Negative pleural suction in thoracic trauma patients: A randomized controlled trial.

BACKGROUND: The study aimed to establish the benefits of using chest tubes with negative pleural suction against trapped water in patients with penetrating or blunt chest trauma who underwent tube thoracostomy, in terms of the incidence of complications, such as persistent air leak, clotted hemothorax, empyema, and duration of stay. METHODS: Patients who underwent tube thoracostomy because of traumatic pneumothorax, hemothorax, or hemopneumothorax were randomly assigned into one of two groups: in Group 1, the three-bottle drainage system was connected to a negative suction; in Group 2, no suc

[Analysis of postoperative thoracic empyema].

We experienced 15 cases of pyothorax after thoracotomy. Six were pyothorax without bronchial fistula and 9 were with fistula. All of former were cured by the closed drainage and/or open window thoracostomy. Two of pyothorax with fistula, who were treated with the tube drainage, died of respiratory insufficiency. Five cases who underwent the open window thoracostomy survived well. Omentopexy was performed in 2 of pyothorax with fistula and results were excellent.

Management of pleural injuries during retroperitoneal surgical procedures.

PURPOSE: The necessity of a postoperative chest tube for the treatment of pneumothorax after a radical nephrectomy is controversial. MATERIALS AND METHODS: A five-year retrospective study was performed on 91 patients, having undergone a radical nephrectomy. We examined the existence and length of a pleural rent, presence of a postoperative thoracostomy tube, postoperative pneumothorax, postoperative pneumonia, postoperative atelectasis, pain scores, hemoglobin saturation, and length of hospital stay with univariate and multivariate analysis. RESULTS: Incidence of a pleural rent occurred in 2

Successful Medical Management of Bilateral Pneumothorax Due to Nasogastric Tube Misplacement in a Cat.

A 7 yr old female spayed domestic shorthair was evaluated for suspected lily ingestion and acute vomiting. The cat had vomited suspected lily plant material before presentation, and a nasogastric tube (NGT) was placed to continue to administer activated charcoal. The NGT was passed with sedation and limited restraint. To confirm placement, a single lateral radiograph was taken, which showed that the tube was in the trachea, bronchus, through the pulmonary parenchyma, and extending into the region of the craniodorsal retroperitoneal space. The tube was subsequently removed, resulting in a tensi

Predictors of retained hemothorax in trauma: Results of an Eastern Association for the Surgery of Trauma multi-institutional trial.

BACKGROUND: The natural history of traumatic hemothorax (HTX) remains unclear. We aimed to describe outcomes of HTX following tube thoracostomy drainage and to delineate factors that predict progression to a retained hemothorax (RH). We hypothesized that initial large-volume HTX predicts the development of an RH. METHODS: We conducted a prospective, observational, multi-institutional study of adult trauma patients diagnosed with an HTX identified on computed tomography (CT) scan with volumes calculated at time of diagnosis. All patients were managed with tube thoracostomy drainage within 24 h

Iatrogenic cardiac injury following prehospital needle decompression: a case report.

Tension pneumothorax is a life-threatening condition that necessitates prompt intervention, typically via prehospital needle thoracostomy. However, this procedure carries risks such as cardiac injury. We report the case of a 20-year-old man involved in an all-terrain vehicle accident who presented with a low Glasgow Coma Scale (GCS) score, respiratory distress, and hypotension. Prehospital intubation and left-sided needle thoracostomy were performed for suspected pneumothorax and depressed GCS score. Pulsatile bleeding from the catheter was noted. Imaging in the trauma bay revealed catheter pe

Chest tube perforation of esophagus following repair of esophageal atresia.

The two major acute thoracic complications of closed chest tube thoracostomy are pulmonary laceration and vascular compression. We have noted that closed chest tube thoracostomy can also perforate an esophageal anastomosis or myotomy site. Clinically, such a perforation produces a profuse discharge of gas and/or fluid through a chest tube positioned at the level of the anastomosis or myotomy site. Plain films demonstrate an accumulation of extrapleural gas and/or fluid adjacent to the distal portion of the chest tube. If untreated, these accumulations may form into an extrapleural abscess.

Pleural disease and acquired immune deficiency syndrome.

Patients with acquired immune deficiency syndrome (AIDS) do not frequently have pleural complications. However, pneumothorax is a troublesome complication of patients with AIDS. At some medical centres, more than 50% of patients with spontaneous pneumothorax have AIDS. Most patients with spontaneous pneumothorax and AIDS have Pneumocystis carinii infection and necrotic subpleural blebs. The pneumothoraces in these patients usually cannot be managed with tube thoracostomy alone. Patients who do not respond to tube thoracostomy are best managed with a Heimlich valve or with thoracostomy with sta

Water-seal duration and chest tube removal outcomes after traumatic pneumothorax: a retrospective cohort study.

INTRODUCTION: Thoracic trauma may result in pneumothorax or hemothorax requiring chest tube (CT) insertion. CTs can be managed with active suction and/or a water-seal (WS) system, but the optimal WS duration before safe removal remains unclear. We evaluated the association between WS duration and tube thoracostomy failure in traumatic pneumothorax. MATERIALS AND METHODS: We performed a retrospective cohort study of patients who underwent tube thoracostomy for traumatic pneumothorax at an urban Level 1 trauma center from January 2016 to October 2022. Two multivariable logistic regression model

A selective non-operative approach to thoracic stab wounds is safe and cost effective - a South African experience.

INTRODUCTION: Penetrating thoracic trauma is common and costly. Injuries are frequently and selectively amenable to non-operative management. Our selective approach to penetrating thoracic trauma is reviewed and the effectiveness of our clinical algorithms confirmed. Additionally, a basic cost analysis was undertaken to evaluate the financial impact of a selective nonoperative management approach to penetrating thoracic trauma. MATERIALS AND METHODS: The Pietermaritzburg Metropolitan Trauma Services electronic regional trauma registry hybrid electronic medical records were reviewed, highlight

Characteristics of medically and surgically treated empyema patients: a retrospective cohort study.

BACKGROUND: The role of drainage, intrapleural fibrinolytics, and/or surgery in the management of thoracic empyema is controversial. OBJECTIVES: We aimed to investigate the operational practice of empyema management at our hospital. METHODS: Between January 2001 and December 2008, all patients with thoracic empyema were retrieved. After exclusion of patients with malignant effusion, traumatic or iatrogenic empyema, and a history of pleurodesis or tuberculosis, we compared the characteristics of medically versus surgically treated empyema patients. RESULTS: Seventy-eight of 215 retrieved pat

A novel technique for Endobronchial Watanabe Spigot placement for bronchopleural fistula: a traction method.

Empyema with bronchopleural fistula is a challenging condition that often requires open-window thoracostomy. Bronchial occlusion with Endobronchial Watanabe Spigot is an effective and less invasive procedure to close the fistula; however, delivering a spigot into the target bronchus requires high technical skills. Herein, we report a case of bronchopleural fistula occluded with a spigot using a new 'traction method' in which the spigot is tied as an anchor to a guidewire and pulled into the target bronchus. Our method allows selective and steady placement of the spigot for patients who undergo

Endotracheal tube as chest tube: a back-up alternative in resource limited settings.

Thoracostomies, and subsequent placements of chest tubes (CTs), are a standard procedure in several domains of medicine. In emergency medicine, thoracostomies are indicated to release a relevant hemothorax or pneumothorax, particularly a life-threatening tension pneumothorax. In many cases, an initial finger-assisted thoracostomy is followed by placement of a CT to ensure continuous decompression of blood and air. CTs prevent the reoccurrence of a hemothorax or pneumothorax, which may otherwise develop by closure of the initial thoracostomy incision. CTs are commercial, purpose-made products;

The risk factors and management of posttraumatic empyema in trauma patients.

BACKGROUND: Posttraumatic empyema increases patient morbidity, mortality and length of hospital stay, and the cost of treatment. The aim of this study was to identify the risk factors for posttraumatic empyema and to review our treatment outcomes in patients with this condition. METHODS: A total of 2261 patients who were admitted with thoracic traumas and underwent tube thoracostomy between January 1989 and January 2006 were investigated retrospectively. Posttraumatic empyema developed in 71 patients. Logistic regression was used to assess the association between potential risk factors for po

Management of spontaneous pneumothorax in patients with COVID-19.

OBJECTIVES: The coronavirus disease 2019 (COVID-19) pneumonia may cause cystic features of lung parenchyma which can resolve or progress to larger blebs. Pneumothorax was more likely in patients with neutrophilia, severe lung injury and a prolonged clinical course. The timely diagnosis and management will reduce COVID-19-associated morbidity and mortality. METHODS: We present 11 cases of spontaneous pneumothorax managed with chest tube thoracostomy or high-dose oxygen therapy. Isolated spontaneous pneumothorax was detected in all cases. RESULTS: Eight cases were male and 3 cases were female.

Colorimetric capnography adjunct during prehospital needle decompression of tension pneumothorax improves decompression success and reduces need for repeat prehospital decompression.

BACKGROUND: Tension pneumothorax is not uncommon. Effective Decompression of tension pneumothorax is lifesaving. Current guidelines recommend needle decompression (ND) as the initial decompression procedure. The metric of successful decompression is for operators to use "a gush of air" and monitor changes in vital signs. Effective needle decompression for tension pneumothorax in the prehospital environment is challenging. Studies report consistently a high failure rate of decompression. We hypothesize treatment of tension pneumothorax with needle decompression incorporating colorimetric indica

Unexpected Fatal Tension Pneumothorax: A Case Report Regarding a Patient With Multiple Traumas on Air Medical Transportation.

A 45-year-old male driver was assessed by the road rescue team after a car crash. He was in shock and had a deep second-degree burn, multiple bone fractures, and chest wounds. After stabilization, he was delivered to the nearest local clinic by an ambulance. The doctor decided to refer him via air transportation due to the lack of sufficient facilities and the urgent need for an intensive care unit after coordination with the nearest hospital and air medical team. A Bell 214C medical helicopter was called, and transfer was initiated after patient preparation by the air medical team. A few minu

A Novel Technique for Temporally Securing a Chest Tube in a Resource-Limited Environment.

In the prehospital, transport, and resource-limited setting, patients with traumatic hemothorax, pneumothorax, or cardiac arrest require emergency tube thoracostomy for stabilization and transport. With the possibility of multiple patients, limited providers, and inability to commit a 1:1 provider-to-patient ratio for safe tubeless thoracostomies, a chest tube is often the safest option. Mercy Health Life Flight Air Medical program has developed practice over decades using towel clamps and tape to achieve securement rapidly and reliably. We report on this subject as an option for temporarily s
