This case report presents a 21-year-old male construction worker with ipsilateral inferior shoulder and posterior elbow dislocations, accompanied with other injuries. Such a combination of injury undoubtedly occurs; however, to the best of our knowledge, this is the first reported case. The prompt closed reductions without waiting for anesthesia were accomplished without the help of an assistant using a novel technique. The patient returned to his job with considerable recovery of motion and strength despite his permanent paralysis of the deltoid muscle and he was still employed as a worker af

This paper presents the background, methodology and results of a quality improvement project undertaken at a district general hospital. The project was launched in response to the concerning results from audit data which showed significant delays in the treatment of patients with shoulder dislocations and a high percentage of patients receiving procedural sedation. Using 'Plan-Do-Study-Act' cycles involving training sessions, written protocols and an online video, we were able to train a large cohort of nurse practitioners in the use of the Shoulder Reduction Bench. This is a relatively novel,

Total shoulder arthroplasty is indicated in the treatment of locked posterior fracture-dislocations of the shoulder where there is extensive damage to the articular surfaces of the humerus and glenoid. This report reviews the surgical technique, postoperative rehabilitation program, and functional results of seven shoulders in five patients who had locked posterior dislocations of the shoulder. Activities involving overhead positioning of the arm and heavy lifting continued to be challenging for this group of patients. The American Shoulder and Elbow Surgeons' Shoulder Score improved from 20.1

Two right-handed archers presented with posterior instability of the shoulder. A 19-year-old Japanese and a 26-year-old white male archer developed pain and instability of the shoulder of 6 months' duration. Both had engaged in archery for several years. Both exhibited a positive apprehension test and recurrent posterior subluxation and dislocation by flexing the arm to 80 degrees with internal rotation. Both could reduce the instability with a snap by extending the arm. For the subluxation, Neer's inferior capsular shift procedure via a posterior approach was performed. For the dislocation, a

Dislocation of any joint is an orthopaedic emergency and needs immediate attention by the attending physician. A delay in reducing a dislocated joint can lead to disastrous complications both immediately as well as in the long run. Although anterior dislocation of a shoulder joint is by far the commonest dislocation encountered by any emergency care physician, other joints may also get dislocated. In certain cases two joints may get dislocated simultaneously. Such dislocation is known as a double dislocation. Double dislocation of the proximal interphalangeal joint and the distal interphalange

UNLABELLED: Shoulder dislocation is relatively uncommon in the younger pediatric population. Because of the relative strength of the surrounding soft tissue structures of the shoulder compared with the proximal humeral physis, subluxation or dislocation resulting from a traumatic event or application of force is rare and instead a proximal humeral physeal injury occurs. Case presentation - We present a 5-year-old male who presented to the office with post-traumatic left shoulder pain for about 1 week. Radiographs of the left shoulder indicated inferior subluxation of the humeral head. He was u

BACKGROUND: Long-standing anterior glenohumeral dislocation results in both humeral and glenoid bone loss, as well as concomitant soft tissue pathologies. Reverse shoulder arthroplasty (RSA) is an established procedure to restore both stability and function in cuff-deficient shoulders. However, fixation of the glenoid component is prone to failure in cases of advanced glenoid vault destruction and requires substantial bone graft. The purpose of this study was to evaluate the outcome of glenoid bone grafting in RSA for neglected anterior dislocation with significant glenoid bone loss.

INTRODUCTION: Management of a first-time anterior shoulder dislocation (FTASD) involves important clinical and policy decisions. Predictive disease modeling can improve the quality of information disseminated in treatment discussions. In this paper, we describe a general-purpose, publicly available model and illustrate its potential as a tool for management of a FTASD.
METHODS: A Markov decision model of the natural history of a FTASD was constructed. Outcome probabilities and effectiveness were derived from the literature or estimated by expert opinion where necessary. Outcomes were the West

INTRODUCTION: Osteonecrosis of the humeral head is not a very common entity. It is usually associated with comminuted proximal humerus fractures. We report a rare case of osteonecrosis of humeral head after anterior shoulder dislocation without any fracture of the proximal humerus.
CASE REPORT: We report a case of a 24-year-old male who sustained a left-sided anterior subcoracoid shoulder dislocation following a road traffic accident. The shoulder was closed reduced, and a post-reduction radiograph was taken to confirm concentric reduction. At 6-month and 2-year follow-up, the radiographs sho

Bilateral anterior dislocation of the shoulder is a rare condition. Most of the cases are seen as posterior dislocations which generally occur during diffuse tonic-clonic contractions of epileptic seizures or after being exposed to strong electric shock. In this article, we report a 21-year-old epileptic patient who had severe bilateral shoulder pain and joint restriction. Diagnoses of bilateral, locked, anterior shoulder dislocation and Hill-Sachs lesions were made with shoulder X-rays and computed tomography. Arthroscopic Bankart repair and remplissage technique were applied in surgical trea

PURPOSE: Posterior shoulder dislocations (PSDs) comprise a small subset of shoulder dislocations, and there are few evidence-based treatment protocols and no actual algorithm for the treatment of PSDs available in the literature. This article provides a systematic review of the literature, as well as an overview of clinical and radiologic diagnostic techniques, and presents an algorithm for treatment of PSDs, including minimally invasive treatment options.
METHODS: For a systematic review of current literature, a systematic search was performed in the Medline and Cochrane databases. Journal a

Axillary artery transection after recurrent anterior shoulder dislocation is extremely rare. We present 2 such patients. The first, a 62-year-old man, presented with acute ischemia and a large hematoma in the axilla and chest wall. The second, a 63-year-old man, had a pseudoaneurysm and palpable peripheral pulses. Both underwent urgent computed tomography, which confirmed the clinical diagnosis, and the patients were taken to the operating room. In the first patient, intraoperative angiogram through both the brachial and the femoral route showed complete disruption of the axillary artery rende

Inferior dislocation of the shoulder (luxatio erecta) is a rare type of shoulder dislocation. In this short article we review a case of a 56-year-old man who sustained a subcoracoid inferior dislocation of the right shoulder after accidental axial loading of the right arm. Post-reduction magnetic resonance scan showed supraspinatus tendon rupture. Such type of shoulder dislocation is associated with significant complications (such as rotator cuff tears) and also late complications (including instability, chronic pain and reduced motion). A brief description of presentation, diagnosis and manag

BACKGROUND: Dislocation injuries of the glenohumeral joint are common in the general public and generally are corrected without complication. One serious complication with shoulder dislocations, or the subsequent reduction, is a lesion to the axillary artery. This specific complication is most frequently seen in the elderly population, where vascular structures have become less flexible. Also, these injuries are most common in association with anterior dislocations of the shoulder.
OBJECTIVES: To bring awareness to the possibility of axillary artery injury with inferior dislocation of the sho

INTRODUCTION: The standard method of treating acute primary dislocation of the glenohumeral joint is immobilization of the arm in adduction and internal rotation with a sling. The recurrence rate for anterior instability after nonoperative treatment in young active patients is extremely high (up to 90%) and well reported. A new method of immobilization with the arm in external rotation improves the position of the displaced labrum on the glenoid rim. With the use of control MRI before and after immobilization in external rotation, a study on this new repositioning of the labrum is evaluated.
