Interfragmentary screw fixation of the zygomatic arch in complex midface and zygomaticomaxillary fractures.

Zygomatic arch reduction and fixation is a key point in the treatment of complex midface and zygomaticomaxillary fractures. High-impact frontal trauma can cause posterior displacement of zygomatic bone, with a sagittal fracture of the root of the zygomatic arch extending posteriorly to the glenoid fossa. Miniplate and screw fixation of this fracture requires a large detachment of soft tissue, thus being technically more difficult for proper fixation and increasing the risk of soft tissue damage. This report describes an operative approach for fixation of this type of fracture using an adaptati

[Supportive composite-hybrid fixation of percutaneous screw fixation of tibial head fractures].

Recent operative techniques with percutaneous screw fixation of the tibial plateau require a high level of patient compliance. Geriatric, non-cooperative patients and fractures with severe soft tissue injury have had to be excluded so far from this therapeutic regimen. Since September 1993, composite hybrid fixation, as a combination of ring fixation of the epimetaphyseal tibia with monolateral AO fixation of the tibial shaft, has been performed in 12 patients. The data were collected prospectively. Fractures were classified according to the AO and Moore classification; soft tissue damage was

Staged treatment using external fixation for comminuted fracture of first metatarsal: a case report.

In high-energy midfoot fractures with severe soft-tissue damage, immediate osteosynthesis is often challenging owing to the lack of subcutaneous tissue and forefoot swelling. We herein report a case of comminuted first-metatarsal fracture with marked shortening and poor skin condition, managed using a staged approach with external fixation. A 52-year-old man presented with left forefoot pain after a tiller-related mishap. Computed tomography revealed a comminuted first-metatarsal fracture with shortening. Owing to poor condition of the skin, a staged treatment was adopted. In the first stage,

Editorial Commentary: Open Distal Tibial Allograft With Screw Fixation for Distal Tibial Allograft Glenoid Reconstruction in Patients With Shoulder Instability May Result in Lower Recurrence Rates Than Button Fixation.

The glenoid rim is altered in up to 90% of shoulders with recurrent instability. Distal tibial allograft, an option for anatomic glenoid reconstruction (AGR), has excellent midterm outcomes and low recurrence and resorption rates using open or arthroscopic technique. Optimal graft position in both the medial-lateral and vertical axes, adequate fixation, and restoration of soft-tissue balance is the key to successful outcomes. Regarding fixation methods, metal screw fixation is a standard for AGR with free bone block or Latarjet procedure. The main disadvantage of screw fixation is hardware pro

Application of a novel suture anchor to abdominal wall closure.

BACKGROUND: Mesh suture used in high-tension wound closures produces large knots susceptible to increased palpability, infection, and foreign body response; yet has superior tensile strength and increased resistance to cutting through tissue compared to standard suture. This study investigates mesh suture fixation in abdominal tissue with a knotless novel, low-profile anchor-clip. METHODS: Single and double end fixation of mesh suture in swine rectus abdominus fascia with an anchor-clip, a knot, and predicate device fixation underwent cyclic testing followed by pull-to-failure testing. Rethinking tissue preservation: A review of non-toxic and environmentally sustainable fixatives.

Fixatives are substances used in histopathology to preserve tissue samples by preventing degradation and keeping them structurally intact for microscopic analysis. The common examples include formaldehyde, glutaraldehyde, and alcohols. The widely used fixative, formaldehyde, carries significant health hazards, like respiratory distress, mucosal irritation and carcinogenic effects. Consequently, there is a growing academic impetus to explore natural alternatives to traditional chemical fixatives. Studies were obtained from PubMed database using keywords that covered a wide range of fixatives, i

Medial Column Support in Pilon Fractures Using Percutaneous Intramedullary Large Fragment Fixation.

Pilon fractures are complex injuries to the tibial plafond requiring stable fixation in the setting of effective soft tissue management, particularly in high-energy injuries, open fractures, or in geriatric individuals. Medial column support of the distal tibial metaphysis is often an essential component when applying balanced fixation. However, the biologic implications of multiple surgical approaches in the setting of damaged tissue, devitalized bone, or significant bone loss may contribute to increased complications. Percutaneous intramedullary large fragment screws offer both stability and

Total fractures of the tibial pilon.

Complete fractures of the tibial pilon are rare and their treatment difficult. The pathophysiology includes three groups: (A) high-energy trauma (motor vehicle injuries), with severe articular and soft tissue lesions, (B) rotation trauma, (skiing accidents), with modest articular and soft tissue damage, and (C) low-energy trauma in elderly people. These three groups occasion very different problems and complications. In emergency situations, these fractures should be stabilized, most often using external fixation to restore length and prepare definitive fixation. The second stage can be applie

Unlocking the brain: A new method for Western blot protein detection from fixed brain tissue.

BACKGROUND: Aldehyde fixation is a common process used to preserve the complex structure of biological samples ex vivo. This method of fixation relies on the formation of covalent bonds between aldehydes and amines present in the biomolecules of the sample. Aldehyde fixation is routinely performed in histological studies, however fixed tissue samples are rarely used for non-histological purposes as the fixation process is thought to make brain tissue unsuitable for traditional proteomic analyses such as Western blot. Advances in antigen-retrieval procedures have allowed detectable levels of pr

Surgical Fixation of Flail Chest Injuries: Reduction and Fixation Options, Tips and Tricks.

Modern techniques of rib fracture fixation surgery follow the AO principles of fracture reduction, fixation, and appropriate soft tissue handling. Fixation techniques can be performed using anatomic reduction and rigid fixation, or bridge plate fixation for comminuted fractures. Anatomic and nonanatomic plates can be used, although titanium precontoured locking plates are the most commonly used. Other contemporary intra- and extramedullary fixation strategies have also been developed. Intrathoracic fixation is a novel technique, however, limited literature on clinical outcomes currently exists

[Combination of the Ilizarov ring fixator with the unilateral AO tube fixator. Initial clinical experiences with the hybrid system].

Ring fixation of the tibia is difficult because of soft tissue transfixation and the size and weight of the external fixator. To increase the patients comfort K-wire and half-pin fixation are combined in a new hybrid configuration. The Ilizarov ring fixator with K-wire or Schanz screw fixation is combined with the unilateral AO fixator. This configuration has so far been used in 17 cases: in the proximal and intraarticular tibia in 7 cases, in the distal tibia in 8 cases and in the femur in 2 cases. As experience with the hybrid system widens indications accepted are metaphyseal and intraartic

Sirka: An alternative to the formalin fixative.

Formalin fixation is the most essential step of routine histopathology practice. During the last few years, various fixatives have been developed for use in histopathology practice as an alternative to formalin, to overcome its side effects on health. Here we have demonstrated an interesting and novel idea of using sirka or sugar cane vinegar as an alternative to the formalin with the adequate result.

The Immunohistochemical Expression of Programmed Death Ligand 1 (PD-L1) Is Affected by Sample Overfixation.

Humanized antibodies targeting programmed death receptor 1 (PD-1) or its ligand (PD-L1) have been approved for the treatment of different cancers. Some of these antibodies show a correlation between the tissue expression of PD-L1 and response. Evaluation of PD-L1 expression presents multiple challenges, but some preanalytical issues such as tissue fixation have been scarcely evaluated. With the hypothesis that immunohistochemical staining of PD-L1 may be impacted by the time of specimen fixation, we evaluated differences in its expression in tonsil samples exposed to predefined fixation times.

Epitope Lability of Phosphorylated Biomarkers of the DNA Damage Response Pathway Results in Increased Vulnerability to Effects of Delayed or Incomplete Formalin Fixation.

Phosphorylated biomarkers are crucial for our understanding of drug mechanism of action and dose selection during clinical trials, particularly for drugs that target protein kinases, such as DNA-damage-response (DDR) inhibitors. However, tissue fixation conditions needed to preserve DDR-specific phospho-biomarkers have not been previously investigated. Using xenograft tissues and tightly controlled formalin fixation conditions, we assessed how preanalytical factors affect phosphorylated DDR biomarkers pRAD50(Ser635), ɣH2AX(Ser139), pKAP1(Ser824), and non-phosphorylated biomarkers cMYC and ATM.

A novel dual fixation method for improving the reliable assessment of pulmonary vascular morphology in pulmonary hypertension rats.

This study introduced a novel dual fixation method for the pulmonary vasculature and lung tissue in pulmonary hypertension (PH) rats, addressing the limitations of traditional fixation methods that failed to accurately preserve the in vivo status of pulmonary vascular morphology. The modified method involved a dual fixation process, combining individualized ventilation support and vascular perfusion to simulate the respiratory motion, pulmonary artery pressure and right ventricular output of the rat under in vivo conditions. Utilizing a monocrotaline-induced PH rat model, this study compared t

The influence of fixing condition on myoglobin stainability of striated muscle as a tool for forensic diagnosis.

It is reported that immunostaining of Myoglobin (Mb) is useful for forensic diagnosis. In this study, we investigated the condition of fixation of striated muscle in 10 % neutral-buffered formalin to obtain appropriate stationarity of Mb in immunostaining. Firstly, criteria for staining intensity and definition of the stainability of examined were determined for sheep muscle tissue. Sheep myocardial tissue was fixed using 10 % neutral-buffered formalin under the 21 different conditions based on combinations of the following: three ratios of volume of formalin (mL) to weight of myocardium (g) (

Staged management of high-energy proximal tibia fractures.

High-energy proximal tibia fractures are complicated by soft tissue compromise and this may result in sub-optimal outcomes. There is a high association of open injuries, compartment syndromes, and vascular injuries with these bony disruptions. Surgical treatment of these injuries has been associated with significant complications such as infection, knee stiffness, malunion, loss of fixation, soft tissue failure, and amputations. The loss of fixation is an issue especially in the elderly, with failure associated with age more than sixty years, premature weight bearing, preoperative displacement

Horseradish peroxidase retention and washout in blood-brain barrier lesions.

Horseradish peroxidase (HRP) is widely used to visualize blood-brain barrier (BBB) breakdown. In addition to its visual properties, HRP activity can be measured and can, therefore, be used to quantitate barrier leakage rapidly and simply. In addition, the same tissue can be used for electron microscopic studies in which the structural basis for the leakage can be resolved. Here we describe two methods for quantitation of barrier damage using HRP: one in small, multifocal barrier lesions caused by ethanol combined with barbiturates, and a second in large, single-focus, necrotic lesions. We dete
