[Posterior scleritis: apropos of 6 cases].

In a retrospective study of 6 patients (9 eyes) with posterior scleritis, we analyse the symptoms, the clinical signs and the ancillary tests of the disease. A decreased vision and an ocular or periocular pain are found in 8 cases of 9. Anterior involvement like redness is found in all the cases. In 7 eyes, we observe a diffuse or localized exudative retinal detachment. B-scan ultrasonography is the most helpful ancillary test, showing in all the examined eyes a scleral thickening. In 2 patients, we find a systemic disease associated with the posterior scleritis.

Necrotising anterior scleritis after pterygium surgery.

Necrotising anterior scleritis is a rare sequela of ocular surgery. Typically an intense scleral inflammation develops at or adjacent to the site of the previous surgery. Necrosis supervenes and may then progress circumferentially to involve much of the anterior sclera. Surgically induced necrotising scleritis (SINS) has been previously described after cataract extraction, trabeculectomy, retinal detachment and strabismus surgery. We report three cases of SINS following pterygium excision. In each case the disease was prolonged, progressive and required protracted immunosuppressive therapy.

Infectious scleritis: report of four cases.

While systemic autoimmune diseases are the main possibilities in the differential diagnosis of scleritis, other less common etiologies such as infections must also be considered. The authors report four cases of infectious scleritis to review predisposing factors, clinical characteristics, methods of diagnostic approach, and response to therapy. Two patients had primary scleritis and two patients had secondary scleritis following extension of primary corneal infection (corneoscleritis). Diagnoses included three local infections (one each with Staphylococcus. Acanthamoeba, and herpes simplex) a

Acute scleritis in porphyria cutanea tarda.

We examined three patients who had dermatologic and biochemical manifestations of porphyria cutanea tarda and localized thinning or excavation in the sun-exposed interpalpebral sclera, adjacent to the cornea. All three patients had signs of acute scleritis. The acute scleritis responded to oral indomethacin in one patient, but systemic corticosteroids were required to control the inflammation in the other two patients. Phlebotomy, protection from sunlight, and refraining from alcohol played an important part in the treatment of the patients.

Posterior scleritis with secondary ciliochoroidal effusion.

Posterior scleritis is a granulomatous inflammation occurring either within the scleral tissue or in the vascular networks adjacent to it. This inflammatory process can cause leakage of fluid from the choroid into the suprachoroidal space leading to ciliochoroidal effusion and an exudative retinal detachment. The following is a case report of a 64 year old white male, who presented with an acute angle closure attack and myopic shift in one eye, and later developed an exudative choroidal and retinal detachment. Posterior scleritis was diagnosed based on ultrasonography and the orbital CT scan f

Scleritis and perceptive deafness: case report.

There is some evidence in the past decade that a similar syndrome to Cogan's syndrome may exist in which a scleritis substitutes for the nonsyphilitic interstitial keratitis. A case is reported of a female patient, aged 39, with nonrheumatoid scleritis and associated bilateral perceptive deafness.

Incidence of scleritis requiring surgical repair from subconjunctival triamcinolone acetonide injections in patients with scleritis in TriNetX.

PURPOSE: To estimate the yearly and 15-year cumulative incidence of scleritis requiring surgical repair in patients with a prior diagnosis of scleritis who received a subconjunctival or subTenon triamcinolone acetonide injection (STAI). DESIGN: A retrospective cohort study was conducted using the TriNetX US Collaborative Network. TriNetX is an electronic health records database with anonymized, deidentified encrypted data from 69 healthcare networks. METHODS: Subjects with a history of scleritis were identified using ICD-10 (International Classification of Diseases, 10th Revision) code H15.0

Immunopathological Analysis of a Mouse Model of Arthritis-Associated Scleritis and Implications for Molecular Targeted Therapy for Severe Scleritis.

Scleritis involves inflammation of the sclera, which constitutes 75% of the wall of the eye. This pathology is often seen as an ocular lesion associated with systemic inflammatory diseases. Severe types of scleritis such as posterior scleritis require urgent immunosuppressive treatments, including molecularly targeted therapies to avoid permanent visual impairment. Which molecules should be selected as targets has remained unclear. To clarify the pathogenesis of scleritis and propose appropriate target molecules for therapy, we have established novel animal model of scleritis by modifying the

Update on Therapy for Non-Infectious Scleritis.

Non-infectious scleritis is a potentially vision threatening inflammatory disease which often requires long-term treatment. The general approach to management consists of a thorough systemic workup and subsequent selection of the most appropriate therapeutic agent(s) for inflammatory control. This article reviews current and emerging therapies for scleritis treatment, including topical agents, systemic non-steroidal anti-inflammatory drugs and corticosteroids, conventional immunomodulatory therapies, and biologics.

The enigma of sclera-specific autoimmunity in scleritis.

Scleritis is a severe and painful ophthalmic disorder, in which a pathogenic role for collagen-directed autoimmunity was repeatedly suggested. We evaluated the presence of sclera-specific antibodies in a large cohort of patients with non-infectious scleritis. Therefore, we prospectively collected serum samples from 121 patients with non-infectious scleritis in a multicenter cohort study in the Netherlands. In addition, healthy (n = 39) and uveitis controls (n = 48) were included. Serum samples were tested for anti-native human type II collagen antibodies using a validated enzyme-linked immunos

Uveitis and Scleritis as a Risk Factor for Mortality.

PURPOSE: Uveitis and scleritis may be caused by local or systemic infection, or associated with noninfectious systemic inflammatory autoimmune disease. This study explored the all-cause mortality following an individual's first presentation with uveitis/scleritis. METHODS: A cross-sectional study was conducted on all uveitis/scleritis patients diagnosed by uveitis specialists and treated in a single tertiary referral center in New Zealand between 2006 and 2020(15y). Masquerade syndromes including intraocular lymphoma were excluded. Outcome measures: demographics, etiology of uveitis/scleritis

Tofacitinib in Refractory Scleritis: A Case Series.

Tofacitinib, a Janus kinase inhibitor, has been recently investigated as a potential therapy for refractory scleritis. Despite treatment with systemic immunosuppressive agents, scleritis is refractory to conventional therapy in a significant number of patients. Hereby, we report the use of tofacitinib as a steroid-sparing immunomodulatory agent in three patients with refractory scleritis who were either recalcitrant or intolerant to conventional therapy.

[Diagnostic and Therapeutic Management of Episcleritis and Scleritis].

While episcleritis is a benign disease only affecting the episclera, scleritis is an ocular inflammation with typically severe pain and not rarely affecting adjacent tissue.Scleritis is classified into anterior and posterior forms. Anterior scleritis is further subdivided into diffuse, nodular, necrotizing with inflammation, and necrotizing scleritis without inflammation (scleromalacia perforans). A systemic disease such as rheumatoid arthritis or granulomatosis with polyangiitis is associated with up to 50% of all patients with scleritis or episcleritis, consequently a systemic work-up with b
