Linkage of speech sound disorder to reading disability loci.

BACKGROUND: Speech sound disorder (SSD) is a common childhood disorder characterized by developmentally inappropriate errors in speech production that greatly reduce intelligibility. SSD has been found to be associated with later reading disability (RD), and there is also evidence for both a cognitive and etiological overlap between the two disorders. The present study tested whether SSD is linked to replicated risk loci for RD. METHOD: One hundred and eleven probands with SSD and their 76 siblings were tested with measures of speech, phonological memory (Nonword Repetition-NWR), and phonolog

Speech motor control disorder after HIV infection.

We examined the clinical characteristics of six right-handed patients who developed speech motor control disorders after human immunodeficiency virus (HIV) infection. They exhibited an ataxic dysarthria, characterized by irregular articulatory breakdowns in consonant and vowel timing; were slow in timed decision-making tasks; and had impaired procedural learning. Other aspects of the neurologic examination revealed signs of diffuse CNS involvement including action-intention tremors, ataxic gait, and release signs. None developed HIV-associated dementia during 1 year of follow-up. Motor speech

[Buccolinguofacial apraxia--a probably psychogenic speech and deglutition disorder].

Difficulties in swallowing and speaking may in rare cases be due to buccolinguofacial apraxia, which usually results from lesions of the frontal operculum--mostly on the left side--or of connections to and from it. Two cases with this disorder, probably due to a multiple sclerosis, are presented and the differential diagnosis is discussed.

Cortical tracking of natural speech by children with developmental language disorder (DLD): An EEG speech decoding investigation.

The sensory/neural Temporal Sampling (TS) theory of developmental language disorder (DLD) is based on the sensory and linguistic impairments in rhythm processing that are found in children with both developmental dyslexia (DD) and DLD. These sensory/linguistic impairments include decreased sensitivity to amplitude rise times (ARTs, the sensory triggers related to automatic cortical speech tracking), syllable stress patterns and speech rhythm. At the neural level TS theory predicts impairments in the cortical tracking of different rates of amplitude modulation (AM) in the speech signal <10 Hz.

Validity of childhood disintegrative disorder apart from autistic disorder with speech loss.

In order to test clinical validity of DSM-IV childhood disintegrative disorder (CDD), 10 CDD children (mean age = 8.2 years, SD = 3.8; 7 male and 3 female) and 30 age- and gender-matched children with DSM-IV autistic disorder (AD) with speech loss (SL) (ADSL) were compared on 24 variables not directly related to CDD criteria. Compared with the ADSL children, the CDD children showed fearfulness significantly more frequently during the period of SL; displayed epilepsy significantly more frequently and stereotypy significantly more prominently at first visit on average about 6 years after SL; and

Hierarchical Temporal Structuring of Speech: A Multiscale, Multimodal Framework to Inform the Assessment and Management of Neuromotor Speech Disorder.

PURPOSE: Hierarchical temporal structuring of speech is the key to multiscale linguistic information transfer toward effective communication. This study investigated and linked the hierarchical temporal cues of the kinematic and acoustic modalities of natural, unscripted speech in neurologically healthy and impaired speakers. METHOD: Thirteen individuals with amyotrophic lateral sclerosis (ALS) and 10 age-matched healthy controls performed a story-telling task. The hierarchical temporal structure of the speech stimulus was measured by (a) 26 articulatory-kinematic features characterizing the

When does speech sound disorder matter for literacy? The role of disordered speech errors, co-occurring language impairment and family risk of dyslexia.

BACKGROUND: This study considers the role of early speech difficulties in literacy development, in the context of additional risk factors. METHOD: Children were identified with speech sound disorder (SSD) at the age of 3½ years, on the basis of performance on the Diagnostic Evaluation of Articulation and Phonology. Their literacy skills were assessed at the start of formal reading instruction (age 5½), using measures of phoneme awareness, word-level reading and spelling; and 3 years later (age 8), using measures of word-level reading, spelling and reading comprehension. RESULTS: The presence

A mechanistically informed framework of hierarchical temporal speech organization for objective differential assessment of motor speech disorders in neurodegenerative diseases.

INTRODUCTION: Speech is organized into discrete utterances and pauses in accordance with underlying linguistic structure, temporal coordination, prosodic organization, and respiratory demands. Temporal speech characteristics, including rhythmic and pausing patterns, are governed by multiple neurophysiological mechanisms and provide valuable insights for the assessment and management of motor speech disorders. This study employed a mechanistically informed analytic framework of hierarchical temporal speech organization to derive interpretable, objective measures for assessing rhythmic and pausi

An evaluation of speech production in two boys with neurodevelopmental disorders who received communication intervention with a speech-generating device.

BACKGROUND: Children with neurodevelopmental disorders often present with little or no speech. Augmentative and alternative communication (AAC) aims to promote functional communication using non-speech modes, but it might also influence natural speech production. METHOD: To investigate this possibility, we provided AAC intervention to two boys with neurodevelopmental disorders and severe communication impairment. Intervention focused on teaching the boys to use a tablet computer-based speech-generating device (SGD) to request preferred stimuli. During SGD intervention, both boys began to utte

Childhood apraxia of speech and multiple phonological disorders in Cairo-Egyptian Arabic speaking children: language, speech, and oro-motor differences.

Childhood apraxia of speech is a neurological childhood speech-sound disorder in which the precision and consistency of movements underlying speech are impaired in the absence of neuromuscular deficits. Children with childhood apraxia of speech and those with multiple phonological disorder share some common phonological errors that can be misleading in diagnosis. This study posed a question about a possible significant difference in language, speech and non-speech oral performances between children with childhood apraxia of speech, multiple phonological disorder and normal children that can be

The articulatory basis of phonological error patterns in childhood speech sound disorders.

Speech acquisition involves complex coordination of articulatory structures, primarily the jaw, lips, and tongue. Typically developing children acquire speech sounds in a hierarchical sequence governed by progressive neuromotor maturation. However, disruptions in speech motor control can lead to systematic phonological error patterns, commonly attributed to cognitive-linguistic deficits. This study explores the articulatory foundations of phonological error patterns in preschool-aged children diagnosed with moderate-to-severe speech sound disorders. Using data from 48 children who participated

Reevaluating the classification of pediatric speech sound disorders: a ground truthing perspective.

Pediatric Speech Sound Disorders (SSDs) are conventionally diagnosed using auditory-perceptual assessments, heavily relying on International Phonetic Alphabet (IPA) transcriptions. This approach, while prevalent, is increasingly criticized due to inherent perceptual biases, limited sensitivity to subtle speech motor variations, and insufficient reflection of underlying speech mechanisms. This paper critically re-examines a widely used diagnostic classification system for pediatric SSDs, namely Dodd's Model of Differential Diagnosis (MDD), emphasizing the limitations of perceptual methods and a

Treatment approaches to Motor Speech Disorders: A step towards Evidence Based Practice.

Motor Speech Disorders is an umbrella term for a set of separate dysfunctions of speech outcome associated with neurological disorders. Motor speech disorders (MSD) are classified as Speech Motor delay (SMD), Childhood dysarthria (CD), Childhood Apraxia of Speech (CAS), and Concurrent CD and CAS. The incidence and prevalence of MSD in population is uncertain. A research gap exists, making evidence-based practice questionable as regards intervention for MSD and is an area of research. Hence, current narrative review was conducted to review and highlight treatment of MSD since evidence-based tre

A cross-linguistic approach to treating speech sound disorders in bilingual children.

The present study explores generalisation of production skills across languages when treating speech sound disorders in bilingual children. Early work suggests that treating shared sounds across languages may facilitate cross-linguistic generalisation. Thus, selecting shared sounds across languages as targets may have clinical advantages. In this study, we asked if cross-linguistic generalisation can be facilitated for targets using shared sounds in bilingual children with phonological delays from Spanish (L1) into English (L2) when treating only the L1. Two Spanish-English bilingual children

Application of the Challenge Point Framework During Treatment of Speech Sound Disorders.

Purpose The purpose of this article is to provide trial-by-trial practice performance data in relation to learning (outcome probe data) as collected from 18 treatment sessions provided to children with severe speech sound disorders. The data illustrate the practice-learning paradox: Specific, perfect practice performance is not required for speech production learning. Method We detailed how nine student speech-language pathologists (SSLPs) implemented and modified the motor learning practice conditions to reach a proposed challenge point during speech practice. Eleven participants diagnosed wi

Systematic review of ultrasound visual biofeedback in intervention for speech sound disorders.

BACKGROUND: As cost and access barriers to ultrasound technology have decreased, interest in using ultrasound visual biofeedback (U-VBF) as a tool for remediating speech sound disorders (SSD) has increased. A growing body of research has investigated U-VBF in intervention for developmental SSD; however, diversity in study design, participant characteristics, clinical methods and outcomes complicate the interpretation of this literature. Thus, there is a need for a synthesis and review of the evidence base for using U-VBF in intervention for SSD. AIMS: To synthesise and evaluate the research e
