Spinal Epidural Hematoma Managed Conservatively After a Spinal Cord Stimulation Trial: A Case Report.

BACKGROUND: Spinal cord stimulation (SCS) is used to treat chronic pain but carries a rare risk of spinal epidural hematoma (SEH), which can cause serious neurologic injury. Urgent surgical decompression is often considered when neurologic deficits develop. CASE REPORT: A 73-year-old man developed severe back pain during an SCS trial and was found to have a thoracolumbar SEH after trial lead removal. He remained neurologically stable during hospitalization and was managed with close observation. His pain improved, and follow-up imaging showed a reduction in the hematoma without surgery. CONC

Epidural spinal cord stimulation plus quipazine administration enable stepping in complete spinal adult rats.

We hypothesized that epidural spinal cord stimulation (ES) and quipazine (a serotonergic agonist) modulates the excitability of flexor and extensor related intraspinal neural networks in qualitatively unique, but complementary, ways to facilitate locomotion in spinal cord-injured rats. To test this hypothesis, we stimulated (40 Hz) the S(1) spinal segment before and after quipazine administration (0.3 mg/kg, ip) in bipedally step-trained and nontrained, adult, complete spinal (mid-thoracic) rats. The stepping pattern of these rats was compared with control rats. At the stimulation levels used,

Phrenic nerve stimulation for diaphragm pacing with a spinal cord stimulator: technical note.

BACKGROUND: Diaphragm pacing with electrical stimulation of the phrenic nerve is an established treatment for central hypoventilation syndrome. The device, however, is not readily available, at least in Japan. We used the spinal cord stimulator for pain control for phrenic nerve stimulation. The purpose of this study is to evaluate the efficacy and feasibility of phrenic pacing with the compromise method. METHODS: We implanted a stimulator for spinal cord stimulation (Itrel 3 or X-trel, Medtronic, MN) in 4 patients with chronic hypoventilation because of brainstem dysfunction of various origi

Combining cortical and spinal stimulation maximizes the improvement of gait after spinal cord injury.

Most spinal cord injuries (SCI) spare descending and sublesional circuits that can be targeted through motor cortex and spinal cord stimulation to mitigate locomotor deficits. However, the potential synergy between cortical and spinal stimulation remains unknown. Here, we first investigated the phase-locked electrical stimulation of the motor cortex and lumbar spinal cord at 40 Hz in a rat model of unilateral SCI. Combining cortical and lumbar stimulation around the anticipated lift synergistically enhanced leg movements. When integrated into rehabilitation training, cortical stimulation prove

Spinal cord stimulation for the treatment of chronic pain in patients with lumbar spinal stenosis.

OBJECTIVE: Chronic back and leg pain associated with lumbar spinal stenosis (LSS) is common in the elderly. Surgical decompression is usually performed when conservative treatments fail. We present an evaluation of the long-term outcome of patients suffering from symptomatic LSS treated with spinal cord stimulation (SCS). MATERIALS AND METHODS: Data were collected prospectively in three independent registries in three European centers. Pooled data were analyzed retrospectively. Changes in pain intensity, functional status, and analgesic medication were compared at baseline and at the last ava

Early repetitive transcranial magnetic stimulation in the spinal cord region for the treatment of spinal cord injury: A case report.

RATIONALE: Currently, the global incidence of spinal cord injury (SCI) ranges from approximately 10.4 to 83 cases per million individuals, with an estimated 500,000 new cases diagnosed annually. Current clinical treatments for SCI primarily include early surgical intervention, pharmacological therapy, and personalized rehabilitation programs. Transcranial magnetic stimulation (TMS) is a noninvasive, painless, and relatively safe treatment option that avoids the side effects and dependency associated with medications. In recent years, TMS has emerged as a promising approach in SCI rehabilitatio

Glia to neuron ratio in the posterior aspect of the human spinal cord at thoracic segments relevant to spinal cord stimulation.

Spinal cord stimulation (SCS) applied between T8 and T11 segments has been shown to be effective for the treatment of chronic pain of the lower back and limbs. However, the mechanism of the analgesic effect at these medullary levels remains unclear. Numerous studies relate glial cells with development and maintenance of chronic neuropathic pain. Glial cells are electrically excitable, which makes them a potential therapeutic target using SCS. The aim of this study is to report glia to neuron ratio in thoracic segments relevant to SCS, as well as to characterize the glia cell population at thes

The Effect of Non-invasive Spinal Cord Stimulation on Anorectal Function in Individuals With Spinal Cord Injury: A Case Series.

Spinal cord injury (SCI) is a devastating condition that impacts multiple organ systems. Neurogenic bowel dysfunction (NBD) frequently occurs after a SCI leading to reduced sensation of bowel fullness and bowel movement often leading to constipation or fecal incontinence. Spinal Neuromodulation has been proven to be a successful modality to improve sensorimotor and autonomic function in patients with spinal cord injuries. The pilot data presented here represents the first demonstration of using spinal neuromodulation to activate the anorectal regions of patients with spinal cord injuries and t

[Subcutaneous stimulation as additional therapy to spinal cord stimulation in a post-laminectomy syndrome patient].

Spinal cord stimulation as treatment of chronic low back pain via neuromodulation has been frequently performed in recent years. The dorsal column is stimulated by an electrode placed at the epidural region. In the case presently described, subcutaneous lead was implanted in a patient with failed back syndrome after spinal cord stimulation was inadequate to treat back and gluteal pain. A 65-year-old male had undergone surgery to treat lumbar disc herniation, after which he received physical therapy and multiple steroid injections due to unrelieved pain. He was admitted to the pain clinic with

Spinal Cord Stimulation in Adult Tethered Cord Syndrome: Case Report and Review of the Literature.

BACKGROUND: Spinal cord stimulation (SCS) is an evidence-based treatment for chronic neuropathic pain; however, there is a dearth of evidence investigating this modality in patients with tethered cord syndrome. CASE DESCRIPTION: We present a case of 55-year-old woman with history of lipo-myelomeningocele repair and multiple detethering surgeries who presented with chronic low back and leg pain accompanied by progressive gait dysfunction. After a successful trial, she underwent SCS paddle lead placement that resulted in decrease of her visual analog scale for pain from 9/10 to 0-2/10 as well a

Treatment Efficacy and Technical Advantages of Temporary Spinal Nerve Root Stimulation Compared to Traditional Spinal Cord Stimulation for Postherpetic Neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) is a common complication after herpes zoster infection. While conventional dorsal column temporary spinal cord stimulation (tSCS) has been shown as an effective treatment option for this pain condition, recent data suggests ipsilateral temporary spinal nerve root stimulation (tSNRS) as a safe alternative for treating PHN. However, there is no direct clinical comparison between the newer tSNRS and the traditional tSCS. OBJECTIVES: The current retrospective study aimed to describe the technical factors and the therapeutic efficacy of tSNR for patients wi

Combined Dorsal and Ventral Spinal Cord Stimulation for Treatment of Refractory Neuropathic Pain After Spinal Cord Injury: A Case Report.

BACKGROUND: Central neuropathic pain that follows spinal cord injury is often refractory to medical therapy. Spinal cord stimulation (SCS) is an established treatment for chronic neuropathic pain, but conventional dorsal column placement may not capture all pain regions. CASE REPORT: A 63-year-old man with C6-T2 spinal cord infarction and C7 ASIA C paraplegia experienced severe refractory pain in both feet and the left leg for over a decade. Two dorsal leads provided coverage for the feet but not the leg. An additional ventral lead at T12 captured the buttock and hamstring, reducing pain from

Brain and spinal cord paired stimulation coupled with locomotor training affects polysynaptic flexion reflex circuits in human spinal cord injury.

Neurorecovery from locomotor training is well established in human spinal cord injury (SCI). However, neurorecovery resulting from combined interventions has not been widely studied. In this randomized clinical trial, we established the tibialis anterior (TA) flexion reflex modulation pattern when transcranial magnetic stimulation (TMS) of the primary motor cortex was paired with transcutaneous spinal cord (transspinal) stimulation over the thoracolumbar region during assisted step training. Single pulses of TMS were delivered either before (TMS-transspinal) or after (transspinal-TMS) transspi

Spinal Cauda Equina Stimulation for Alternative Location of Spinal Cord Stimulation in Intractable Phantom Limb Pain Syndrome: A Case Report.

Phantom limb pain is a phenomenon in which patients experience pain in a part of the body that no longer exists. In several treatment modalities, spinal cord stimulation (SCS) has been introduced for the management of intractable post-amputation pain. A 46-year-old male patient complained of severe ankle and foot pain, following above-the-knee amputation surgery on the right side amputation surgery three years earlier. Despite undergoing treatment with multiple modalities for pain management involving numerous oral and intravenous medications, nerve blocks, and pulsed radiofrequency (RF) treat

Directional leads applied to spinal cord stimulation: A computational modelling study.

Spinal cord stimulation (SCS) is widely used to treat various types of pain. Directional leads, which exhibit directional focusing properties, have been applied in deep brain stimulation. Their targeting capability holds potential for further enhancing clinical SCS protocols. However, the actual stimulation effects of directional leads remain unclear and require further investigation. This study employed a computational modelling approach to develop SCS simulation models and a multi-compartment cable model of sensory fibers. These models were used to simulate the stimulation effects of directi

Pulsed Ultrahigh Frequency Spinal Cord Stimulation for Arrhythmia After Myocardial Infarction.

BACKGROUND: Acute myocardial infarction (MI) increases sympathetic tone and increases the risk of life-threatening ventricular arrhythmias. We tested the hypothesis that spinal cord stimulation (SCS) using pulsed ultrahigh frequency (PUF) from T1 to T3 remodels the sympathetic activity and reduces ventricular arrhythmias. METHODS: Twenty pigs were used in this study. Six were assigned to the control group, 7 received MI (MI Group), and 7 received MI plus PUF-SCS (MI+SCS Group). Seven pigs underwent a second surgery to implant the spinal stimulator. Echocardiography, 7-day Holter monitoring, a

Cervical spinal cord stimulation disrupts proprioception yet improves voluntary arm reaching.

Whether proprioception is necessary for upper-limb motor control has been debated for decades. Classic studies in deafferented animals and humans suggested that proprioception may be dispensable for rapid, goal-directed movements. However, chronic sensory loss conflates the absence of proprioceptive input with years of compensatory adaptation. As a result, the field has lacked a strong causal test of proprioception's contribution to motor control. Here, we leveraged a clinical trial of cervical spinal cord stimulation (SCS) in individuals with chronic post-stroke hemiparesis to study if electr
