Symptomatic Extra-Adrenal Myelolipoma in the Spleen.

A 42-year-old male patient presented with intermittent abdominal pain and gastrointestinal discomfort present for 4 years. Work-up included ultrasound and computed tomography, which identified a fat-containing splenic mass 5.6 cm in size. Due to recurrent symptoms, the patient sought medical care again. Subsequent images showed an increase in size to 7.6 cm, which was concerning for neoplasm. This was removed via open splenectomy, which was challenging due to intra-abdominal adhesions despite never having had any abdominal surgery. The patient's recovery was uncomplicated. Pathologic assessmen

Splenic rupture secondary to amyloid light-chain (AL) amyloidosis associated with multiple myeloma.

Splenic rupture in the absence of major trauma is a rare occurrence, which may occur by idiopathic means or a specific pathologic process. One such condition, amyloidosis, involves the extracellular deposition of abnormally folded 'amyloid' protein, which can affect the spleen. Protein infiltration in the organ may cause splenomegaly and potentially capsular rupture in advanced cases. We describe a 68-year-old male with a history of end-stage renal disease status-post living donor renal transplant on chronic immunosuppression and Coumadin that presented with abdominal pain, weakness and hypote

Splenogonadal fusion.

Splenogonadal fusion (SGF) is a rare congenital non-malignant anomaly characterized by fusion of splenic tissue to the gonad, and can be continuous or discontinuous. Very few cases have been diagnosed preoperatively, and many patients who present with testicular swelling undergo unnecessary orchiectomy under the suspicion of testicular neoplasm. A 16-year-old boy presented with a left scrotal mass and underwent total excision of a 1.6-cm tumor without damaging the testis, epididymis or its accompanying vessels. Pathologic examination revealed SFG (discontinuous type). If clinically suspected b

Twenty-three years of the Warshaw operation for distal pancreatectomy with preservation of the spleen.

OBJECTIVE: To describe our series of distal pancreatectomies with preservation of the spleen utilizing the Warshaw operation with a focus on possible long-term complications due to the development of gastric varices. BACKGROUND: The Warshaw operation was first described in 1988. The splenic vessels are resected and the spleen survives via the short gastric and left gastroepiploic vessels. METHODS: Retrospective review of 721 patients who underwent a distal pancreatectomy between February 1986 and February 2009. RESULTS: The spleen was preserved via the Warshaw operation in 158 patients (22%

Screening colonoscopy for the detection of neoplastic lesions in asymptomatic HIV-infected subjects.

BACKGROUND: Although non-AIDS defining malignancies are rapidly increasing as HIV-infected subjects live longer, little is know about the results of screening for colonic neoplasms (adenomatous polyps and adenocarcinomas) in this population. METHODS: We conducted a screening colonoscopy study to determine the prevalence of colonic neoplasms in 136 asymptomatic HIV-infected subjects >or=50 years of age and 272 asymptomatic uninfected control subjects matched for age, sex, and family history of colorectal cancer. Advanced neoplasms were defined as adenomas >or=10 mm or any adenoma, regardless o

A tale of two colons and two cancers. Distinct carcinogenesis and clinical outcome according to location proximal or distal to the splenic flexure.

The central hypothesis explored in this paper is that colorectal cancer (CRC) is a heterogeneous disease. The initial clue to this heterogeneity was provided by genetic findings; however, embryological and physiological data had previously been gathered, showing that proximal (in relation to the splenic flexure) and distal parts of the colon represent distinct entities. Molecular biologists have identified two distinct pathways, microsatellite instability (MSI) and chromosomal instability (CIN), which are involved in CRC progression. In summary, there may be not one, but two colons and two typ

Completely laparoscopic subtotal pancreatectomy with splenic artery preservation.

INTRODUCTION: Laparoscopic distal pancreatectomy has emerged as an attractive minimally invasive alternative for selected patients. Although technically challenging, distal pancreatectomy with splenic artery preservation has consistently been correlated with reduced blood loss and perioperative morbidity in multiple studies. Herein presented is our technique for completely laparoscopic (non-hand-assisted) subtotal pancreatectomy with splenic artery preservation (LSP-SAP). METHODS: An 87-year-old woman with an incidentally identified 3-cm cystic lesion in the pancreatic body-tail interface und

Cecal Adenoma Presenting as a Large Bowel Intussusception in an Adult.

An adult intussusception is a relatively rare entity and is more commonly confined to the small bowel when it is found. The majority of the colonic intussusceptions in adults are caused by malignant neoplasms. Here we present the case of a 65-year-old female with a cecal adenoma as the lead point causing intussusception all the way to the splenic flexure. Controversy still exists regarding optimal treatment strategies, specifically the question of if it is safe or not to perform preoperative reduction prior to surgical resection in adult large bowel intussusceptions.

Marginal zone B-cell lymphomas.

The term marginal-zone lymphoma (MZL) encompasses three closely related indolent B-cell non-Hodgkin's lymphoma subtypes, namely extranodal MZL or MALT lymphoma, nodal MZL, and splenic MZL. Although these neoplasms may share a common cell of origin, being the marginal zone B-cell, they display different characteristics with evident clinical and biological variations according to the organ where the lymphoma arises. The past 2 decades have spawned an avalanche of new data that encompasses the genetic aberrations and pathogenic mechanisms leading to these diseases. This article briefly addresses

Evaluation of ERG as a biomarker for immunohistochemical diagnosis of canine splenic hemangiosarcoma.

Hemangiosarcomas (HSAs), aggressive malignant neoplasms originating from endothelial cells lining blood vessels, are the most common splenic tumors in dogs. Immunohistochemical diagnosis of HSAs currently relies on endothelial markers such as CD31, which, while useful, have suboptimal sensitivity and specificity. In human pathology, E-26 transformation specific-related gene (ERG), a nuclear transcription factor expressed in endothelial cells, has demonstrated high diagnostic performance for vascular tumors. Therefore, this study aimed to evaluate the diagnostic utility of a human-specific anti

[Warm autoimmune hemolytic anemia and IgM-monoclonal gammopathy following BNT162b2 COVID-19 vaccine in a patient with splenic marginal zone lymphoma].

There is currently no evidence that a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) mRNA vaccine might be associated with the development of autoimmune hemolytic anemia or disease progression in patients with mature B-cell neoplasm. Our patient was a 71-year-old man with indolent mature B-cell neoplasm who had been monitored for many years without treatment. After receiving the second dose of the BNT162b2 mRNA COVID-19 vaccine, he developed severe warm autoimmune hemolytic anemia. Although steroid therapy improved his anemia, he continued to develop IgM-monoclonal gammopathy, re

Mucinous cystic neoplasm in heterotopic pancreas presenting as colonic polyp.

CONTEXT: Pancreatic heterotopia in itself is rare in the colon and to the best of our knowledge a neoplasm arising in a heterotopic tissue in the colon has not been reported. We herein report a pancreatic cystic neoplasm arising from heterotopic pancreatic tissue in colon. CASE REPORT: A 44-year-old lady presented with a history of lower abdominal colic, associated with mucoid loose stools 10-15 times/day. Fecal occult blood was positive on two occasions. On examination, a vague mass was palpable in the left upper quadrant of abdomen. Colonoscopy showed a polypoid growth at the splenic flexur

Stepwise approach to laparoscopic appendicectomy in children.

In this case report of Muir-Torre syndrome (MTS), we describe a 47-year-old man with a personal and family history of colon cancer and a personal history of keratoacanthoma who presented with a sebaceous carcinoma and, subsequently, had a cystic sebaceous tumor. Immunohistochemical examination of the patient's colonic tumor, located proximal to the splenic flexure, revealed absence of MutL homolog (MLH)-1 protein. MTS is a rare genodermatosis defined clinically by the occurrence of a sebaceous neoplasm and an internal malignancy in the absence of other predisposing factors. Most patients prese

Targeting TRAF3 Downstream Signaling Pathways in B cell Neoplasms.

B cell neoplasms comprise >50% of blood cancers. However, many types of B cell malignancies remain incurable. Identification and validation of novel genetic risk factors and oncogenic signaling pathways are imperative for the development of new therapeutic strategies. We and others recently identified TRAF3, a cytoplasmic adaptor protein, as a novel tumor suppressor in B lymphocytes. We found that TRAF3 inactivation results in prolonged survival of mature B cells, which eventually leads to spontaneous development of B lymphomas in mice. Corroborating our findings, TRAF3 deletions and inactivat

Cytology and histopathology have poor to fair agreement for determination of neoplastic or nonneoplastic lesions in dogs with splenic masses or nodules.

OBJECTIVE: To evaluate the correlation between cytology and histopathology of splenic masses or nodular lesions in dogs. METHODS: Retrospective medical record review from January 2014 to July 2022 of dogs that had a splenic mass or nodular lesion and cytologic and histopathologic evaluation within 90 days of each other. Slide review was conducted by a single pathologist from each subspecialty and recorded as neoplastic, possibly neoplastic, or nonneoplastic. RESULTS: 33 dogs were included and had a median of 4 days (range, 0 to 90 days) between cytologic and histopathologic evaluation. Cytol

Application of Nuclear Medicine Liver-Spleen Scan for Evaluation of Littoral Cell Angioma of the Spleen: A Case Report.

Littoral cell angioma (LCA) is a rare primary splenic vascular neoplasm originating from the littoral cells of the reticuloendothelial system. Splenectomy is the accepted mode of definitive diagnosis and treatment. With fewer than 200 reported cases, LCA remains poorly understood. Herein, we provide an enhanced insight into its histology and highlight the role of nuclear imaging in aiding LCA diagnosis. A 63-year-old female with a history of stage II multiple myeloma (MM) and rheumatoid arthritis was incidentally found to have a slowly enlarging splenic mass over a 6-year period. Given her can

Pathologic diagnosis, origin, and natural history of pseudomyxoma peritonei.

Mucinous ascites and pools of mucin within the peritoneal cavity associated with neoplastic, mucinous epithelium are the characteristic features of pseudomyxoma peritonei (PMP). Clinically, PMP presents with abdominal distension and gelatinous ascites. In female patients, pelvic masses can be seen. Radiologic findings on computed tomography include scalloping of the hepatic and splenic margins and dense ascites. Surgically, PMP is encountered as grossly visible mucin in the peritoneal cavity. The presence of mucin outside of the appendix, in the right lower quadrant, and beyond is an important

Diffuse Gastric Uptake on 18 F-FDG PET/CT Caused by Involvement of Mastocytosis in a Patient With Systemic Mastocytosis.

A 65-year-old man with systemic mastocytosis and associated hematological neoplasm (plasma cell myeloma, SM-AHN) received an 18 F-FDG PET/CT for staging. In addition to the typical bone marrow and splenic uptake, PET/CT also showed diffuse 18 F-FDG uptake in the gastric wall. It suggested the involvement of myeloma or mastocytosis. Subsequent histopathologic analysis of gastric biopsy specimens through endoscopy confirmed the diagnosis of mastocytosis involvement.

Effects of indinavir in a preliminary phase I study on dogs with stage III slenic hemangiosarcoma.

HIV protease inhibitors are antiretroviral drugs able to prevent production of infectious particles. It has been shown that these protease inhibitors are able to inhibit cancer-promoted angiogenesis in patients affected by Kaposi's sarcoma. A preliminary phase I study on dogs with stage III splenic hemangiosarcoma was designed in order to evaluate the efficacy and toxicity of the protease inhibitor Indinavir to delay the progression of this advanced neoplasm. The results suggest that Indinavir is potentially beneficial in dogs affected by microscopic residual disease.
