Etiology and response to antibiotic therapy of community-acquired pneumonia in French children.

The aim of this study was to determine the etiologic agents associated with community-acquired pneumonia in 104 French children ages 18 months to 13 years. Potential respiratory pathogens were identified in 87 (85%) cases; these included respiratory syncytial virus in ten, other viruses in 20, Streptococcus pneumoniae in 14 and Mycoplasma pneumoniae (diagnosed by serologic procedures) in 43. Of 32 patients with Mycoplasma pneumoniae infection who were initially treated with beta-lactam antibiotics, 30 failed treatment. Recovery from mycoplasma infection occurred rapidly in patients treated wit

Synthesis and evaluation of in vitro and in vivo anti-Toxoplasma gondii activity of tetraoxane-substituted ursolic acid derivatives.

A series of derivatives of ursolic acid (UA) were synthesised, the anti-Toxoplasma gondii activity was tested, and the selectivity index (SI) of these compounds was calculated to determine the derivative with the best anti-Toxoplasma gondii activity. Compound A7 showed the best activity against the Toxoplasma gondii (IC50 in T. gondii infected GES-1 cells: 9.1 ± 7.2 μM), better than the lead compound UA and the positive control drug Spiramycin. Compound A7 was selected for further in vivo research: A7 was tested for its effect on the inhibition rate of tachyzoites in mice and its biochemical p

[Therapeutic approaches to cryptosporidiosis. A review of the literature].

Cryptosporidiosis is a coccidian infection that usually occurs in children an immunocompromised patients. With the AIDS (Acquired Immunodeficiency Syndrome) epidemic there have been an increased number of clinical cases and still we don't have an optimal therapeutic regimen to eradicate the infection. Since 1907 when the organism was first described, a large amount of anti-infective agents have been used without success. We present herein a review of the new therapeutic approaches, although none of them is satisfactory and new studies are required for the development of an optimal treatment. S

Treatment of cryptosporidiosis in captive green iguanas (Iguana iguana).

There are no standard guidelines for the treatment of cryptosporidiosis in reptiles. The aim of this study was to evaluate the efficacy of two cryptosporidiosis therapies in captive green iguanas. Eight green iguanas aged 2-6 years, including 6 (1 ♂ and 5 ♀) animals with chronic diarrhea, received treatment for cryptosporidiosis. The presence of Cryptosporidium sp. oocysts was determined in 8 iguanas (100%), Isospora sp. oocysts were detected in 3 animals (37.5%), and Oxyuridae eggs were observed in 5 iguanas (62.5%). The animals were divided into two therapeutic groups (A and B). Group A igua

Matrix effect in high-performance liquid chromatography-tandem mass spectrometry analysis of antibiotics in environmental water samples.

This paper describes the matrix effect during the analysis of ten antibiotic compounds in water by SPE followed by HPLC-ESI-MS/MS. The target analytes were tetracycline, oxytetracycline (tetracyclines), sulfathiazole, sulfamethazine, sulfadiazine (sulfonamides), erythromycin-H(2)O, roxithromycine, spiramycin (macrolides), ofloxacin, and norfloxacin (quinolones). The matrix effect was examined for internal standards and the target analytes in five different water matrixes, with signal suppression being increased in the order: ultrapure water, tap water, river water, sewage effluent, and sewage

Congenital toxoplasmosis treatment.

OBJECTIVES: Congenital toxoplasmosis is a particular manifestation of Toxoplasma gondii infection, which may present as a mild or severe neonatal disease. This pathology remains a difficult challenge in terms of therapy for the pediatrician and gynecologist. In this article we have set ourselves the objective to provide an overview of the main aspects of the disease, with particular attention to the treatment, based on the information in the literature. RESULTS: Two kinds of treatment are currently available: prenatal and postnatal. When pregnant women seroconvert, spiramycin is administered

Toxoplasmosis.

Toxoplasmosis is caused by the protozoan organism, Toxoplasma gondii. Infection with this organism primarily results from contact with infected cats and from ingestion of improperly cooked meat. Most adults with toxoplasmosis are asymptomatic. When symptoms are present, they typically resemble a mononucleosis or flulike illness. The diagnosis of toxoplasmosis in the pregnant adult is best made using serological techniques to detect IgM antibody and to document significant changes in the IgG antibody titer. Congenital toxoplasmosis usually occurs as a result of primary maternal infection. The m

Anti-toxoplasma activity of a semisynthetic azasteroid derived from β-sitosterol.

β-Sitosterol from Hylotelephium erythrostictum (Miq.) H. Ohba, an abundant plant sterol, was converted into eight A-ring-modified derivatives, introducing a seven-membered ring bearing an exocyclic formyl group-a motif associated with antiparasitic activity. Evaluation against Toxoplasma gondii revealed that A-ring expansion with C-3 formylation and 2-aza substitution correlated with increased activity, while the parent compound and 4-aza analogues were inactive (IC50 values exceeded 500 µM). The most active derivative, N-formyl-2-aza-A-homo-3-chlorostigmast-5-ene, exhibited an IC50 of 206.22

[Clinical use of acetylspiramycin for primary atypical pneumonia in the field of pediatrics].

Thirty-one patients with primary atypical pneumonia (16 patients diagnosed as Mycoplasma pneumonia) who visited our Department of Pediatrics between March and December, 1981 were treated with 200 mg tablets of acetylspiramycin. Clinical effects were studied, and the results were as follows. 1. Acetylspriramycin, in principle 30 mg/kg/day, was given to 31 patients with primary atypical pneumonia. The effective rate was 87.1%. (As for the patients with Mycoplasma pneumonia, the effective rate was 93.8%). 2. There was 1 ineffective case and it was judged as a case of other viral pneumonia by the

Occurence of antimicrobial residues in milk and labneh consumed in Lebanon.

Antimicrobials are administered in livestock for different uses leading to milk contamination and several undesirable effects. Because there is a lack of surveillance of antimicrobial residues (AMRs) in milk and dairy products in Lebanon, this study aims to determine the occurrence of AMRs in 90 Lebanese samples of milk and labneh (concentrated yoghurt). Multi-residue screening methods with suitable sample preparations were applied to detect 71 AMRs in milk and labneh, respectively, using LC-MS/MS. Of the total number of samples, 71% was contaminated with AMRs and (fluoro)quinolones and macrol

An updated review of chemical compounds with anti-Toxoplasma gondii activity.

The opportunistic apicomplexan parasite Toxoplasma gondii is the etiologic agent for toxoplasmosis, which can infect a widespread range of hosts, particularly humans and warm-blooded animals. The present chemotherapy to treat or prevent toxoplasmosis is deficient and is based on diverse drugs such as atovaquone, trimethoprim, spiramycine, which are effective in acute toxoplasmosis. Therefore, a safe chemotherapy is required for toxoplasmosis considering that its responsible agent, T. gondii, provokes severe illness and death in pregnant women and immunodeficient patients. A certain disadvantag

Synthesis and biological evaluation of ursolic acid derivatives bearing triazole moieties as potential anti-Toxoplasma gondii agents.

Ursolic acid (UA), a plant-derived compound, has many properties beneficial to health. In the present study, we synthesised three series of novel UA derivatives and evaluated their anti-Toxoplasma gondii activity both in vitro and in vivo. Most derivatives exhibited an improved anti-T. gondii activity in vitro when compared with UA (parent compound), whereas compound 3d exhibited the most potent anti-T. gondii activity in vivo. Spiramycin served as the positive control. Additionally, determination of biochemical parameters, including the liver and spleen indexes, indicated compound 3d to effec

Brucella infection in a child with progressive familial intrahepatic cholestasis type 2 who had undergone liver transplantation.

Brucellosis is considered the most widespread zoonosis in the world. In endemic regions of brucellosis, childhood brucellosis includes up to one-third of all cases of human brucellosis. Brucellosis constitutes a public health problem in Turkey. A boy aged 12 yr who had PFIC2 had undergone deceased-donor liver transplantation in 2008 at the age of seven. The boy presented with fatigue, fever, and pain in the right leg and hip and was admitted to the hospital. Brucella melitensis grew in the blood culture, and the SAT was positive at a titer of 1:640. The patient was treated with oral doxycyclin

Telithromycin: visual disorders.

Telithromycin is a macrolide antibiotic with a negative risk-benefit balance. Its efficacy is no better than that of other macrolides. Telithromycin carries a risk of serious adverse effects, including loss of consciousness, QT prolongation, severe liver damage, aggravation of myasthenia gravis, and a high risk of drug interactions. In Finland, a 7-year pharmacovigilance review identified 20 reports of visual disorders among patients taking telithromycin, including blurred vision, accommodation disorder and diplopia, mainly in young patients and women. Visual disorders occurred in about 1% of

[Toxoplasmic retinochoroiditis].

Retinochoroiditis is a frequent ocular lesion in toxoplasmosis. It may be severe, and even lead to blindness. Sixteen cases of toxoplasmic retinochoroiditis verified through IFA were presented in this paper. Among these cases, 1 was of the multifocal type, its appearance being similar to that of the Coats disease; while the other 15 were of the focal type resembling to the central exudative retinitis (Rieger disease) or retinochoroidal scar. Serological finding of all the cases showed a positive IFA titer of > 1:80. A combination of pyrimethamine and sulfadiazine as well as spiramycin in addit

Antimicrobial susceptibility and susceptibility testing of Mycoplasma hominis: a review.

The determination of the minimal growth-inhibiting concentration (MIC), the minimal metabolism-inhibiting concentration (MMC), and the minimal mycoplasmacidal concentration (MCC) of various antimicrobial compounds for Mycoplasma hominis is influenced by the pH of the test media, the inoculum size, and the incubation time, although each of these factors generally do not affect the minimal concentration more than fourfold. M. hominis is resistant to beta-lactam antibiotics, vancomycin, sulfonamides, trimethoprim, and polymyxin B. There are great differences in the susceptibility of M. hominis to
