Simultaneous video-assisted thoracoscopic surgery sleeve lobectomy and thymectomy.

Thymic carcinoid tumour associated with pulmonary squamous cell carcinoma is very rare. We present a case of synchronous lung cancer and mediastinal tumour treated with simultaneous video-assisted thoracoscopic surgery (VATS) sleeve lobectomy and thymectomy. A 67-year old man presented with cough and bloody sputum. Chest computed tomography showed an anterior mediastinal mass with a right hilar nodule. A right upper sleeve lobectomy and a thymectomy were performed via a VATS approach. Thymic carcinoid tumour associated with pulmonary squamous cell carcinoma was diagnosed, and the patient recei

Mediastinoscopic extended thymectomy for pediatric patients with myasthenia gravis.

BACKGROUND: Extended thymectomy is indicated for children with myasthenia gravis (MG) when drug-resistance or dependence is seen. We have employed a technique for mediastinoscopic extended thymectomy (MET) on children with MG. METHOD: A total of 14 children underwent MET at Kanagawa Children's Medical Center between 2005 and 2013. A mediastinal operation field was made by a V-shaped hook infrasternally to extirpate the thymus with adipose tissue around the thymus. RESULTS: The operation time and the amount of blood loss were 182±44 minutes and 34±43 ml, respectively. Postoperative complicati

Thoracoscopic thymectomy: technical pearls to a 21st century approach.

PURPOSE: Thoracoscopic approaches to thymectomy have increased as imaging and instrumentation have advanced. Indications for the thoracoscopic approach are evolving. We reviewed our experience in the transition from sternotomy to thoracoscopy and have gleaned technical points to aid in performing thymectomy. METHODS: The experience during the transition of sternotomy to thoracoscopy was reviewed. RESULTS: THE FOLLOWING COMPONENTS HAVE BEEN OBSERVED TO BE ADVANTAGEOUS: (I) initial patient positioning is crucial; (II) thoracoscopy provides improved visualization (a separate camera setup can fa

Preliminary results of VATS thymectomy for pediatric myasthenia gravis.

Myasthenia gravis (MG) is a debilitating disease which necessitates long-term medical therapy. If left untreated, it can have a high mortality rate. The commonest variety in children, the autoimmune variety, often requires recourse to immunomodulation including prolonged usage of high-dose steroids. Thymectomy has not been a popular option among treating clinicians. There is evidence to suggest that if thymectomy is performed early in the disease, it has a high success rate in reducing the doses of the oral steroid medication and also in inducing remission of the disease. We have performed vid

Discordant thymectomy in identical twins concordant for myasthenia gravis.

BACKGROUND: Thymectomy is standard therapy fornonthymomatousmyasthenia gravis despite the absence of randomized clinical trials (1). Myasthenia gravis is uncommonly reported in monozygous twins; disease concordance occurs in approximately one third of such identical twin pairs; and treatment for myasthenia gravis, when described,is usually concordant in identical twin pairs (2). OBJECTIVE: To report an 11-year clinical course of a pair of identical twins concordant for generalized acetylcholine receptor antibody–positive nonthymomatous myasthenia gravis in whom only 1 was treated with extende

Thymic carcinoma in myasthenia gravis developing years after thymectomy.

We report two patients with myasthenia gravis (MG) who underwent thymectomy but developed thymic carcinoma years after the initial surgery. In one patient, the initial thymic pathology was normal, whereas the other had an encapsulated benign thymoma that was found only on pathological assessment. These cases demonstrate that MG may occur as part of a "new" paraneoplastic syndrome even after thymectomy. The late appearance of metastatic thymoma raises questions about monitoring for these patients.

Mid-term results of thoracoscopic thymectomy for myasthenia gravis.

CONTEXT: Video-assisted thoracic surgery (VATS) has been proposed as a less invasive technique for treatment of myasthenia gravis. MATERIALS AND METHODS: A total of 31 patients underwent a right-sided VATS to remove all anterior mediastinal fat and thymic tissue during a 4-year period in our institution. None of the patients had associated thymoma. RESULTS: All procedures were performed successfully with no conversion to sternotomy. The mean operating time was 190 minutes. The median intubation time and assisted ventilation were 24 and 18 hours, respectively. The median ICU and hospital stay

Thymectomy to achieve primary closure of total sternal cleft.

Total sternal cleft is a rare congenital malformation that may represent a challenge for the surgeon. In neonatal period, the primary closure is usually achievable, but at older age, this can be impossible. Moreover, the closure of the 2 sternal bars can bring to an exaggerated compression on mediastinal structures. Many different techniques for the treatment of sternal clefts have been reported in literature. We describe one case of total sternal cleft repaired at 4 months of age in which a partial thymectomy was performed to achieve the primary closure. We suggest considering this simple pro

Thymectomy for myasthenia gravis: prognostic factors in 70 patients.

Thymectomy has become increasingly accepted as an efficacious procedure for myasthenia gravis, with high rates of complete clinical remission. Predictors of the response to thymectomy for myasthenia gravis vary in the literature. We retrospectively reviewed the clinical records of 70 patients (63% female; mean age, 38 years) diagnosed with myasthenia gravis from August 1993 to August 2004, to determine the factors predicting outcome. Complications occurred in 20%, but there was no hospital mortality. Complete clinical remission was obtained postoperatively in 47%. Our results indicate that pat

Video-assisted thoracic surgery thymectomy for nonthymomatous myasthenia gravis.

STUDY OBJECTIVES: Minimal-access thymectomy has become increasingly popular as surgical treatment for patients with nonthymomatous myasthenia gravis (NTMG) because of its comparable efficacy, safety, and lesser degree of tissue trauma compared with conventional open surgery. We reviewed and analyzed our data on video-assisted thoracic surgery (VATS) thymectomy and present the clinical outcomes according to the Myasthenia Gravis Foundation of America classification. DESIGN: A retrospective review of VATS thymectomy for NTMG in a university hospital over a 12-year period. Data were collected fr

Effect of thymectomy in immune diseases other than myasthenia.

In the course of thymectomy of patients with myasthenia gravis, surprising data concerning the recovery or considerable improvement of other immune disorders have been observed. Among these disorders rheumatoid arthritis figured in six instances, immune thrombocytopenia in two cases, polymyositis and psoriasis in one case each. Thymectomy as a powerful immunosuppressive procedure may have a role in the therapeutic management of some immune disorders other than myasthenia gravis.

Long-term prednisone followed by thymectomy in myasthenia gravis.

The present series of thirty patients has led us to certain conclusions concerning the management and treatment of patients with myasthenia gravis. The use of cholinesterase inhibitors alone is reserved for those patients with purely ocular myasthenia whose deficits can be satisfactorily corrected with those agents. Some of those with ocular involvement may be disabled; and in light of our excellent results with that small group, as well as similar findings presented by Fischer et al., patients with disabling or refractory ocular myasthenia should be considered for treatment with prednisone. Consequences of neonatal thymectomy in New Zealand black mice.

The possible role of the thymus in autoimmune disease was studied by comparing the effects of neonatal thymectomy on New Zealand Black (NZB) mice (which develop a Coombs positive haemolytic anaemia) and on C3H/Bi, F1 (C57BL × C3H/Bi), C57BL and TO mice. The neonatally thymectomized NZB mice, in common with those of other strains, showed lethal wasting, a lymphocyte deficit in their lymphoid organs and blood, their packed cell volume was reduced and some had liver lesions associated with the hepatotrophic virus MHV-1. As in C3H/Bi and hybrid mice, thymectomy had little effect on the levels of i

Thymectomy promotes maintained complete stable remission in myasthenia gravis.

AIMS: This study aimed to evaluate the impact of thymectomy on the long-term prognosis of myasthenia gravis (MG) patients, using maintainable complete stable remission (mCSR) as the primary outcome. METHODS: We performed a retrospective analysis of prospectively collected data from the MG database at West China Hospital. mCSR was defined as the cessation of all MG-related treatments with symptom-free status for at least 1 year until the last follow-up. Kaplan-Meier curves and multivariable Cox proportional hazards model were used to assess thymectomy's effect on mCSR. RESULTS: A total of 421

Initial results of uniportal and robot-assisted subxiphoid thymectomy.

BACKGROUND: We previously reported on subxiphoid uniportal thymectomy (SUT) and subxiphoid robotic thymectomy (SRT). This descriptive study aimed to evaluate the feasibility and safety of both SUT and SRT techniques. METHODS: Between March 2011 and December 2022, 268 patients underwent subxiphoid thymectomy. In cases demonstrating no evidence of invasion into other organs, SUT was selected due to its minimal invasiveness. In cases where the tumor was in contact with the innominate vein or those with suspected invasion into other organs, SRT with additional intercostal ports was selected due t

Robotic radical thymectomy-right-sided approach for a thymoma.

The utilization of a radical robotic thymectomy, with clearance of the mediastinal tissue between the two phrenic nerves for a thymoma, is well established. The complex mediastinal anatomy, limited space and the location of the heart introduce a distinctive challenge, particularly when adopting a right-sided approach to identify and dissect along the left phrenic nerve. We present our technique for performing a robotic radical thymectomy from a right-sided approach, tailored for a localized thymoma. Our goal is to showcase the efficacy and safety of this method, offering insights that can enha
