead to SUDEP but can be prevented by autoresuscitation and other restorative respiratory response mechanisms mediated by the action of serotonin on the periaqueductal gray.

Sudden unexpected death in epilepsy (SUDEP) is a major cause of death in people with epilepsy (PWE). Postictal apnea leading to cardiac arrest is the most common sequence of terminal events in witnessed cases of SUDEP, and postconvulsive central apnea has been proposed as a potential biomarker of SUDEP susceptibility. Research in SUDEP animal models has led to the serotonin and adenosine hypotheses of SUDEP. These neurotransmitters influence respiration, seizures, and lethality in animal models of SUDEP, and are implicated in human SUDEP cases. Adenosine released during seizures is proposed to

The No-Win Resuscitation: Ventricular Septal Rupture and Associated Acute Aortic Occlusion.

A 66-year-old patient was admitted under continuous resuscitation for pulseless electrical activity. After return of spontaneous circulation ECG showed signs of acute inferior ST-elevation myocardial infarction, and echocardiography showed acute right ventricular failure with a dilated right ventricle. Carotid pulses were present in the absence of femoral pulses. Subsequent computed tomography demonstrated inferior myocardial infarction with ventricular septal rupture and thrombotic occlusion of the thoracic aorta, resulting in a heart-brain-circulation with loss of perfusion downstream of the

Cardiopulmonary arrest secondary to compression of the heart owing to esophageal hiatal hernia: a case report.

BACKGROUND: Esophageal hiatal hernia is commonly encountered in clinical practice. We describe a case of cardiac compression caused by an esophageal hiatal hernia that resulted in circulatory failure and cardiac arrest. CASE PRESENTATION: An 82-year-old woman presented to our hospital with vomiting, which progressed to cardiac arrest in the emergency room after computed tomography (CT) imaging. CT revealed gastric herniation into the mediastinum, with marked cardiac compression. Cardiopulmonary resuscitation was performed, and a nasogastric tube was inserted for gastric decompression, which r

Cardiopulmonary resuscitation (CPR) using periodic acceleration (pGz) in an older porcine model of ventricular fibrillation.

Cardiopulmonary resuscitation (CPR) can be achieved by repetitive motion of the body headwards to footwards in the spinal axis, at 2 Hz and +/- 0.6 G in a juvenile pig model of ventricular fibrillation. Return of spontaneous circulation and normal neurological outcome occurred after a total of 22 min of ventricular fibrillation that included a 3-min noninterventional period [Resuscitation 56 (2003) 215; Resuscitation 51 (2001) 55]. Since older pigs have stiffer rib cages than juvenile pigs and their hemodynamic response to various stimuli might differ, this study was carried out to determine w
