Influence of rotator cuff tearing on glenohumeral stability.

This study hypothesizes that full-thickness tearing of the rotator cuff can lead to joint instability and that the degree of instability depends on the size and location of the tear. Twelve cadaveric shoulder specimens were divided into two groups: group 1 had a circular tear centered at the critical area, and group 2 had a circular tear centered at the rotator interval. Each group was tested at 2.5 cm and 5 cm tear sizes. Unloaded, and with the arm in 90 degrees flexion and full internal rotation, the humeral head shifted posteriorly. With loading, a large and more anteriorly located defect h

Captured shoulder: a complication of rotator cuff surgery.

Thirteen patients who developed restrictive subdeltoid adhesions after rotator cuff repair were identified. These patients underwent second-look arthroscopy and takedown of adhesions at an average of 37 weeks after their index surgery. Clinical findings include pain and restricted motion that does not yield to manipulation under anesthesia. Arthroscopic findings are subdeltoid adhesions and a chondral lesion (companion lesion) of the humeral head articular surface. Patients were reevaluated at 26 weeks after their release of adhesions. Arthroscopic evaluation and treatment of the rotator cuff.

Arthroscopy has improved our diagnostic assessment of rotator cuff disease, especially in understanding patterns of articular surface partial-thickness tears. Arthroscopic subacromial decompression is a proven and reliable alternative to open acromioplasty. Despite continued controversy, most partial-thickness rotator cuff tears may be satisfactorily treated by arthroscopic debridement and decompression. The role for debridement and decompression without repair for full-thickness rotator cuff tears seems less than initially suggested, as several recent independent studies have documented resul

Functional and anatomical results after rotator cuff repair.

The anatomic condition of the rotator cuff and the functional results obtained were studied in a homogeneous series of 100 full thickness cuff tears in 98 patients with an average followup of 4 years. Constant's functional score, used by the European Society for Shoulder and Elbow Surgery, was done preoperatively and postoperatively in each patient, in addition to ultrasonography at followup. There was a close correlation between the anatomic condition of the cuff and Constant's functional score before surgery (p = 0.0063) and after repair, irrespective of the type of tear repaired (p = 0.0012

Suprascapular nerve block for persistent rotator cuff lesions.

The suprascapular nerve supplies sensory nerves to the posterosuperior aspect of the shoulder, including major portions of the rotator cuff. Suprascapular nerve block using steroid/bupivacaine is temporarily effective in reducing pain in rotator cuff tendinitis and tears, improving movement range in tendinitis and is possible in an outpatient setting with little or no complication risk.

Thermography of frozen shoulder and rotator cuff tendinitis.

The role of thermography in the diagnosis of soft tissue lesions of the shoulder was evaluated by screening 28 patients with unilateral frozen shoulder and 86 patients with unilateral rotator cuff lesions. Index shoulders were then compared with the normal side. Differences in skin temperature distribution were found in 82% of subjects with frozen shoulder, nearly three-quarters of whom had reduced skin temperature. There was no consistent pattern of shoulder skin temperature found in rotator cuff tendinitis patients (49% normal, 28% reduced, 23% increased). Thermography can be helpful in the

Arthroscopic acromioplasty for lesions of the rotator cuff.

Arthroscopic acromioplasty was done for a lesion of the rotator cuff in 165 patients: 100 who had stage-II impingement syndrome (no actual tear of the rotator cuff) (group 1), forty who had a partial tear (group 2), and twenty-five who had a full-thickness tear (group 3). The operation consisted of acromioplasty, resection of the coracoacromial ligament and subacromial bursa, and removal of osteophytes, when present, near the inferior aspect of the acromioclavicular joint. In the patients who had a partial or complete tear, minimum debridement of the rotator cuff also was performed. In group 1

[Rotator cuff lesions: evaluation with RM and US].

Physical and instrumental examination of the patients with an "aching shoulder" is often difficult and a positive out-come of surgical treatment may be impaired by the late diagnosis of long-standing rotator cuff lesions causing tendon adhesions and muscle atrophy. The authors report the results obtained in 25 selected patients with an aching shoulder examined with arthrography, real-time ultrasound (US) with a 5 MHz linear probe, and high-field (1.5 T) Magnetic Resonance (MR) imaging. Arthrography was able to demonstrate a cuff tear in 14/25 cases; US detected signs of tear in 14/25 (1 false

Compensatory activation of the deltoid and remaining rotator cuff muscles in patients with rotator cuff tears using real-time tissue elastography.

INTRODUCTION: Rotator cuff (RC) tears are a prevalent condition leading to dysfunction and shoulder pain. Spatial changes in the behavior of the deltoid and remaining RC muscles during arm elevation in patients with RC tears have not been investigated. The purpose of this study was twofold: (1) to investigate factors related to the changes in the deltoid muscle behavior during scapular plane abduction (scaption) in patients with RC tears, and (2) to examine the functional relationship between the deltoid and remaining RC muscles, particularly remaining infraspinatus (ISP) and teres minor (TMin

Rotator Cuff Tendon Healing Using Human Dermal Fibroblasts: Histological and Biomechanical Analyses in a Rabbit Model of Chronic Rotator Cuff Tears.

BACKGROUND: Tenocytes derived from tendons have been reported to be effective in the treatment of rotator cuff tears through the expression of extracellular matrix proteins. Human dermal fibroblasts, known to express collagen types I and III as tenocytes do, may likely be substitutes for tenocytes to enhance healing rotator cuff tears. PURPOSE: To demonstrate the capability of human dermal fibroblasts to enhance healing of rotator cuff tears. STUDY DESIGN: Controlled laboratory study. METHODS: The cellular properties and expression profiles of growth factors were compared between human derm

Effect of tamoxifen on fatty degeneration and atrophy of rotator cuff muscles in chronic rotator cuff tear: An animal model study.

Fatty degeneration of the rotator cuff muscles is an irreversible change resulting from chronic rotator cuff tear and is associated with poor clinical outcomes following rotator cuff repair. We evaluated the effect of Tamoxifen, a competitive estrogen receptor inhibitor, on fatty degeneration using a mouse model for chronic rotator cuff tear. Sixteen adult mice were divided into two diet groups (Tamoxifen vs. Regular) and subjected to surgical creation of a large rotator cuff tear and suprascapular nerve transection in their left shoulder with the right shoulder serving as a control. The rotat

Reproducibility: Reliability and agreement of short version of Western Ontario Rotator Cuff Index (Short-WORC) in patients with rotator cuff disorders.

INTRODUCTION: Recently, a shorter version of Western Ontario Rotator Cuff Index (Short-WORC) was proposed as a subset of 7 items from the original 21-item WORC. However, the reproducibility of the Short-WORC has not been established. PURPOSE OF THE STUDY: To determine reproducibility (reliability and agreement) of the Short-WORC among patients with rotator cuff disorders (RCDs). METHODS: Patients (n = 153) diagnosed with RCD completed the WORC at baseline and at 3 months post-operatively (n = 146). The Short-WORC was extracted from the full version of WORC. From this retrospective cohort, 43

Operative vs Nonoperative Treatment for Atraumatic Rotator Cuff Tears: A Trial Protocol for the Arthroscopic Rotator Cuff Pragmatic Randomized Clinical Trial.

IMPORTANCE: Rotator cuff disorders remain the most common cause of shoulder pain and are among the most common reasons for patients to seek care in primary and specialty settings. Although operative and nonoperative treatments are offered to patients with atraumatic rotator cuff tears, there is a lack of evidence to support operative vs nonoperative treatment. This paucity of evidence has been highlighted by several professional agencies and experts. OBJECTIVE: To perform a pragmatic randomized clinical trial, the Arthroscopic Rotator Cuff trial, comparing pain and functional outcomes in pati

Using support vector machines to optimally classify rotator cuff strength data and quantify post-operative strength in rotator cuff tear patients.

Shoulder strength data are important for post-operative assessment of shoulder function and have been used in diagnosis of rotator cuff pathology. Support vector machines (SVM) employ complex analysis techniques to solve classification and regression problems. A SVM, a machine learning technique, can be used for analysis and classification of shoulder strength data. The goals of this study were to determine the diagnostic competency of SVM based on shoulder strength data and to apply SVM analysis in efforts to derive a single representative shoulder strength score. Data were taken from fourtee

[Painful shoulder… not only the rotator's cuff].

A painful shoulder is a frequent complaint in general practice. In this clinical vignette, we discuss the case of an elderly patient with pain and lack of strength in the right shoulder one month after a hospital stay. The diagnosis is an injury of the accessory nerve after the placement of a central venous catheter in an emergency setting. The different causes of non-orthopaedic causes of shoulder pain are reviewed with emphasis on neurological etiologies.
