Dual Impact of Tolvaptan on Intracellular and Extracellular Water in Chronic Kidney Disease Patients with Fluid Retention.

Objective Tolvaptan, an oral selective V2-receptor antagonist, is a water diuretic that ameliorates fluid retention with a lower risk of a worsening renal function than conventional loop diuretics. Although loop diuretics predominantly decrease extracellular water (ECW) compared with intracellular water (ICW), the effect of tolvaptan on fluid distribution remains unclear. We therefore examined how tolvaptan changes ICW and ECW in accordance with the renal function. Methods Six advanced chronic kidney disease patients (stage 4 or 5) with fluid retention were enrolled in this study. Tolvaptan (7

Albuminuria and tolvaptan in autosomal-dominant polycystic kidney disease: results of the TEMPO 3:4 Trial.

BACKGROUND: The TEMPO 3:4 Trial results suggested that tolvaptan had no effect compared with placebo on albuminuria in autosomal-dominant polycystic kidney disease (ADPKD) patients. However, the use of categorical 'albuminuria events' may have resulted in a loss of sensitivity to detect changes. The aim of this study is to investigate the effects of tolvaptan on albuminuria as a continuous variable. METHODS: Post hoc analysis of a 3-year prospective, blinded randomized controlled trial, including 1375 ADPKD patients. Albuminuria was measured in a spot morning urine sample prior to tolvaptan d

Efficacy of tolvaptan in a patient with right-sided heart failure and renal dysfunction refractory to diuretic therapy.

The use of loop diuretics has been shown to deteriorate renal dysfunction and is associated with a poor prognosis in patients with heart failure (HF). Tolvaptan, a vasopressin V2-receptor antagonist, has been reported to be effective in treating HF due to its potent effects of water diuresis and is expected to improve fluid retention without adversely affecting renal function. The present case is a 77-year-old man with pulmonary hypertension associated with chronic pulmonary artery thrombosis and old pulmonary tuberculosis who developed worsening right-sided HF with marked fluid retention and

Hyponatremia in cirrhosis and end-stage liver disease: treatment with the vasopressin V₂-receptor antagonist tolvaptan.

Hyponatremia is common in patients with cirrhosis and portal hypertension, and is characterized by excessive renal retention of water relative to sodium due to reduced solute-free water clearance. The primary cause is increased release of arginine vasopressin. Hyponatremia is associated with increased mortality in cirrhotic patients, those with end-stage liver disease (ESLD) on transplant waiting lists, and, in some studies, posttransplantation patients. Clinical evidence suggests that adding serum sodium to model for ESLD (MELD) scoring identifies patients in greatest need of liver transplant

Short-term effects of low-dose tolvaptan on hemodynamic parameters in patients with chronic heart failure.

BACKGROUND: We evaluated the short-term effects of low-dose tolvaptan treatment on hemodynamic parameters in patients with chronic heart failure (HF). METHODS: We studied 22 patients (69 ± 10 years) with chronic HF and excess fluid retention despite receiving appropriate medical therapy, including loop and/or thiazide diuretics. The therapeutic effects of low-dose (7.5mg) once-daily tolvaptan on hemodynamics associated with changes in fluid balance and neurohumoral activations were investigated after a seven day treatment period. RESULTS: After the treatment period, body weight decreased (-2

Effect of grapefruit juice on the pharmacokinetics of tolvaptan, a non-peptide arginine vasopressin antagonist, in healthy subjects.

PURPOSE: Tolvaptan is a selective vasopressin V2 receptor antagonist that can be given orally once daily for treatment of clinically significant hypervolemic and euvolemic hyponatremia (US and Europe) or extracellular volume expansion despite taking other diuretics (Japan). In vitro studies indicated that tolvaptan was a CYP3A4 substrate. METHODS: A single-center, randomized, crossover trial of 60-mg tolvaptan with 240 mL of water or with 240 mL of reconstituted grapefruit juice (washout period of 72 h between doses) was conducted in 20 healthy subjects. Blood samples for tolvaptan plasma con

Right ventricular enlargement predicts responsiveness to tolvaptan in congestive heart failure patients with reduced ejection fraction.

BACKGROUND: Tolvaptan is a vasopressin type 2 receptor antagonist used in heart failure (HF) with refractory diuretic resistance. However, since tolvaptan is also ineffective in some HF patients with reduced ejection fraction (HFrEF), the identification of responders is important. METHODS: The study population consisted of 51 HFrEF patients who were administered tolvaptan (EF, 28 ± 7%). We defined responders as patients with a ≥50% increase in urine volume during the 24-hours after administration of tolvaptan. All patients underwent comprehensive transthoracic echocardiography before administ

Renal Volume Reduction on Magnetic Resonance Imaging During Tolvaptan Therapy in Patients with ADPKD: A Case Series.

The primary effect of tolvaptan therapy in autosomal dominant polycystic kidney disease (ADPKD) is to slow the rate of kidney volume increase and thereby delay progression to end-stage renal disease; however, an actual reduction in kidney volume has rarely been reported. We evaluated four patients with ADPKD who demonstrated a decrease in kidney volume on magnetic resonance (MR) imaging during tolvaptan treatment. This observational case series included four patients who underwent regular clinical and MR evaluations between 2020 and 2025. The diagnosis was confirmed by radiological findings an

Feasibility and safety of intravenous tolvaptan administration immediately upon ICU admission after cardiac surgery with cardiopulmonary bypass.

PURPOSE: Tolvaptan sodium phosphate (TSP; Samtas®, Otsuka Pharmaceutical, Tokyo, Japan) is a novel intravenous aquaretic diuretic; however, evidence for its use in patients after cardiac surgery is limited. We evaluated the early postoperative administration of TSP in patients who underwent cardiac surgery. METHODS: We enrolled 91 patients who underwent cardiac surgery with cardiopulmonary bypass at our institution. Of these, 46 received TSP immediately after intensive care unit admission, and 45 served as controls. After propensity score matching, 20 matched pairs were analyzed. A subgroup a

Association Between Tolvaptan Exposure and Venous Thromboembolism Risk in ADPKD: A Real-World Propensity-Matched EHR Study.

AIM: Venous thromboembolism (VTE) has increasingly been reported in autosomal dominant polycystic kidney disease (ADPKD), although the mechanisms underlying this association remain incompletely understood. Whether tolvaptan exposure influences VTE risk in ADPKD has not been well studied. METHODS: We performed a retrospective cohort study using the TriNetX Global Collaborative Network, including adults with ADPKD (ICD-10-CM Q61.2) from 2000 to 2025. Patients with inherited thrombophilia or prothrombin G20210A mutation were excluded. Propensity score matching (1:1) was conducted across 24 covar

Mid-Term Impact of Kampo Goreisan in Patients with Congestive Heart Failure Receiving Tolvaptan-Incorporated Medical Therapy.

Background: Despite considerable advances in contemporary pharmacotherapy for heart failure (HF), residual congestion continues to drive adverse outcomes in a substantial proportion of patients. Goreisan, a traditional Kampo herbal formulation, has seen growing clinical application as an adjunct for refractory fluid retention; yet the prognostic implications of sustaining versus withdrawing goreisan among patients on tolvaptan-based regimens have not been adequately characterized. Methods: We conducted a retrospective single-center observational analysis enrolling patients with HF who received

Tolvaptan treatment is associated with altered mineral metabolism parameters and increased bone mineral density in ADPKD patients.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by a unique bone and mineral phenotype. The impact of tolvaptan treatment on mineral metabolism and bone mineral density (BMD) is unknown. METHODS: We conducted an analysis in the Bern ADPKD Registry, a prospective observational cohort study. Mineral metabolism parameters were measured at baseline and every 12 months thereafter. BMD was determined by dual-energy X-ray absorptiometry at baseline and after 3 years. Multivariable mixed-effects regression models were applied to assess changes in mineral metabolism p

Identifying the main predictors of urine output in autosomal-dominant polycystic kidney disease (ADPKD) patients taking tolvaptan.

BACKGROUND: Few works have analyzed factors associated with urine output in ADPKD patients taking tolvaptan (TVP). METHODS: We selected 24-h urine samples from ADPKD patients treated with TVP. Urine osmolality/creatinine ratio was used as estimator of urinary osmolar load. RESULTS: We included 127 urine samples from 61 patients. After TVP, urine output doubled with a parallel reduction in urine solute concentration. However, when expressed as urine solute/creatinine ratios, no significant changes were observed. Daily osmolar load and osmolality/creatinine ratio did not change significantly.

Stereoselective interaction of tolvaptan with amiodarone under racemic metabolic impact by CYP3A5 genotypes in heart failure patients.

PURPOSE: The diuretic effect of tolvaptan is largely blood level-dependent although it does exhibit interindividual differences according to cytochrome P450 (CYP) 3A5 genotype. This study aimed to investigate the pharmacokinetic relationship between plasma tolvaptan and its monohydroxylate enantiomers and the factors affecting their metabolism in heart failure patients. METHODS: Japanese heart failure patients (n = 88) receiving oral tolvaptan (median dosage 7.5 mg/day) were enrolled. Blood samples were collected prior to the dosing on day 6 or later after first administration to determine th

Optimal equation for estimation of glomerular filtration rate in autosomal dominant polycystic kidney disease: influence of tolvaptan.

BACKGROUND: The reliability of various equations for estimating the GFR in ADPKD patients and the influence of tolvaptan on the resulting estimates have not been examined when GFR is calculated on the basis of inulin clearance. METHODS: We obtained baseline and on-tolvaptan measured GFRs (mGFRs), calculated on the basis of inulin clearance, in 114 ADPKD, and these mGFRs were compared with eGFRs calculated according to four basic equations: the MDRD, CKD-EPI, and JSN-CKDI equations and the Cockcroft-Gault formula, as well as the influence of tolvaptan and of inclusion of cystatin C on accuracy
