Epidemiological characteristics and in-hospital management of acute coronary syndrome patients in Greece: results from the TARGET study.

INTRODUCTION: Conduction of national surveys is needed to depict temporal trends in the risk profile, type of implemented treatment strategy and outcome of patients with acute coronary syndromes (ACS). The TARGET study is a multicenter, observational study that aimed to evaluate the epidemiological characteristics, management pattern and outcome of ACS patients in Greece. METHODS: A total of 418 consecutive patients with ACS (44.7% STEMI, 34.2% NSTEMI, 21.1% unstable angina) from 17 centers (52.9% with catheterization facilities) were enrolled in the study (78.0% males, 63.9 ± 12.9 years). Atrial Septal Aneurysm Leading to Ischemic Stroke: A Case Report and Literature Review.

Stroke is the most common cause of death and disability in the world, and ischemic etiology plays a major role. Atrial septal aneurysm (ASA) is a localized saccular deformity of the atrial septum, associated with ischemic stroke independently or in association with other atrial septal defects (ASD). There is a higher incidence of stroke in the population with ASD. In these patients, the presence of ASA is an important predictor of recurrent stroke. This is a case of ischemic stroke in a 44-year-old who presented with sudden-onset right-sided body weakness, expressive aphasia, and non-specific

Acute Stroke Management: Overview and Recent Updates.

Stroke is a leading cause of morbidity and mortality in the United States. Whether hemorrhagic or ischemic, stroke leads to severe long-term disability. Prior to the mid-1990s, the treatment offered to a patient who presented with an acute stroke was mainly limited to antiplatelets. The lack of adequate treatment, in particular, one without reperfusion contributed to the disability that ensued. There have been many advances in stroke care within the past two decades, especially with the acute management of ischemic stroke. Even with these advances, it is quite alarming that only a fraction of

Enzymes approved for human therapy: indications, mechanisms and adverse effects.

Research and drug developments fostered under orphan drug product development programs have greatly assisted the introduction of efficient and safe enzyme-based therapies for a range of rare disorders. The introduction and regulatory approval of 20 different recombinant enzymes has enabled, often for the first time, effective enzyme-replacement therapy for some lysosomal storage disorders, including Gaucher (imiglucerase, taliglucerase, and velaglucerase), Fabry (agalsidase alfa and beta), and Pompe (alglucosidase alfa) diseases and mucopolysaccharidoses I (laronidase), II (idursulfase), IVA (

[The best of thrombosis in 2002].

In the area of myocardial infarction one is reminded of the publication of the CADILLAC study which has reopened the debate on the systematic use of GpIIbIIIa inhibitors in the acute phase of myocardial infarction complementing primary angioplasty with the placement of an endoprosthesis. New modalities for thrombolysis are in the course of evaluation, notably Eptibaphide Alteplase combination in the INTRO-AMI study and Tenecteplase Abciximab in association with enoxaparine or non-fractionated heparin in the TIMI 23 study. Several studies comparing angioplasty to lysis have been published. STOP

Beyond the Catheter: Contemporary Aspiration Pump Systems in Neurointerventional Stroke Therapy.

Acute ischemic stroke is the greatest cause of morbidity and mortality among cerebrovascular pathologies. Over time, several therapeutic options have been developed for treating patients with stroke, including IV r-tPA (intravenous recombinant tissue-type plasminogen activator) and IV-TNK (intravenous tenecteplase) thrombolysis, and endovascular mechanical thrombectomy, with the latter showing favorable recanalization rates. Stent retrievers and mechanical aspiration, the 2 types of mechanical thrombectomy, are used for acute ischemic stroke due to large-vessel occlusions. The first reported a

BACKGROUND: Central retinal artery occlusion (CRAO) is a vision-threatening neuro-ophthalmic emergency, analogous to acute ischemic stroke. Delayed presentation, limited therapeutic options, and lack of randomized prospective trials continue to restrict effective treatment. EVIDENCE ACQUISITION: This review summarizes current evidence on the emergent management and evolving treatment strategies for CRAO, based on recent observational studies, meta-analyses, and early randomized trials. RESULTS: Conservative interventions, such as ocular massage, paracentesis, and vasodilator use, have shown

Thrombolysis in Prosthetic Valve Thrombosis: A Safe and Effective Alternative?

Prosthetic valve thrombosis (PVT) is a rare but life-threatening complication of mechanical heart valves. Surgery remains the standard of care, but thrombolysis has emerged as a valuable alternative in patients with high surgical risk or refusal. We describe a 38-year-old woman, five years after double mitral-aortic mechanical valve replacement, who had discontinued anticoagulation one month prior to admission. She presented with acute angina-like chest pain and severe dyspnea (New York Heart Association (NYHA) class IV). Clinical examination revealed diminished prosthetic clicks and pulmonary

Fibrinolytic Therapy for Thromboembolic Diseases: Approved Indications and Future Directions.

Fibrinolytic therapy plays a pivotal role in the management of thromboembolic diseases by promoting plasmin-mediated fibrin degradation and vessel recanalization, which frequently comes at the cost of increased risk of hemorrhagic events, including intracranial bleeding. Since the discovery of streptokinase in 1933, several additional fibrinolytic agents have been developed. Two plasminogen activators-urokinase and tissue plasminogen activator (tPA)-were identified in human samples. Through recombinant DNA technology, tPA was cloned and modified to produce alteplase. Subsequent efforts to prol

N-acetylcysteine (NAC) as an adjunct to intravenous fibrinolysis in patients with acute ischemic stroke: a single group study (NAC-Safety).

Recombinant tissue plasminogen activator (alteplase) and its derivative tenecteplase are approved for acute ischemic stroke (AIS), but their low recanalization rates remain a limitation. Resistance to intravenous (IV) fibrinolysis may arise from platelet cross-linking during arterial thrombosis, mediated by von Willebrand factor (VWF) multimers. N-Acetylcysteine (NAC) has demonstrated potential to cleave large VWF multimers in preclinical studies, suggesting its potential as an adjunct therapy. The primary safety endpoint was the rate of symptomatic intracranial hemorrhagic (ICHs) transformati

The role of endogenous tissue-type plasminogen activator in neuronal survival after ischemic stroke: friend or foe?

Endogenous protease tissue-type plasminogen activator (tPA) has highly efficient fibrinolytic activity and its recombinant variants alteplase and tenecteplase are established as highly effective thrombolytic drugs for ischemic stroke. Endogenous tPA is constituted of five functional domains through which it interacts with a variety of substrates, binding proteins and receptors, thus having enzymatic and cytokine-like effects to act on all cell types of the brain. In the past 2 decades, numerous studies have explored the clinical relevance of endogenous tPA in neurological diseases, especially

Engineering mammalian cells for detection and treatment of cardiac injury.

Early detection of myocardial abnormalities or other ischemic heart diseases is critical for effective treatment. Here, we aimed to engineer a cell-based system to sense cardiac troponin I (cTnI), an early marker of acute myocardial infarction (AMI), and respond by releasing a thrombolytic agent. To detect cTnI, we engineered a chimeric troponin receptor (TropR) that contains extracellular single-chain variable fragments (scFvs) and signals via intracellular domains of interleukin 6 receptor subunit beta (IL6RB), epidermal growth factor receptor (EGFR), fibroblast growth factor receptor 1 (FGF

Feasibility of a Multi-Laboratory Model of Middle Cerebral Artery Thromboembolic Stroke with Thrombolysis: TE-MCAo.

No single animal stroke model satisfies all needs of translational stroke investigation. While the nylon filament model is widely accepted in preclinical translational stroke research, thromboembolic models have more physiological relevance. Thromboembolic models are technically difficult, time consuming, and show variable results, making them problematic for multi-laboratory preclinical network use. We sought to develop and validate a multi-laboratory thromboembolic middle cerebral artery occlusion model that encompasses vessel occlusion and subsequent thrombolysis. To reduce the numbers of d

Treatments in Ischemic Stroke: Current and Future.

BACKGROUND AND AIM: Despite progress made over the last 30 years, stroke is still a leading cause of disability and mortality; likewise, its burden is expected to increase over the next decades, due to population growth and aging. The development of drugs with better safety-efficacy profiles as well as strategies able to improve ischemic stroke management from the pre-hospital setting is needed. SUMMARY: The pathophysiology of ischemic stroke involves multiple pathways resulting in cerebral artery obstruction and brain tissue ischemia. To date, the only approved drug for acute ischemic stroke

Management of No-Reflow With Intracoronary Thrombolysis: A Case Report.

No-reflow is a recurring phenomenon during coronary angioplasty that consists of the failure to reestablish blood flow after clearing an occluded coronary artery. Several treatments have been proposed for the management of no-reflow, including intracoronary thrombolysis, which has emerged in recent years as one of the means of managing this phenomenon. We report the case of a 68-year-old female patient with a high cardiovascular risk, admitted for the management of non-ST-elevation acute coronary syndrome, who presented a no-reflow phenomenon after angioplasty of the right coronary artery. Synchronous Acute Ischemic Stroke and Acute Myocardial Infarction: A Case Report.

The synchronous presentation of acute ischemic stroke (AIS) and acute myocardial infarction (AMI), termed concurrent cardiocerebral infarction (CCI), is a rare but fatal condition that poses challenges in diagnosis and management. This case report contributes to the limited literature on the successful management of a patient presenting with CCI while emphasizing the risk-benefit balance in thrombolysis and dual antiplatelet therapy. We report a 51-year-old male who presented to the emergency department with a stroke alert after experiencing right-sided flaccid paralysis and aphasia. Unique Presentation of Vertebral Artery Dissection: A Case Report.

BACKGROUND: Vertebral artery dissection (VAD) is a common cause of stroke within the younger patient population. It is usually associated with headache or neck pain, as well as signs and symptoms of a posterior circulation stroke, such as vertigo, dysarthria, and ataxia. However, there are very scarce reports of patients with this pathology presenting with focal neurologic deficits, which are more indicative of a cerebral stroke. CASE PRESENTATION: This case involves a 46-year-old woman who presented to the emergency department (ED) with complaints of right-sided headache and neck pain, which
