Voice disorders in the elderly.

OBJECTIVES: Despite the prevalence of voice disorders, as well as the physiological and functional changes of the aging larynx, there is a lack of data analyzing dysphonia in the geriatric population. The goal of this study was to investigate dysphonia in this cohort. STUDY DESIGN: Retrospective chart review. METHODS: This study analyzes the histories, demographics, Voice Handicap Index (VHI) questionnaires, and objective voice measures (OVMs) for 175 patients with voice complaints, age ranging from 65 to 89 years. Diagnoses of any vocal fold pathology were made via strobovideolaryngoscopy a

Nanofat for Injection Laryngoplasty: Office and Operative Results.

OBJECTIVE: Nanofat is fat emulsified into micron-sized particles. Unlike microfat, nanofat increases the release of adipose-derived stem cells by destroying lipocytes. This study evaluated whether nanofat injection laryngoplasty alone leads to lasting bulking and sustained long-term voice improvement. METHODS: We retrospectively reviewed patients with glottal insufficiency due to atrophy, paresis, or scarring who underwent operative (n = 13) or office-based (n = 18) nanofat injection laryngoplasty. The average injected fat volume was 0.5 mL per side. Median follow-up was 12 ± 4 months. Strobo

The effect of vocal fold augmentation on cough symptoms in the presence of glottic insufficiency.

OBJECTIVE: To determine the effect of injection augmentation of the vocal folds on chronic cough symptoms in patients with glottic insufficiency. METHODS: Medical records from 146 consecutive patients who underwent vocal fold injection augmentation by a fellowship-trained laryngologist between 2013 and 2015 were reviewed. Twenty-three patients (12 male) met inclusion criteria of a vocal fold augmentation injection, cough symptoms lasting more than 8 weeks, and glottic insufficiency as determined by shortened closed phase on stroboscopy. Exclusion criteria included lack of cough complaints, di

Analysis of laryngoscopic features in patients with unilateral vocal fold paresis.

OBJECTIVES/HYPOTHESIS: The diagnosis of paresis in patients with vocal fold motion impairment remains a challenge. More than 27 clinical parameters have been cited that may signify paresis. We hypothesize that some features are more significant than others. STUDY DESIGN: Prospective case series. METHODS: Two laryngologists rated laryngoscopy findings in 19 patients suspected of paresis. The diagnosis was confirmed with laryngeal electromyography. A standard set of 27 ratings was used for each examination that included movement, laryngeal configuration, and stroboscopy signs. A Fisher exact t

Assessment of Tongue Position and Laryngeal Height in Two Professional Voice Populations.

Purpose To advance our current knowledge of singer physiology by using ultrasonography in combination with acoustic measures to compare physiological differences between musical theater (MT) and opera (OP) singers under controlled phonation conditions. Primary objectives addressed in this study were (a) to determine if differences in hyolaryngeal and vocal fold contact dynamics occur between two professional voice populations (MT and OP) during singing tasks and (b) to determine if differences occur between MT and OP singers in oral configuration and associated acoustic resonance during singin

Presbyphonia and Minimal Glottic Insufficiency.

Presbyphonia is defined as aging of the voice, and its growing attention as a medical concern parallels the continuing increase of the geriatric population worldwide. It results from physiologic changes to several laryngeal structures, including the musculature, surrounding cartilage, and lamina propria of the vocal folds. Characterized by glottic insufficiency, dysphonia in the elderly typically presents as a deterioration of voice quality, altered pitch and fundamental frequency, vocal fatigue, and strain. Fortunately, there are multiple treatment options that have proven successful in impro

Dynamic performance of microelectromechanical systems deformable mirrors for use in an active/adaptive two-photon microscope.

Active optics such as deformable mirrors can be used to control both focal depth and aberrations during scanning laser microscopy. If the focal depth can be changed dynamically during scanning, then imaging of oblique surfaces becomes possible. If aberrations can be corrected dynamically during scanning, an image can be optimized throughout the field of view. Here, we characterize the speed and dynamic precision of a Boston Micromachines Corporation Multi-DM 140 element aberration correction mirror and a Revibro Optics 4-zone focus control mirror to assess suitability for use in an active and

Comparison between palpatory findings of the hyoid position and their acoustic, videostroboscopic, and perceptual attributes in patients with muscle tension dysphonia (with and without organic lesions).

SUMMARY: The direction in which the hyoid is pulled may change as a result of muscle tension dysphonia (MTD). The disposition usually occurs owing to paralaryngeal muscular imbalance (decompensation), which can create laryngeal dysfunction and consequently deteriorating the voice quality. This study aimed at reporting on a comparison between palpatory findings of hyoid position and their acoustic and perceptual and stroboscopic attributes among patients with MTD. DESIGN: Cross-sectional random triple-blinded study. METHOD: Palpatory, acoustic, audioperceptual, and stroboscopic findings of 39

[The ultra-rapid cinematography of the larynx, its contributions in speech pathology].

The development in the medical field of high speed cinematography and its dissemination in the field of speech pathology will probably change the way we view the larynx and diagnose its disorders. So far only the stroboscope could inform us about the appearance of vocal cord vibration but with certain limitations. Indeed the wave motion of the vocal cords is really only apparent motion. It is made possible through the phenomenon of retinal persistence of images and light intermittent vocal folds set to the frequency of the voice and out of phase by a few Hertz. This technique has several disad

[Phonosurgery of chronic vocal cord edema].

INTRODUCTION: Chronic (Reinke's) oedema of the vocal folds is a frequent and declicate objective of phonosurgery. It is characterized by a marked bilateral subepithelial oedema, which develops by degrees, as a non-specific reaction of the vocal folds to various irritative noxious agents (especially smoking), in patients with some predisposition. It is found, by the light and electron microscopes and immunohistochemistry, that oedema is characterized by subepithelial fissure-like spaces, which accumulate a protein-rich fluid, and develops like neobursae. Therefore, mechanical factors and functi

Laryngeal dysfunction after thyroid surgery: diagnosis, evaluation and treatment.

Because of the close anatomical relationships between thyroid gland and laryngeal nerves, sensory-motor impairment of the laryngeal functions is a well known possible complication of thyroid surgery. Laryngeal nerve paralysis can present with various and often associated symptoms like dysphagia, aspiration, voice alteration or dyspnea. Several examination procedures are mandatory to perform a complete neuro-laryngeal evaluation: rigid and flexible video-stroboscopy will assess the abductor, adductor and tensor functions in breathing, sniffing, talking and eventually singing tasks. Laryngeal el

Effects of Balance Training Combined With Either Transcranial Direct Current Stimulation or Stroboscopic Vision Training on Balance Performance in Individuals With Chronic Ankle Instability: A Systematic Review and Meta-Analysis.

INTRODUCTION: Despite the benefits of balance training, persistent postural deficits remain common in individuals with Chronic Ankle Instability (CAI). The aim of this study investigates the efficacy of Stroboscopic Vision Training (SVT) and transcranial Direct Current Stimulation (tDCS) as adjunct modalities to improve balance in this population. METHODS: This review was developed in accordance with the PRISMA checklist. Five electronic databases (Web of Science, Scopus, PubMed, EBSCO, and Embase) along with three supporting search platforms and libraries (ScienceDirect, Cochrane Library, an

Sulcus Vocalis.

Sulcus vocalis, defined as a type of groove along the free edge of the vocal fold, disrupts the normal, pliable vocal fold cover, causing alterations in the intrinsic mucosal wave. The primary symptom is breathy, effortful dysphonia. Diagnosis may be challenging, and this classically has led to difficulties with identification and management. Treatment options, although often promising, have been notoriously unreliable. Current understanding, considerations for management, and future treatment options are explored.
