Tilt testing with combined lower body negative pressure: a "gold standard" for measuring orthostatic tolerance.

Orthostatic tolerance (OT) refers to the ability to maintain cardiovascular stability when upright, against the hydrostatic effects of gravity, and hence to maintain cerebral perfusion and prevent syncope (fainting). Various techniques are available to assess OT and the effects of gravitational stress upon the circulation, typically by reproducing a presyncopal event (near-fainting episode) in a controlled laboratory environment. The time and/or degree of stress required to provoke this response provides the measure of OT. Any technique used to determine OT should: enable distinction between p

Episodic syncope in hypertrophic cardiomyopathy: evidence for inappropriate vasodilation.

Symptoms of impaired consciousness (syncope and pre-syncope) occur in 15-25% of patients with hypertrophic cardiomyopathy (HCM).1 In young patients a history of recurrent syncope is associated with an increased risk of sudden death.2(-)5 Syncope usually occurs without warning or symptoms suggestive of the cause. Detailed investigations identify a probable mechanism in a minority, usually paroxysmal atrial fibrillation or ventricular tachycardia. In the majority however no likely mechanism is found despite repeated 24-hour ambulatory echocardiography (ECG) or patient-activated monitoring, exerc

Upright tilt test: correlation between results and patient clinical features.

The aim of our study was to analyze how the clinical history and the main clinical characteristics of patients suffering from loss of consciousness may influence the results of the upright tilt test. A series of 745 patients (333 males, 412 females; mean age 44 +/- 18 years) with recurrent episodes of syncope or presyncope underwent complete clinical and noninvasive laboratory examination, including vagal maneuvers and upright tilt test (60 degrees for 45 min). Cardiological and neurological findings were normal in every case. Upright tilt test was positive in 462 patients (62%). Patients with

The effects of orthostasis on the ventricular-evoked response.

AIMS: The effects of orthostasis on the morphology of the ventricular-evoked response were investigated. METHODS AND RESULTS: Seven patients (five females; 74.9 +/- 4.6 years) with second- or third-degree atrioventricular block received DDD pacemakers Physios CTM 01 with fractal-coated ventricular leads (both Biotronik, Germany). At their 3-month follow-up sessions, these patients underwent tilt-table tests. Heart rate, mean arterial blood pressure and ventricular-evoked response were measured continuously. Signals were averaged over 15 consecutive beats. Coefficients of correlation between s

Autonomic syncope.

The term "autonomic syncope" has recently been introduced in the literature to encompass non-cardiac causes of syncope in which the autonomic nervous system plays a key role in its pathophysiology. The phenotypic classification into bradycardic, hypotensive, and mixed types has enabled a more precise approach to diagnosis and treatment. This narrative review will present the latest updates with a practical focus, emphasizing the importance of diagnostic tools such as ambulatory blood pressure monitoring and the tilt table test. Additionally, advances in personalized treatments will be discusse

Clinical Reasoning: A 66-Year-Old Man With Chronic Orthostatic Hypotension.

A 66-year-old man presented with a 9-year history of chronic orthostatic intolerance and recurrent syncope. A review of systems also revealed several other symptoms including dry eyes and mouth, chronic nausea and bloating, constipation, urinary urgency and urge incontinence, and erectile dysfunction. He had no motor, sensory, or cognitive concerns. He took no medications that could cause autonomic dysfunction. His neurologic examination was unremarkable. The tilt table test revealed neurogenic orthostatic hypotension. His plasma norepinephrine level was low. Eventually, a treatable etiology o

Adult vasovagal syncope with abdominal pain diagnosed by head-up tilt combined with transcranial doppler: a preliminary study.

BACKGROUND: Syncope is a common condition that increases the risk of injury and reduces the quality of life. Abdominal pain as a precursor to vasovagal syncope (VVS) in adults is rarely reported and is often misdiagnosed.. METHODS: We present three adult patients with VVS and presyncopal abdominal pain diagnosed by synchronous multimodal detection (transcranial Doppler [TCD] with head-up tilt [HUT]) and discuss the relevant literature. RESULTS: Case 1: A 52-year-old man presented with recurrent decreased consciousness preceded by six months of abdominal pain. Physical examinations were unre

Cerebrovascular regulation in patients with vasovagal syncope and autonomic failure due to familial amyloidotic polyneuropathy.

INTRODUCTION: While there is strong evidence for autonomic involvement in cerebrovascular function acutely, long-term role of autonomic nervous system in cerebrovascular function has been controversial. We assessed autoregulation in 10 healthy individuals, nine patients with vasovagal syncope (VVS), and nine with Familial Amyloidotic Polyneuropathy (FAP), in response to head-up tilt test (HUTT). METHODS: Arterial blood pressure heart rate, cardiac output, and bilateral cerebral blood flow velocity (CBFV) at the M1 segment of middle cerebral artery (transcranial Doppler ultrasound) were record

The Cardiac Autonomic Response Recovery to the Modified Tilt Test in Children Post Moderate-Severe Traumatic Brain Injury.

OBJECTIVE: To assess the recovery of the cardiac autonomic control system (CACS) response to the modified tilt-test during rehabilitation, in children post moderate-severe TBI at the subacute phase post-injury. METHOD: Thirty-seven children aged 6-18 years, 14-162 days post moderate-severe TBI, participated in the study. The assessment included CACS values evaluation (heart rate (HR), heart rate variability (HRV) and blood pressure) during the modified tilt-test: five minutes lying supine and five minutes passive standing. Re-assessment was performed after eight weeks of rehabilitation. RESU

Comparison of trinitroglycerin and adenosine as provocative agents for head-up tilt test in patients with unexplained syncope: a semi-crossover randomized clinical trial with prospective follow-up.

PURPOSE: Head-up tilt test (HUTT) is a reasonable diagnostic evaluation for patients with suspected vasovagal syncope; however, its lengthy duration is a remarkable limitation. Although adenosine (AD), as an alternative provocative agent, is a promising option for tackling this shortcoming, it received little appreciation in the literature. We aimed to compare the efficacy and the time to elicit a positive response to HUTT for sublingual trinitroglycerin (TNG) and intravenous AD. Furthermore, we evaluated patients' outcomes in the follow-up. METHODS: Patients with a chief complaint of transie

Pathophysiology of Noncardiac Syncope in Athletes.

The most frequent cause of syncope in young athletes is noncardiac etiology. The mechanism of noncardiac syncope (NCS) in young athletes is neurally-mediated (reflex). NCS in athletes usually occurs either as orthostasis-induced, due to a gravity-mediated reduced venous return to the heart, or in the context of exercise. Exercise-related NCS typically occurs after the cessation of an exercise bout, while syncope occurring during exercise is highly indicative of the existence of a cardiac disorder. Postexercise NCS appears to result from hypotension due to impaired postexercise vasoconstriction

[Postural tachycardia syndrome].

Erect posture could be a source of diverse chronic symptoms usually referred as orthostasis intolerance. Increasing number of patients suffering from orthostatic syndromes is recognized. Characterization and distinction of these orthostatic syndromes are based on the clinical presentation; including the typical history of orthostasis intolerance, and the unique hemodynamic responses on assuming upright posture. The diagnosis of postural tachycardia syndrome (POTS) requires orthostatic heart rate acceleration in excess of 120 beat/min, or an absolute increase of > or = 30 beat/min, in the absen

Lack of association of heart rate variability parameters with head-up tilt-test responses in patients with syncope.

BACKGROUND: Neurocardiogenic syncope is the most common type of syncope. Head-up tilt testing is the investigation of choice for diagnosis of patients with neurocardiogenic syncope. In this study, we aimed to findout any association between heart rate variability parameters and type of tilt-test response in patients with syncope. METHODS AND RESULTS: Forty-nine cases with unexplained syncopal attacks were enrolled into our study and were grouped according to the tilt-test responses. Tilt test was performed in all patients after excluding other causes of syncope. In case of a negative basal ti

Syncope in the elderly.

Syncope is defined as a self-limited loss of consciousness, usually combined with falling due to the inability to maintain postural tone. The underlying mechanism is a transient global cerebral hypoperfusion. The aetiology essentially includes cardiac disorders (structured heart disease or arrhythmias), neurally-mediated reflex syndromes, orthostatic hypotension and carotid sinus syndrome. History and physical examination will lead to the diagnosis in up to 50%. The most important step is to differentiate patients with heart disease from others, since the mortality of these patients is doubled

Preliminary observations on the use of midodrine hydrochloride in the treatment of refractory neurocardiogenic syncope.

Recurrent episodes of neurocardiogenic syncope that occur without warning are a common cause of recurrent syncope that can be identified during head upright tilt table testing. While the use of beta blockers, theophyllines, fludrocortisone, disopyramide, and serotonin reuptake inhibitors can be useful in the prevention of episodes, some patients are either unresponsive to or poorly tolerant of these agents. We investigated the use of the peripheral alpha stimulating agent midodrine in preventing both tilt-induced and spontaneous neurocardiogenic syncope. Twenty-five patients (16 women, 9 men,

Syncope recurrence can better be predicted by history than by head-up tilt testing in untreated patients with suspected neurally mediated syncope.

BACKGROUND: Head-up tilt testing is widely used in the evaluation of patients with suspected neurally mediated syncope. Although it remains unclear which patients require medical therapy to prevent recurrent syncope, most centres initiate empiric medical therapy in all patients in whom neurally mediated syncope has been diagnosed. To determine the natural history of this condition, we followed 80 untreated patients. METHODS: All 80 study patients fulfilled the following inclusion criteria: (1) > or = 1 syncope in the upright position, (2) absence of structural heart disease, (3) unremarkable

Tissue oximetry for the diagnosis of neurally mediated syncope.

This study examined the potential clinical contribution of noninvasive brain and skeletal muscle oximetry as a diagnostic aid in neurally mediated syncope. Tilt table testing was performed in 15 patients with a history of syncope and 9 healthy volunteers. Spatially resolved reflectance near-infrared spectroscopy was used to examine regional oxyhemoglobin saturation in the frontal cerebral cortex and pectoral muscle. During upright tilt, syncope occurred in four patients. Each episode was associated with bradycardia, hypotension, and brain oxygen desaturation of > 20% from the supine reference
